The subject permit, issued May 9, 1997 by this office for a
single family well and/or on-site wastewater system, has
expired as of May 9, 1998.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. Ail inspection rePorts
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Septic System Design
Impacts to Adjacent Properties
Dear On Site Services Engineer:
We hereby apply for a permit to construct a well and septic system on Lot
3, Block 2, Hillside North Subdivision, Addn. No. 6. The attached site plan
and backup documentation identify the location, size and components of
the new well and septic system proposed to serve the 3 bedroom house to
be constructed on the lot. Testholes placed in the area of the proposed
absorption trench revealed a silty gravel with sand with a percolation
rate of 8 minutes per inch. No groundwater was noted during the
monitoring period nor is any present now. The soil near the alternate site
appears to percolate much faster and will require a 2' sand filter as
detailed in the attached plan.
The surface of the lot slopes from east to west at varying grades, 'but
drops off dramatically toward the northwest corner of the prope, rty. The
absorption trench will be placed to maintain a minimum of 50' separation
from any 25% break in grade. The surface of the ground in the area of the
absorption trench is fairly flat. The lot is of sufficient size so that no
encroachments will exist between the septic system and any future wells
in the area.
If the system is constructed as designed the following statements can be
made:
The system, if constructed as designed, will have no adverse impact
on the wells in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact
on existing septic systems in the area or those to be constructed in
the future.

impact on reserved space, either surface or subsurface, on any lots
located in the area.
The system, if constructed as designed, will have no adverse impact
on drainage patterns in the area. The current drainage pattern will
be maintained.
