Waiver Request, Trench to Property Line
Dear On-Site Services Engineer:
The design for the septic system on this lot showed the new trench set further from the property
line to achieve 10’ separation. However, due to the steep slopes, and the very limited working area,
the trench was installed closer to the property line than intended. This being said, the 6’ separation
to the existing trench was maintained, and the existing trench was not disturbed. The new trench
does not encroach on the well or septic system for the neighboring lot, and does not undermine
any structures.
For this reason, we are requesting a waiver for a 9.2’ separation from the trench to the property
line. This approval will not have any negative consequences on the home or property, nor will it
negatively affect any of the neighboring lots.

Septic system design
Dear On-Site Services Engineer:
The owner of the above lot has an existing 3 -bedroom home with a septic trench that has reached
end of life, so we are submitting this permit application for the construction of a new septic system.
The attached site plan identifies the location of the home as well as the existing well and proposed
septic location. No conflicts exist between this proposed system and any other well or septic
system, whether on this lot or adjacent lots.
The ground surface on the lot slopes steeply toward the road greater than 25%. However, the
existing system is in a regraded/flattened area, and both that and the proposed alternate site
received a waiver for a 35’ separation to 25% slopes. The proposed system will be located uphill
side and approximately 50’ from any slopes ≥25%. This has been confirmed with a site visit.
Stormwater drainage will not impact this septic system. The new trench will be constructed parallel
to the slope as much as possible.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank. Please refer to the attached
test hole log, plan and profile pages for the septic design. If this design is followed, there will be
no adverse impacts to adjacent properties.

Ref: Proposed Well and Septic System for Lot 6, Block 5; Mountainside Village Subdivision
To whom it may concern:
The proposed 3 bedroom house is to be served by a private well and septic system. The proposed
septic system will consist of a 1250 gallon S.T.E.P. tank and a 5 -wide trench type drainfield. A
well/septic permit (SW910335) was issued in 1991 but was not installed. Three test holes were
dug on the property for the original design by Carey S. Meyer P.E. on 10/4/1991. The septic
system will be designed around the 30 foot radii of these test holes. Comments regarding the
design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutestinch
b. Proposed Application Rate: 1.0 gallons/day/ft
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 450 ft2
f. Total Depth: 5 feet (maximum at any point)
g. M.O.A. Sand Filter: 2 feet
h. Effective Depth: 0.5 feet
i. Width: 5 feet
j. Reduction Factor: N/A
k. Minimum Length: 90 feet
1. Effective absorption area = 450 ft
Note: The distribution line is to be 1 '/4 inch sch. 40 pvc pipe with 1/8 inch holes spaced every 15
inches on center (36 holes per lateral/72 holes total).
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.

4. TOPOGRAPHY: As can be seen on the attached topography site plan, the average
topography in the area of the proposed drainfield are 25-35 percent slopes running approximately
east to west. All slopes 35 feet downhill from the proposed drainfield will be graded to less than
25 percent slopes.
5. WAIVER REQUEST: We request a 35 foot waiver from the drainfield to slopes greater
than 25 percent. The area below the drainfield will be graded so that no slopes exceeds 25
percent. Given the porosity of the soil the effluent should move downward rather rapidly with
minimal lateral migration. Based upon these factors it is reasonable to assume that there is little
risk with the granting of this waiver.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

Dear Mr. Roth:
Attached is the permit application for the approval of an on-site septic system on the above
referenced lot. Below is a narrative description of probable impacts to adjacent properties:
1. Wells - There are no existing wells within 100 feet of the proposed septic system.
Adjacent lots within the subdivision have not been developed.
2. Wastewater Systems - No existing on-site wastewater systems are located within 100 feet
of this lot.
3. Reserved Space - Additional percolation tests were completed on the site to evaluate the
subsurface conditions relative to future absorption field installations. The lot is relatively
large and there is room for several future systems downhill of the proposed absorption
field.
4. Drainage - This lot is relatively steep (20-25%). Positive drainage away from the field
will be maintained, No concentrated surface water will be directed toward the field and
no existing streams or roadside ditches are within 100 feet of the proposed field.
The installation of this system will have no probable impacts to adjacent well or septic systems.
The proposed system's separation distance radius will include parts of adjacent lots, but should
not prevent on-site systems being constructed on these lots.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site watersupply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. 1 further verify that based on the
information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
