If a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. Ail inspection reports must be submitted within 30 days of construction completion. When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit; $120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit. Box 196650 Anchorage, Alaska 99519-6650 Subject: New sewer/well permit - Paradise Valley S/D Lot 10, Block 5 Gentlemen: The owner has requested we proceed forward to obtain a well and septic permit on the subject lot. There is two previous testholes which were dug in Jtme 1994. We have designed our system utilizing these te'stholes, however, we anticipate verifying soils and may possibly modify the system, with your concurrence, The soils had a percolation rate less than one minute and inch, but we have provided a sieve analysis of the underlying sand which provides suitable filter material for the overlying gravel and we feel negates the need for a sand filter. The results of the existing test and water monitoring are attached. We propose to install a 2' wide deep trench. The original testholes indicated no water, and we did not find any water during our monitoring. Additional fill will be placed over the system to provide a minimum of 3' of cover when complete. There are no public or private wells within 200' of our proposed system location except as noted. There is neither surface water within 100' nor any curtain drain within 50'.
