LEGAL: PARADISE VALLEY BLOCK 6, LOT 8
To whom it may concern:
We have been requested to proceed forward to obtain a septic permit to upgrade the aged
septic tank on the subject lot. The proposed upgrade will serve the existing 4-bedroom
house.
The lot and area is served by a private wells and there are no wells within 100 of the
proposed septic tank. The proposed design will not impact any of the neighboring
properties due to the lot layout. Please contact Brent M. Western or me if you have any

RE: NEW SEPTIC TANK PERMIT & CONDITIONAL COSA
LEGAL: PARADISE VALLEY BLOCK 6, LOT 8
Per discussions with MOA On -Site staff, we are requesting a conditional COSA with the
permitted septic tank upgrade and well head improvements. The property has been vacant for
an extended period of time and the buyer is obtaining a construction loan to address these
issues after this short -sale purchase is completed. Attached is the associated lender letter.
A new as -built survey with the inspection report will be provided after septic tank installation.
Work will be completed within 30 days of the closing and all anticipated engineering / MOA
fees will be paid prior to closing. The existing crib is adequate for 4 bedrooms and was tested
for such,:, based on the soil ratings and square footage of the crib.
It is assumed that the extended vacancy, dormancy of the property has contributed to the
higher nitrate levels and once Linder normal daily use, those levels may drop. It is also
anticipated that the replacement of the aged tank and wellhead improvements (casing extension
& new seal) will also improve nitrate levels. In the event that the nitrate levels do not improve,
the buyer is aware that other mitigating steps may need to be made such as pressure grouting
the annular seal and replacing the aged galvanized pipe.
The proposed septic upgrade and other improvements with the issuance of a Conditional COSA will not
impact any of the neighboring properties or pose a public health risk. Please contact Brent M. Western
