NARRATIVE FOR UPGRADE ON SEPTIC SYSTEM
On March 1,2011, our office performed an adequacy test on the above-referenced property. The
well passed,but the septic system failed the absorption requirement.
The property was then inspected to determine a suitable location for the installation of an alternate
field. On March 7th, a test pit was dug to a depth of 17.2', at which time no water was observed.
Below the surface organic layer, soils were observed to be GM soils to the bottom of the test pit.
Following a seven-day monitoring period, no water was observed.
The original gravity system was installed in 1977. The original 1,000 gallon Sunset plastic tank will
stay in place. The replacement leach field will be a 30 inch wide by 50 foot long deep trench with
an effective depth of 8 feet. A Zobel diversion valve or equivalent will connect the new absorption
trench with the existing one.
The lot is treed with spruce, birch, and some alders, and the area where the leach field will be
installed is relatively level. The lot slopes steeply to the south from the back of the house to
Stillwater Drive.
The area of the lot is 48,620 square feet. The placement of the new system will have no adverse
effect on adjoining lots. See drawings submitted for site details and absorption field placement
details.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 d~ys from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for-a period of up to .one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

REQUEST FOR COSA FROM CONDITIONAL
A Conditional COSA was obtained to complete the sale of a house on the above-described
property. The Conditional COSA was for septic and well.
Following is paperwork for the new COSA. Dean Construction did the excavation work. The
Sunset fiberglass 1000 septic tank remain in place and a Zobel Diversion connects the old 3' wide
x 6' E.D. x 56' long trench with the new 2.5' wide x 8' E.D. x 50' long trench. See the attached
sheets.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private'or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

REQUEST FOR CONDITIONAL COSA
At the request of Realtor Audrey Mason, an adequacy test was performed on March 1,2011, on
the on-site well and septic systems at the above-referenced property. The test on the septic
system was stopped because the water level rose above the invert of the drainpipe before
reaching 450 gallons. The well test was continued for 4 hours and produced 794 gallons with an
average well flow of 3.3 GPM. An additional test was performed on the leach field on March 5 at a
reduced water flow of 0.6 GPM, and the water still rose above the inverted drainpipe before
reaching 450 gallons.
System Description
According to the latest Well And Septic Inspection Report, the existing system is comprised of a
1,000 gallon Sunset plastic tank. The leach field is a 56 foot long by 3 foot wide deep trench with
6 feet of effective depth. There is one sump installed at the end of the trench. In 2005 another
engineering firm tested the well and septic systems and noted the sump measured only 18 inches
of depth below the invert of the drainpipe.
Due to the fact that the leach field has been found to be deficient and in need of replacement and
the cost would be significantly higher during the winter months, the owner is requesting that a
Conditional COSA be granted, as the house has been sold and the sale is scheduled to be closed
on March 28.
