If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. Ail inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

8. EXCAVATE THE ABSORPTION TRENCH. BO~FOM & SIDES OF EXCAVATION SHALL BE LEVEL AND SCARIFIED.
RECORD ELEVATIONS AT BEGINNING, MIDDLE, AND END OF TRENCH BOTTOM, CONSTRUCTION EQUIPMENT
SHALL NOT OPERATE ON THE FLOOR OF THE EXCAVATION. ANY MATERIAL COMPACTED BY THE OPERATION
OF CONSTRUCTION EQUIPMENT SHALL BE REMOVED AND REPLACED WITH UNCOMPACTED MATERIAL. SLOPE OR

RE: Septic System Design Approval / Parcel No. 1 T13N R4W Section 25
Dear Jim,
Enclosed are the revised design drawings for the above referenced project. With regard to
our telephone conversation, I have redesigned the system for 600 gallons per day (4 bedroom
single family residence). I have split the system into two leach fields. Therefore, I have
assumed 200 gallons per day to each leach field.
Based on our previous soils investigation, I believe the soils on the lot to be very uniform.
Therefore, I am requesting that we receive design approval to construct contigent upon the
engineer digging another test hole and conducting another percolation test to verify the soils
at the location proposed for the westerly leach field.
Should any thing change from the design as approved, we will contact you for approval prior
to proceeding with actual construction.
