Drain pipe between the house and STEP tank and from the filter box to residual drainfield was ASTM 3034. Residual drainfield pipe was also ASTM 3034. 2. Pump: OSI - 209SC1F-02; 1/2 HP 110 Volt, 20 gpm, single phase. 3. The Electrical Inspection and wiring of controls was by Alcan Electric. 4. The timer was set to recirculate every 30 minutes as per instructions with the controls in the Orenco Kit. 5. A note was placed on the Plan View showing the Air Difffuser pump is located in the crawl space of the house. The 3/4 " Diameter airline was also shown running from the house to the filter box. A drawing was also added showing a plan view of the air diffuser 3/4" pipe was coiled in the boss. A cross section indicatinbng how the pipe was insulated between the house & the box was also shown. I have made the changes as contained in your October 15 th transmittal and as we discussed on Oct. 25th. The following comments explain what was done with or clarify your Oct. 15th comments. 1.2 Since much of the fill that Mr. Lyons hauled onto the lot containe6 so much silts and fine material,d I selected a location for the residual disposa]_ field and had PTr. Lyons dig a trench thru the fill material to the organic "peat" layer heneath the fill. The trench was filled with a 3/4 inch minus washed gravel. 2.1 The STEP Tank was change to 1500 gallons. 2.4 I intend to confir with you and Orenco as to whether a timer or floats set to dose a preset amount are the best. 3.2 Concur with the comment. Will construct the box 19'-4" by 19'-4" with insulation on the inside. 3.3 Will use Orenco sampler. 3.5 Have specified source, sampling at site. 3.6 Orenco piping will be used. I believe that all your comments have been incorporated into the specifications. I again want emphasize that the intent is to have Por. Lyons order a kit from Orenco, or Anchorag Tank, that contains everything necessary to construct a 19' x 19' intermittent sand filter modified for Alaska conditions. Box 196650 Anchorage, Ak. 99510 Reference L3B6 Diann Subd. Addn. #2 Dear P•1r. Smith: I am writing to request your issuance of a permit to proceed with an innovative disposal system for this lot. Mr Lyons is willing to finance an innovative system in order to occupy the lot. I propose to develope plans for an Orenco Systems Intermittent Sand Filter similar to one of those installed in the MOA Demonstration Projects this year. P -1r. Lyons is aware that a sampling program is required for the first year of operation and will comply with your requirements. I would collect the samples on the schedule required and deliver them to the Lab. Since this lot is located in an area that is zoned for future sewers, I feel that development of this sand filter would be an acceptable solution for an on-site system on this lot. SPECIFICATIONS for Intermittent Dosing Sand Filter System L3B6 Mann Subdivision, Addition No, 2 1.0 Residual Disposal Field: 1.1 A 5 ft. wide x 60 ft. long residual disposal drainfield shall be constructed as shown on the plot plan. 1.2 The level bottom of the residual disposal drainfield shall be placed over the clean gravel plug installed over the organic peat layer. The 4 inch dia. perforated drain pipe shall be buried level in 12 inches of approved sewer rock with the invert of the drain pipe at least 6 inches above the bottom of the sewer gravel. 1.3 The top of the sewer rock shall be covered with approved filter fabric plus 2 inches of rigid, burial type insulation, then backfilled with at least 2 feet of soil cover. 2.0 STEP Tank: 2.1 The septic tank shall be a Municipality approved, 2 - compartment STEP tank with a capacity of 1500 gallons. The second compartment shall be equipped with an Orenco package lift station. Each compartment of the tank shall be equipped with a 4 inch diameter cleanout, and physical access to the second compartment shall be provided by means of a watertight, insulated riser and manhole cover. 2.2 The STEP tank shall be installed in the approximate location shown on the site plan. The tank. shall be insulated with a minimum of 2 inches of approved burial type insulation. 2.3 The 4 inch waste line from the residence shall have a minimum slope of 1/4" per foot. All pipe connections to the tank shall be equipped with waterproof mechanical couplings. 2.4 The on and off floats shall be set at the elevations specified by the supplier. The high level audible and visible alarm shall be installed inside the residence to alert the occupants when the liquid level in the tank rises above the design elevation. 2.5 Municipal Regulations require a valid electrical permit for installation of a lift station pump. It shall be the responsibility of the owner to obtain the necessary permit and Municipal inspection. 3.0 Intermittent Sand Filter: 3.7. Site preparation for the sand filter shall consist of excavation of a 20' x 20 ft. level pad set as deep as possible to allow a gravity discharge into the residual disposal field. The surface of the excavated area shall be leveled by placing and compacting 3 inches of medium sand to achieve a surface leveled within +-/- 1. inch. 3.2 The four foot high sidewalls of the 19'-411x19'-4"sand filter enclosure shall be constructed of 1/2" cdx plywood, with sufficient framing to hold in position during placement of the fillter materials. The walls shall be insulated on the inside with 2" burial type rigid insulation. The space between the plywood and sidewalls of the excavated area shall be filled with sand at the same rate the inside is filled. 3.3 A 30 mil PVC liner, as shown on the drawings shall be placed inside the filter box prior to placement of filter materials. This 30 mil liner should be purchased from "Orenco" A 4" diameter underdrain of slotted ASTM D-30344 pipe shall be laid in the bottom of the filtwer box as shown on the drawings. The 4" diameter discharge line between the sand filter and the residual disposal field shall have a minimum slope of 1/4" per foot and shall be equipped with a "sampling well" purchased from Orenco. 3.4 A 9" thick layer of "Pea Gravel shall be placed in the bottom of the filter liner surrounding the 4" diameterr underdrain. The Pea Gravel shall be a washed material containing no fines ( 0% passing the #100 sieve) and a maximum size of 1/4 inch diameter. 3.5 A 2.8" thick layer of "Concrete Sand"shall be placed on top of the Pea Gravel. The sand used shall be a washed sand and shall meet ASTM C-33 Concrete Sand specifications. It is intended that the builder will purchase this sand from Central Paving Products in Palmer. It is understood that this sand will contain no more than 2 or 3% of material passing the #100 seive, ands that this material will be removed by washing. The permittee will be instructed to have the beds of trucks used for hauling the filter sand washed prior to loading the sand to insure that the sand be kept as clean as possible. A sample will be taken from each load of sand at the job site and this will be sent to a lab for a sieve analysis to insure that sand used in the filter meets these requirements. 3.6 An 8" thick layer of Pea Gravel shall be placed on top of the sand layer. The effluent distribution piping as shown on Orenco Drawing ISF 1919 shall be buried in this pea gravel stratum such that the pipe inverts are no less than 4" above the top of the underlying layer of Sand. The PVC laterals shall have 1/8" diameter orifices drilled in the tops spaced 2' on centers, and shall have Orifice shields installed as shown on the drawings. 3.7 The top of the Pea Gravel stratum shall be flush with the top of the plywood sides of the sand filter enclosure. it shall be covered with Orenco Model LIFF or equivalent filter fabric. Two inches of rigid burial type insulation plus a minimum of 2 feet of soil cover shall then be placed. The 4 inch dia. perforated drain pipe shall be buried level in 12 inches of approved sewer rock with the invert of the drain pipe at least 6 inches above the bottom of the sewer gravel. 1.3 The top of the sewer rock shall be covered with approved filter fabric plus 2 inches of rigid, burial type insulation, then backfilled with at least 2 feet of soil cover. 2.0 STEP Tank: 2.1 The septic tank shall be a Municipality approved, 2 - compartment STEP tank with a capacity of 1250 gallons. The second compartment shall be equipped with an Orenco package 'lift station. Each compartment of the tank shall be equipped with a 4 inch diameter cleanout, and physical access to the second compartment shall be provided by means of a watertight, insulated riser and manhole cover. 2.2 The STEP tank shall be installed in the approximate location shown on the site plan. The tank shall be insulated with a minimum of 2 inches of approved burial type insulation. 2.3 The 4 inch waste line from the residence shall have a minimum slope of 1/4" per foot. All pipe connections to the tank shall be equipped with waterproof mechanical couplings. 2.4 The on and off floats shall be set at the elevations specified by the supplier. The high level audible and visible alarm shall be installed inside the residence to alert the occupants when the liquid level in the tank rises above the design elevation. 2.5 Municipal Regulations require a valid electrical permit for installation of a lift station pump. It shall be the responsibility of the owner to obtain the necessary permit and Municipal inspection. 3.0 Intermittent Sand Filter: 3.1 Site preparation for the sand filter shall consist of excavation of a 20' x 20 ft. level pad set as deep as possible to allow a gravity discharge into the residual disposal field. The surface of the excavated area shall be leveled by placing and compacting 3 inches of medium sand to achieve a surface leveled within +/-- 1 inch. 3.2 The four foot high sidewalls of the 19' s 19' sand filter enclosure shall be constructed of 1/2" cdx plywood, with sufficient framing to hold in position during placement of the fillter materials. The walls shall be insulated on the outside with 2" burial type rigid insulation. The space between the plywood and sidewalls of the excavated area shall be filled with sand at the same rate the inside is filled. 3.3 A 30 mil PVC liner, as shown on the drawings shall be placed inside the filter box prior to placement of filter materials. This 30 mil liner should be purchased from "Orenco" A 4" diameter underdrain of slotted ASTM D-30344 pipe shall be laid in the bottom of the filtwer box as shown on the drawings. The 4" diameter discharge line between the sand filter and the residual disposal field shall have a minimum slope of 1/4" per foot and shall be equipped with a cross fitting ass shown providing a 2 ft. deep capped sump and a 4" riser extending 2 ft. above final grade to allow for subsequent sampling. 3.4 A 9" thick layer of "Pea Gravel shall be placed in the bottom of the filter liner surrounding the 4" diameterr underdrain. The Pea Gravel shall be a washed material containing no fines ( 0% passing the #100 sieve) and a maximum size of 1/4 inch diameter. 3.5 A 28" thick layer of "Concrete Sand"shall be placed on top of the Pea Gravel. The sand used shall be a washed sand and shall meet ASTM C-33 Concrete Sand specifications. 3.6 An 8" thick layer of Pea Gravel shall be placed on top of the sand layer. The effluent distribution piiping as shown on Orenco Drawing ISF 1919 shall be buried in this pea gravel sdtratum such that the pipe inverts are no less than 4" above the top of the underlying layer of Sand. The PVC laterals shall. have 1/8" diameter orifices drilled in the tops spaced 2' on centers, and shall have Orifice shields installed as shown on the drawings. 3.7 The top of the Pea Gravel stratum shall be flush with the top of the plywood sides of the sand filter enclosure. It shall be covered with Orenco Model LIFF or equivalent filter fabric. Two inches of rigid burial type insulation plus a minimum of 2 feet of soil cover shall then be placed. No evidence was found that could prove the owner was indeed doing so and the case was suspended. Today Kerr called to let me know that Zoning Enforcement is getting a warrent and Trooper escort to issue a summons to the occupents, he would like someone from our office to come along and issue a citation to the occupants for illegal wastewater discharge. This will help their case and give reason to keep the people off the property until such time as a suitable solution can be found. Effluent testing will be required for a period of one year from the date that the system receives final as -built approval by DHHS. Parameters which will require testing include fecal coliform, BOD, suspended solids, pH, and nitrate nitrogen. Additional parameters may be required depending on initial test results. The frequency of testing will be weekly for the first month of operation; once every two weeks for the next three months and once a month for the next eight months. This proposed testing frequency assumes that the system performs as specified. Should the system not perform as specified in item No. 2 above, more frequent testing may be required. All test results shall be submitted to DHHS no later than one week after the scheduled sampling date. 4. The construction of a 4 Ft. thick intermittent sand filter will be required as a method of achieving the necessary vertical separation from groundwater. The bottom two feet of filter material must be installed to meet the two foot thick non-organic native material requirements , specified in AMC 15.65.077.A.1. The material must be placed and compacted to 95 percent maximum density and must be field verified to have a percolation rate between 5 and 60 minutes per inch. A monitoring tube must be installed adjacent to each filter which will facilitate sampling and testing of the effluent as it enters the groundwater. 5. The owner had not obtained a building permit or on-site sewer and well permit, and Kerr believed they were discharging wastewater onto the ground surface. No evidence was found that could prove the owner was indeed doing so and the case was suspended. Today Kerr called to let me know that Zoning Enforcement is getting a warrent and Trooper escort to issue a summons to the occupents, he would like someone from our office to come along and issue a citation to the occupants for illegal wastewater discharge. This will help their case and give reason to keep the people off the property until such time as a suitable solution can be found. I reel this is warrented and recommend a citation be issued if necessary. I need to call Kerr back by this afternoon. Please respond as soon as you can. Definitions. A. Alteration. Any change -to the design or function of an AW WTS that Includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. prior to performing any alterations to an A W WTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system Construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. U. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an A W WTS that were included in the original design which would allow the AW WTS to continue to perform as designed. H. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an A W WTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of . (.00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the A W WTS. The tette begins on the date of approval by the Municipality of the installed system and shall continue while the A W WTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations. Installation and Removal or Additional Equipment. Owner agrees not to make any alterations, removal of parts or additions to the AW WTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AW WTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the A W WTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been perrormed on the system pursuant to the terms and conditions contained in the Owner's A W WTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's A W WTS Operating Permit is: C. Owner acknowledges that the fine schedule for failing to maintain and repair an A W WTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the A W WTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for A W WTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AW WTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B.
