This system was installed under the terms and conditions of Permit No. SW #930343 dated Sept, 24, 1993. Under item number 3 on the above referenced permit, and AMC 15.65.150.A, it is stipulated that: "the engineer must notify DHHS at least two hours prior to each inspection". When this office receives notice of either the fkst or second such inspection, a notation is then irrunediately made on our copy of the permit so that we have the opportunity to witness either or both inspections. This office has no record of receiving any notice of inspection on this installation. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. [f you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If the on-site wastewater system has been completed and a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit; $120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit. Hansen 14840 Loc Loman Lane Anchorage, Alaska 99516 Lot 2 of BLM Lot 133 PID #018-281-20 Our Job 93-247.01 Dear Mr, Robinson: Pursuant to our telephone discussions on Monday August 30, 1993, we have completed the design for the septic system upgrade described in your letter of August 2, 1993 addressed to Mr. and Mrs. Hansen. A copy of this letter was faxed to our office on August 30, 1993. The septic system upgrade design for this two bedroom residence is attached to this letter and is submitted to your office for approval so that the home owners may proceed with construction. You will note that the existing well shown on the site plan is also in violation with the Municipality of Anchorage Ordinance 18.AAC.72 for the separation distance between the water supply well and septic system. In this regard, I have discussed the problem with Mr. Hansen and have recommended that he drill a new well. We understand that the existing well is approximately 60 feet deep and would be located approximately 55 feet from the upgrade (located 65 feet from the existing cess pool). This recommendation is based on the cost of a new well with the existing depth to water, the MOA permit review cost, and the cost of engineering in preparing a waiver request. The separation distance is well below the required 100 feet and it is unlikely that a waiver would be approved, even though the permit fee and engineering costs would still have to be incurred. We look forward to a favorable review and approval of this septic system upgrade design.
