This is in response to the submittals that occurred on December 27, 2001, March 4, 2002 and April 17, 2002.1 have reviewed the engineering drawings, the water sample results and the operational recommendations regarding the septic tank and soil absorption system. It is my understood that the class C public water system source well located on Lot 7. Block 2, Paradise Valley Subdivision will not only provide water to the facility on the same lot, but in the near future provide water to other lots within the subdivision. Waivers - Your requests for waivers between the Class C public water system source well and the on -lot wastewater system are as follows: • To lessen the separation distance between fie Class C public water system source well and the two compartment septic tank to a distance of 100 feet • To lessen the separation distance between the Class C public water system source well and the on -lot soil absorption field to a distance of 125 feet. A review of the well logs, soil reports, current water sample results, and the plan drawings has taken place. It appears the waiver requests are reasonable. Therefore, in accordance with the State Drinking Water Regulations (18 AAC 80.020), the following waiver is granted. • The separation distance between the Class C public water system source well located on Lot 7, Block 2, Paradise Valley Subdivision and two compartment wastewater septic tank serving the home on this same lot has been waived from 150 feet to 100 feet. • The separation distance between the Class C public water system source well located on Lot 7, Block 2, Paradise Valley Subdivision and the on -lot soil absorption field serving the home on this same lot has been waived from 150 feet to 125 feet. The distance noted in the waivers are from the March 4, 2002 are from your submittal. It appears that these distances are from the nearest edge of the wastewater disposal system component to the nearest edge of the existing Class C public water system well. If any component of the existing wastewater disposal system faits or contamination can be traced back to the system, the above granted waivers will be invalid. Public Water System It appears that the public water system has been designed in accordance with State Drinking Water Regulations and guidance. 'therefore in accordance with State Drinking Water Regulations (18 AAC $0.200 (h)), approval as a Class C Public Water System is granted. A copy of the signed registration form, constituting the approval is attached. The Public Water Identification Number (PWS1D #) assigned to this public water system is 218659. This number will need to be on all laboratory request forms and test results to allow- the Department to properly credit this public water system. You may contact Ms. Jamie Staul with our Drinking Water Program at 269.7624, if you should need further information regarding samples for your public water system. Wastewater System It appear that the wastewater disposal systems has been designed in accordance with State Wastewater Disposal Regulations and Guidance. The engineerhas.provided a letter of the field investigation which stated the system was placed in use just recently (source well was installed in July 2001) and is in near new condition. Based on the submitted information the existing wastewater disposal system is approved for use by the proposed two bedroom Bed and Breakfast facility. The signed final approval portion of the Construction and Operation Certificate, constituting this final approval, is enclosed. These approvals do not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Thank you for your cooperation. This number will need to be on all lab request forms and test results to ailow the Department to properly credit this public water system. No vents were above ground. We located the tank and raised the vents above ground. We then followed the septic tank outfall line to the bed and replaced the pipe. A new clean out and a new monitor tube was placed in the bed. It was determined that to find and replace all of the vents would have been expensive and impractical. All of the new vents are shown on the surveyor as built submitted with the COSA.
