Waivers Your request is to alter the existing waiver between your Class B public water system and the two compartment septic tank and deep trench soil absorption system as follows: • To lessen the separation distance between the Class B public water system and the two compartment septic tank to a distance of 115 feet. The existing waiver is for 120 feet. • 1 a lessen the separation distance between the Class B pubic water sy stem and the 53 foot' - long, 6 foot deep trench type system to a distance of 140 feet. A review of the well logs, soils reports, current water samples, and the plan drawing has taken place. It appears the waiver requests are reasonable. Therefore, in accordance with the State Drinking Water Regulations (I8 AAC 80.020), the following waivers are granted. • The separation distance between the Class B public water system well located on Lot 14, Section 21, T 12N, R 3 W, SM and the two compartment septic tank serving the Church on this same lot has been waived from 200 feet to 115 feet. • The separation distance between the Class B public water system well located on Lot 14, Section 21, T 12 N, R 3W, SM and the deep trench type wastewater disposal system on this same lot has been waived from 200 feet to 140 feet. The distances noted in the waivers are from your submittal and the file record. It appears that these distances are from the nearest edge of the wastewater disposal system component to the nearest edge of the existing Class B public water system well. If any wastewater disposal system fails or contamination can be traced back to the system, these two waivers will be invalid. Wastewater System It appear that the wastewater disposal system was designed in accordance with State Wastewater Disposal Regulations and Guidance. Therefore, in accordance with State Wastewater Disposal Regulations (18 AAC 72.225), construction approval for the system is granted. The signed construction approval portion of the Construction and Operation Certificate, constituting this approval, is enclosed. To obtain final operational approval, the following items will need to be submitted for review and approval by this office for this system: I. The wastewater system was restricted in use to 600 gallons per day maximum wastewater loading. This restriction is continued. The results of the on site evaluation of the drain field (adequacy testing) after your authorized modifications to the on site wastewater disposal system can be a basis to determine if this restriction on use is still required. 2. Record drawings will need to be submitted to this office under the stamp of a Professional Engineer. The Record drawings will need to verify that the construction was per the approved construction contract and changes were approved by this office. Please remember that the inspection of the project on which the record drawings are based upon is the responsibility of the Professional Engineer sealing the record drawings. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. If construction has not begun with two years after issuance of plan approval, the approval is void, and plans must be submitted to the Department for review and approval. -All future correspondence should include the above project number. Based on this review, I have the following comments. Waiver A review of the record drawings indicates that the septic tank was placed at a scaled distance of 107 feet from the existing source well instead of the 115 feet that had been approved. A review of the well logs, soil reports, the water samples on file, and the record drawings has taken place. It appears the lessened distance is reasonable. Therefore, in accordance with the State Drinking Water Regulations (18 AAC 80.020), the following waivers are granted. • The separation distance between the Class B public water system well on Lot 14, Section 21, T 12 N, R 3 W, SM and the two compartment septic tank serving the Church on this same lot has been waived from 200 feet to 107. • The separation distance between the Class B public water system well located on lot 14, Section 21, T 12 N, R 3 W, SM and the deep trench type wastewater disposal system on this same lot has been waived from 200 feet to 140 feet. If any wastewater disposal system fails or contamination can be traced back to this system, these two waivers will be invalid. Wastewater System It appear that the wastewater disposal system was constructed in accordance with State Wastewater Disposal Regulations and Guidance. Therefore, in accordance with State Wastewater Disposal Regulations (18 AAC 72.240), final operational approval for the system is granted. The signed final operational approval portion of the Construction and Operation Certificate, constituting final operational approval, is enclosed. A restriction of 600 gallons per day maximum wastewater loading was placed on the system. Since the department did not receive results of an on-site evaluation (adequacy testing) of the drainfield as requested, the above noted loading restriction is continued. If your client would like to have this loading restriction removed, I recommend that, you schedule a meeting to discuss what steps will need to be taken. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations.
