Tank and Drain Field Upgrade Request
Design Narrative
This is a design narrative for a permit to install a new septic system to be issued for this property. The
proposed system will serve one (1) bedroom of an existing four (4) bedroom house. Currently the lot is
developed. The proposed system will utilize a 1000.9 septic tank with a conventional drain field. The
surrounding developed lots are served by private wells. There are no wells on surrounding lots within 100
feet of this system and there are no septic systems installed on the surrounding lots within 100 feet of the
existing well.
1. Soils. One test hole was performed in the vicinity of this system by Pannone Engirreering Services in
July of 2015, and groundwater was monitored for at least seven days. Ground water was not observed
to a depth of 13.0' below the surface in the test hole monitor tube in April of 2015. Bedrock was not
encountered in the test hole. An application rate of 1 gallons/day/square feet with a 2.0' sand filter
should was used for a conventional wastewater system in the area of these test holes.
2. Soil Absorption System Design:
a. See Sheet 2 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field.
The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The existing topography slopes from the southeast to the northwest at
approximately 10% in the area of the septic system. There are no steep slopes within 50' of the proposed
drain field. The proposed drain field will maintain 50' separation all steep slopes.
S. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that
the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be _removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no surrounding wells within 100 feet of the proposed septic location or septics within 100 feet of the
proposed well location.

The Grantors, in consideration of $10.00 herein acknow-
ledged, and for other good and valuable consideration, do
hereby convey to themselves, Grantee and Emma Rae Strosin, for
the benefit and enjoyment of Lot No. 2 a determinable easement
to use, to withdraw, to transfer, and to maintain that
certain water and water well and all appurtenances thereof
from a certain well located on the aforementioned Lot No. 2.
Grantee and Emma Rae Strosin are granted the right of egress
and ingress over and upon Lot No. 2 for the purposes of
acquiring the aforementioned water or to repair the water
pipes constituting the water system, or facilities pertinent
to said water system, if such is necessary, but egress and
ingress shall extend only to those reasonable amounts of
land necessary for such purposes as herein described.
Grantee and Emma Rae Strosin shall have the right to
total reasonable use of the water well and all appurtenances
thereof on Lot No. 2.
This easement shall extend in perpetuity for all intent
and purposes and to the extent of the declared uses described
herein, and shall be binding and in full force and effect
and shall run with the land upon all heirs, succesers and
assigns.
Beneficiaries herein acknowledge and concur in the
agreement of the easement to the extent of Beneficiaries'
interest as the beneficiary under the above described Deed
of Trust, and agrees to abide by and be bound by the declaration
of easement described herein to the extent of any rights
which Beneficiaries may now or hereinafter possess on the
property as holders of the prior dead of trust.

This letter is to explain the existence of two septic systems
that are located on my lot. The original septic system was installed
in 1976 and was inspected by the Municipal Inspector. The second
system was installed in order to serve the basement elevation rather
then lowering the original system. This septic was installed in 1978
and was inspected by the Municipality Inspector. The home is not
intended to be used as a multi -family dwelling.
The distance to the well of 85' and 95' for each septic tank was
approved by the City Inspector.

In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

On behalf of our client we are applying for detemvnation of waiver of separation distance for the
above referenced single family property. The requested waivers are for separation distances of 83',
82', and 92' from the private well to a private leachfield trench and two private septic tanks. A site
plan is attached showing the distances from the well to the above mentioned items.
The well log for the subject property and for nearby properties are attached. The subject well was
drilled in 1975 to a depth of 104'. The original log shows sand and gravel to a depth of 45 feet, where
a 10' layer of blue clay is encountered. A layer of silt then is found from 55 to 75 feet, with a 29'
layer of sand and gravel to the bottom of the well. The water is found in the lower layer of sand and
gravel, below both the blue clay and silt layers. This shows a likely isolation of the aquifer of this
well from any surface water of the area. Surrounding well logs are consistent with the subject
property, showing layers of clay, silt and high silt content soils, and hardpan on immediately adjacent
lots. This supports a protective barrier to the aquifer that extends beyond the bounds of the subject
property.
The soil layers near the surface are also consistent with the surrounding lots, showing gravel and sand
as the predominant soil for the area. The attached soil logs confirm that the above is true, and support
the consistency of the soils for the entire area surrounding subject lot. The ground slopes to the
Northwest at over 75 percent on average, and directs surface drainage away from both the well and
septic systems. The house is located between the well and septic, creating a barrier for surface water
flow.

There are two septic systems serving one house on this lot. For the trench septic system on the Iot, the
on-site sewage disposal inspection report was dated September 21, 1978. The inspection ,was
performed by a Municipal Inspector and the recorded distance from the well to the drainfield was 107
feet, the distance from the well to the tank was 92 feet. The seepage pit septic system was installed in
1976 and was reportedly inspected by a municipal inspector also. This is explained in an attached
letter written by the property owner. There is no original paperwork on file for the 1976_ system,
which was asbuilted by our firm in 1987 to show a 1000 gallon tank and unknown capacity leach pit.
This asbuilt documentation was accepted by the MOA for subdivision approval in 1987 and it was
indicated in recent meetings with MOA managers that this documentation would suffice for a current
HAA. Current water samples show zero coliform and a very low nitrate level of 0.95 mg/l.. Both
septic tanks have been pumped.
Based the length of time that the well and septic have been in place, the results of the water samples,
and the small distance being waived, we do not anticipate a significant negative impact on the water
supply by allowing this waiver. Please review the attached file research data and if you have any
