Box 19-6650 Anchorage, AK 99519 Dear Sirs: By means of this letter I am requesting a minor revision of WR 060070 which allowed the absorption field serving the residence on Lot 37, Block 4, Paradise Valley S/D to be within 2.0 feet of the closest lot line. As shown on the as-built inspection report, the installation of the absorption field was accomplished in conformance with this waiver. However, during the installation ofthe "Advantex" tank site constraints in the form oftrees and fences necessitated installing the tank approximately I foot from the closest lot line instead of the normally required 5 foot separation. This discrepancy was not discovered until the tank was already installed and could not be moved; thus we are requesting you to revise the previously granted lot-line waiver for this installation to allow approval of the "Advantex" tank as installed within i foot of the property line common with Lot 36. Lot 36 is vacant and approval of the requested waiver will have virtually no impact on the ability to install future water supply and wastewater disposal systems on that lot. Box 19-6650 Anchorage, AK 99519 Dear Ms. Wockenfuss: In response to your comment sheet dated 9/22/06 regarding Paradise Valley, Block 4, Lot 37 I am submitting the following: (I) A complete copy ofthe signed Ingress/Egress agreement is enclosed. (2) A Muni ROW permit to cross the public walkway is enclosed. To accommodate ROW's requirements I have prepared the enclosed revised pages 2 and 3 of the specifications. (3) The soil absorption trench was sized based on the required application rate for soils with a percolation rate between I and 5 minutes per inch. No sand filter is needed because the native soil is predominantly sand. The fact that the native soil peres faster than i minute per inch (any clean sand will do this) means that it will accept effluent better than soil that peres between I and 5 minutes per inch. Thus, there is no reason to require construction ora larger trench than would be required for native soils that perc between I and 5 minutes per inch. I trust that this adequately addresses your concerns regarding the requested permit. Please let me know wben the permit is issued, as the owner and contractor still hope to construct the system this fall. In an earlier telephone conversation we discussed the situation regarding the grade of the natural ground downslope of the proposed trench being greater than 25%. I addressed this issue in my original cover letter, and depicted on the cross-section drawing the fact that a 25% slope line originating 2 feet above the top of tbe sewer gravel will not daylight within 50 feet. Thus, it is my opinion that a waiver regarding this code requirement is not needed; however if you feel othens'ise please issue such a waiver along with the requested lot line waiver that has already been paid for. Box 19-6650 Anchorage, AK 99519 Dear Sirs: The purpose of this letter is to provide the required design narrative in support ofour application for a permit to construct wastewater disposal facilities on Lots 37 and 38, Block 4, Paradise Valley S/D. Soils logs, perc test results, a site plan, design drawings and specifications are enclosed for your review. We are also requesting issuance ora lot line waiver allowing the south end of the trench to be within 2 feet ofthe south property line of Lot 38. The proposed system will be constructed in the vicinity of test hole # 1. As can be seen from the soil log, the native material between 4 and 16.5 feet below ground level is a clean, coarse sand with a measured perc rate of less than one minute per inch. Because the native material is sand no additional sand filter should be required to provide adequate filtration. Using the soil application rate of 1.2 gpd/sq, ft. specified in the wastewater ordinance, this 6-bedroom residence requires a total absorption area of(6 x 150)/1.2 = 750 square feet. The proposed 48-foot long trench design with 8 feet ofeffective gravel has a total absorption area of 768 square feet. Groundwater monitoring conducted at test hole #2 verified the absence of groundwater to a depth of lg feet. The topography of the lot in the area of proposed construction slopes down towards the west at 15% in the immediate vicinity of the test hole and at 30% starting approximately 15 feet west of the test hole. Further downslope the slope increases to 50% and ends with a cutbank adjacent to the driveway. On the profile view of the design I have illustrated the fact that a 25% slope originating 2 feet above the top of the distribution pipe will not daylight within 50 feet downslope of the septic system. The proposed soil absorption trench will be on Lot 38, while the existing 6-bedroom residence and the proposed 2000-gallon STEP tank are on Lot 37. Both lots belong to the same owner and the enclosed easement provides for on-going use and maintenance ofthe system. The reason for the lot line waiver request is to maximize the separation between the proposed trench and the designated replacement trench, both located on the flatter, upper portion of the lot. DATE:._ Bc, ttop Tec~n~alServ~s 14530 Bcbo Canyon POa~ Anc~oraOe, AK 99516 Pltone (907) 345-I355 Lots 37 & 38, Block 4, Paradise Valley S/D 18221 Spain Drive Wastewater disposal system installation Specifications 1.0 General: I.I The scope ofthe project consists ofabandoning an existing 1000-gallon septic tank and failing soil absorption trenches and installation ora new 2000-gallon STEP tank on Lot 37 that pumps effluent up into a new 4g-foot long soil absorption trench with 8 feet of effective gravel located on Lot 38. 1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor deviations from these drawings may be allowed or required by the engineer conducting the inspections. All construction procedures and material specifications shall conform to Municipal and State requirements. All separation distances shall be in conformance with Municipal requirements, unless specifically waived. 1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work around any buried utilities. i.4 Unless agreed otherwise, the contmctor shall be responsible forany tree clearing and grubbing necessary to complete the project. 1.5 The contractor shall provide adequate cover material and rough grading over all system components to ensure that proper drainage is achieved after settlement and that there are no residual depressions. Insofar as possible the contractor shall minimize damage to trees and existing lawn areas. 1.6 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading after the soil is compacted, as well as placement of topsoil and reseeding all areas disturbed by the construction. 2.0 Septic Tank: 2.1 The existing septic tank must be properly abandoned by thoroughly pumping, removing the top and backfilling with soil. 2.2 The new 2000-gallon, 2 compartment STEP tank shall be Municipally approved and shall be set level on undisturbed soil. Each compartment shall be equipped with a watertight manhole cover and a 4" cleanout. If the tank is buffed less than 4 feet, it shall be insulated with 2 inches of approved burial type, rigid insulation. Care shall be taken to locate the tank no closer than 5 feet from both the property line and the building foundation 2.3 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. The waste line from the residence to the septic tank shall have a minimum slope of 1/4" per foot. A cleanout shall be installed 1 - 4 feet from the building foundation. 3.0 Lift station: (Revised 10/06) 3.1 The lift station shall be installed in the second compartment of the STEP tank which has an additional 250 gallon capacity. The lift station package shall be an "Orenco" system as supplied by Anchorage Tank. 3.2 The contractor shall be responsible to obtain the necessary Municipal electric permit and inspection, and shall provide a copy of the electrical inspector's report to the engineer overseeing the construction of the wastewater disposal system. 3.3 The lift station shall be equipped with a watertight, insulated manhole riser and cover extending a minimum of 6' above final grade with positive surface drainage away from the manhole. The contractor shall be responsible for determining the necessary length of the manhole riser, and verifying that the tank's allowable burial depth is not exceeded. 3.4 The on, off, and alarm floats shall be set at the elevations specified by the lift station supplier, and their proper operations shall be verified by the contractor. 3.5 The discharge line from the lift station to the soil absorption system shall be 1.25" dia. tIDPE. The portion of the discharge line crossing the pedestrian walkway shah be buried a ndnlmum of 4 feet below ground level and shah be encased in steel plpe to avoid/prevent disturbance itt the event that a designated wail~way is ever constructed in the easemenL 4.0 Soil absorption system:' 4.1 The soil absorption system shall be constructed by excavating a 48-foot long trench to a depth of 12 feet at the location shown. The long axis of the excavation shall parallel an existing contour. 4.2 The bottom of the excavation shall be level. Because ofthe coarse nature ofthe native gravelly sand some sloughing or caving in of the sidewalls may be encountered and the contractor should allow for possible overage in gravel backfill quantities. 4.3 A total of 8.5 feet ofapproved sewer gravel shall be placed in the bottom of the excavation with the perforated distribution pipes laid level such that the pipe inverts are no less than 8.0 feet above the bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" screened gravel, with less than 3% passing the #200 sieve. The perforated distribution pipe shall be 1.25" diameter Schedule 40 PVC with 3/I 6" diameter holes drilled in the bottoms at 18" on center. All pipe connections in the distribution system shall be glued, and the ends ofall pipes capped. 4.4 Monitor tubes pipes shall be of 4' diameter and installed in the locations shown on the design drawings. The portion of the monitor tube extending through the sewer gravel shall be perforated. 4.5 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. A minimum of 2 feet of soil cover is to be placed over the filter fabric. If the soil cover thickness is less than 3 feet, two inches of rigid, burial type insulation is to be placed over the entire top surface of the gravel, in addition to the filter fabric. 4.6 The top surface of the cover material shall be raised a minimum of 6 inches higher than the surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. Fill slopes shall be no steeper than 3:1. 4.7 Unless specifically agreed otherwise the homeowner shall be responsible for arranging to have the site finish graded after the backfill material has stabilized, and for placement of adequate topsoil and seed to promote rapid revegetation of all areas disturbed by the construction. 5.0 Inspections: 5.1 A total of 4 engineering inspections will be required during the course ofthe project: (I) initial stakeout with the contractor to establish the location of the system and to discuss the plans, specifications and construction procedures, (2) after the native material has been excavated to expose the infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test information, (3) after the sewer gravel is in place and the distribution pipes have been laid and connected up to the septic tank, but prior to placement of insulation or filter fabric, and (4) after rough backfill and grading is complete. The STEP tank requires one inspection after it is set level and the piping connected, but prior to backfill. This inspection may be incorporated with any of the above inspections. 5.2 The installer shall coordinate the timing ofthe inspections with the engineer sufficiently far in advance to ensure the availability of the engineer. We dug up the field at the point shown on the attached plan. This is the point where the line from the septic tank enters the distribution area. At that point we noted that the lines coming from the tank and going down into the distribution area had a heavy dose of sludge. There was a white scum like layer that is hard to describe, on the top, Photo #1 and #2 show the material encountered. The line going to the north-west was pitched downward. This was allowing excess water to flow into the 32 foot section of the field. The line from the outlet of the septic tank to the 6 foot solid pipe showed little evidence of any Septage having ever been distributed into that area. The rocks were clean. _We did see an excessive amount of sludge in the pipe. It appeared to be a thick high suspended solid effluent from your tank. As you remember we discussed the possible causes of these .sOlids. It is my reco~endation that you and Charles sit.down and try to determine what may be going into that tank that could cause {he sludge to bulk and then be Carried Out'of the ~ptic tank and into the distribution 'area. It appears ~hat you have very little fats that go down the drain. However, there is something that is disturbing the status of the septic tank. I then had the excavator dig into the syste~ at the point where the 6 foot'solid pipe starts. This is shown in Photo #3 and #4. Photo #3 'shows the pipe having been pushed downward. A level placed on the pipe indicated t.hat it was .definitely not level but pitched toward the white 45 degree ell. I disconnected the 45 and raised the pipe and looked up in it. There was sludge there but not as heavy as up above. However it was the same type of sludge. Also it appeared that the engineering cloth that had been placed over the pipe had been damaged. This could have been caused by the dozer operator ~hen they were leveling your lawn. This could also be cause for the pipe being pushed downward. There was a joint beyond the point where the shovel is holding up the pipe. See photo #4. This joi~t is not evident but is approximately 4 feet up stream, While there,.before disconnecting the pipe, we had water run into the broken line up near'the s~ptic tank and at that time there was little distribution Of water from the holes in the pipe which is shown in Photo #3. It could be clearly seen from Photo #3 and #4, that the ~ater was going down into the f~eld below. This means that the bulk of the distribution from your house was being done by a 20 foot section beyond the solid pipe and the 32 foot section to the north-west. When I left, the contractor was in the process of leveling the pipes and putting a slight ~ise, using two fittings, to'make sure that the water is evenly distributed in each one of the pipes before moving into the one below. I would have Roto Rooter come in and clean the lines. We discussed that and I believe that you had made arrangements for that before we left. I believe that using his high pressured jetting system that the system then should be operable. The 34 foot section that was bypassed should continue to function for quite a few years. In consideration &the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater /1~ ~'t~ located , Anchorage, Alaska. Treatment System (AWWTS), described as Definitions. A. Alteration. Any change to the design or function of an AWWTS that Includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an AWWTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On-Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time &property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On-Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AWWTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On-Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of ($ --.00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On-Site approval is issued to the new owner or transferee of the property. 5. Alterati0ns~ Installation and Removal of Additional Equipment. Owner agrees not to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's AWWTS Operating Permit is: C. Owner acknowledges that the fine schedule for failing to maintain and repair an AWWTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On-Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AWWTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with thc same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: Owner: Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jurisdiction: Choice of Law. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Memorandum of Understanding. 10. Severability. Any provisions of this Memorandum of Understanding decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Memorandum of Understanding. The water sample was satisifactory and a copy is attached. Ail the standpipes were capped. The septic tank standpipes were not above ground level. We recommend that a interim approval be granted until spring when it will be feasible to excavate. The well is located 96' from the system and has a sanitary seal. The wires are encased in conduit. Some fill will be needed around the well next spring. A waiver of distance for the well was granted by Mr. Robbie Robinson on January 10, 1984.
