Septic System Upgrade Permit Request
Ladies and Gentlemen:
I am writing to request a permit to upgrade the septic system be issued for this property. The proposed
systems will serve an existing three (3) bedroom house. Currently the lot is developed. The existing trench
and crib are in failure and will be re -used in tandem as a reserve. This lot is served by a private well that
is over 100 feet from the existing and proposed septic system. The surrounding lots are also served by
private wells. There are no wells within 100 feet of this system.
1. Soils. One test hole was excavated by Pannone Engineering Services in February of 2013, and the
ground water monitored for seven days. Groundwater was not observed in the hole monitor tubes after
the seven day monitoring period. Bedrock was not encountered in the test hole. It is my opinion, based
on the results of the percolation tests and overall soils appearance in TH-1; an application rate of 0.45
gallons/day/square feet should be used for a conventional wastewater system. A diverter valve will be
installed and the existing drain field will be re -used as a reserve.
2. Soil Absorption System Design.
a. See Sheet 2 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The topography is flat in the area of the proposed drain field. There are no steep
slopes in the area.
S. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site

The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

DESIGN CRITERIA

On November 8, 1989 a permit was issued for the replacement of the septic system on the
above referenced lot. The homeowner soon thereafter discovered that the failed/failing
system was due in part to a plumbing leak that was fixed, and the problems were at least
temporarily resolved without replacing the septic system.
An adequacy test of the system this month revealed that the existing crib is in failure. This
permit application is essentially to renew the old permit, making modifications where
necessary due to changes in the MOA ordinances.
The home on the lot is a single family, 2 bedroom residence. The test hole that was
excavated on 10/12/89 had a percolation rate of 25 min/inch, which translates into an
application rate for deep trenches of .6 GPD/SF. With a 300 GPD design flow, the area
required would be 300/.6, or 500 SF. The monitoring tube from the 1989 hole was dry at
the end of the monitoring period, but the tube has since been removed. A new tube was
installed to a depth of 13 FT on 10/15/93. This tube was dry as of 10/18/93. Using a
gravel thickness of 5 FT,`the required length would be 500/10, or X40 FT. This will also
require a 1" thick layer of extruded polystyrene insulation in addition to 2 FT of cover
material. Total depth of the gravel is not to exceed 7 FT
The topography in the area of the replacement system is nearly level.

Ladies and Gentlemen
I am writing to request a conditional COSA be issued for this property to accommodate its sale in the near future. A
permit was issued to construct an upgraded septic system, however due to time constraints the system cannot be
installed before the sale. Residential Mortgage LLC has stated that they will place 2X the highest bit into escrow once
the Conditional COSA is approved for construction.
1. COSA. The Owner is in the process of selling this property. The existing system was located and inspected.
The system was tested for operation and was found that it could not accept the required 150 gallons per bedroom per
day (450 gpd). The system is able to absorb approximately 200 gallons per day. The intent is to install an upgraded
septic system in the spring of 2013 by July 31, 2013. This system will not affect the health or welfare of anyone on this
lot or the surrounding lots.
The proposed installation will not affect the future development of the surrounding or existing lots. There are no wells
or septic systems within 100 feet of the proposed septic location.
