Approval certificates based solely upon the representations given in paragraph 5 above by an independent professional
engineer registered in the State of Alaska. The DHEP does this as a courtesy to purchasers of homes and their lending
institutions in order to satisfy certain federal and state requirements. Employees of DHEP do not conduct inspections or

Dear Mr. Merrell:
Your request for a waiver of the required well-to-septic tank separation
has been granted. Waivers of 60 and 95 feet for two separate septic tanks
on the lot have been granted.
It should be noted that future approvals of this septic system may be
contingent upon an inspection of both tanks to insure that they are in
satisfactory condition.
Any modifications or enlargement of the existng septic system will invalidate
this waiver.

Dear Mr. Meyer:
This department cannot approve your request for a waiver of the required
100 foot separation between the well and sept~ ~nk~ ~n the subject lot.
The soils test on file indicates a shallow water ~able?
In order to approve a waiver for the subject lot it will be necessary to
verify the integrity of the tanks in question, including all inlet and
outlet connections. The slope of the shallow water table in the vicinity
of the tanks should be determined if possible. If ground water is not
encountered, it will be sufficient to verify that the soil is unsaturated
to a depth of four feet below the tanks.
This waiver will be reconsidered in light of the additional information
provided by the investigation.
Please provide a roughly scaled site plan in all waiver request submittals.
In some cases, such as this one, a profile drawing including the location
and depth of potential obstructions (the house foundation) and the slope of
terrain and water table (if known) can be very useful in a waiver evaluation.

Variance Request
North 284' of Lot 25, T12N, R3W, S22
Dear John:
We have completed the testing and application preparation
regarding acquiring a Health Authority Approval for the
above-referenced lot and have identified several deficiencies.
A well log was not available so we visually verified well casing
depth. We have directed the owner to provide a well casing 12"
above surrounding ground, and he has complied.
The only other remaining deficiency identified is that the
septic tanks are closer than 100 feet from the well. The well
was drilled in 1966, and the septic tanks were installed in
1983. The on-site sewer inspection report was approved by the
Municipality of Anchorage showing the tanks' installation and
indicating a 98' separation. This is not the case; the actual
separations are 60' and 95'.
We request a variance to the requirement that the tanks be 100
feet from the well for the following reasons:
The system has functioned with no adverse health impacts.
satisfactory water sample test result was obtained. The
well is cased to greater than 40 feet.

3. The system was approved by MOA in 1983.

The first crib is 10-15 years old, and the second crib
(last in line) is 24 years old. ~
DATE OF TEST: April 25, 1983
TEST PROCEDURE:
House water was used normally the morning of the test.
Approximately 2½ hours of no water use occurred in~nediately
prior to t~sting. Water was added to the second crib through
a 4" standpipe in 100 gallon increments. The first 100
gallon input was added at an approximate rate of 100 gpm.

RESULTS:
Backing up into the first crib was noted. The second
100 gallon input was dumped at an approximate rate of
10 gpm. After 140 total gallons had been dumped into
the crib, it was noted by the tester that the first crib
was nearly overflowing, and that water was seeping to the
ground surface out of the second crib. Addition of water
was then stopped. The first crib was pumped, and water
level drop in the second crib was monitored. The level
dropped less than ½" in 30 minutes, which corresponds to
an estimated absorption rate of 300 gallons per day.
The system became full after addition of 140 gallons of
water, causing water to seep out at the ground surface.
It appeared as if the system had overflowed previously.
Absorption capacity results were inconclusive, but were
estimated by the tester at 300 gpd. The system fails to
meet MOA requirements.
COMMENTS:
Upgrading Of the system will be necessary to com~ly with
M0~lreme~'~s. A ~Q00 gallon or larger septic tank will
~ee~ to be installe~, and the soil absorption Unit will
~eed repl~cem~nt. ~
