General The work under this section consists of the performance of all operations pertaining to fumishing and installing the septic tank/lift station, air compressor and piping to the sand filter. B. Materials 1. Tank a. Septic tank shall be a two compartment tank of at least the minimum size indicated on the drawings and MOA approved. b. First compartment shall have a separate 4" pump -out riser. The second compartment shall have a separate 4" pump -out riser. 2. Manhole Riser or Lift Station Tank •R a. An additional second compartment manhole riser shall be capable of being equipped with the following: 1. A junction (Nema 4x) box or equal, bonded or attached to the pump basin. 2. UL listed electrical cord grips, installed in the J -box. b. An additional tank (M.O.A. approved) may be provided for the effluent pumping assembly and the items in (a.) above. 3. Effluent Pumping Assembly, Controls, Alarm & Piping a. Effluent pumping assembly shall be of approved materials, including the following: 1. A 110 volt or equal U.L. Listed effluent pump. 2. High-density PVC cylinder housing the pump, with level controls and screen, serving as a baffle to prevent the screen from clogging. Pump controls and alarm system shall be an approved system with the following options consisting of. 1. Event counter 2. Elapsed time meter 3. Program timer 4. 10 watt heater 5. Remote alarm panel option with a minimum of 80 D13 sound pressure at 24", operating temperature 30AC to 65AC, continuous sound to be located in the home. 6. Oil -tight visual alarm with push -to -silence feature. Automatic audio -alarm reset. 7. 15 amp motor rated toggle switch, double pole, double throw with three positions; manual (man), automatic (auto) and center (off) (II.O.A.). 8. Nema 4x -rated, fiberglass, or equal, enclosure with hinged cover. 9. All controls to be installed as per manufacturer's recommendations. 10. Control panel to be installed on outside wall of home within sight of septic tank/lift station. c. Piping from the septic tank to the sand filter is to be 1 1/4" PVC schedule 40 solvent weld piping properly bedded as described in Materials Specifications and sloped to drain back to the lift station. Insulation board 2" thick and T wide centered over pipe is to be placed where soil cover is less than 4' depth over pipe. d. The airline from the house to the filter is to be HDPE arctic insulated pipe 3/4"-2" SDR - 11. The compressor will have a gauge (0-10 psi) and be extended into the living area or garage for easy monitoring by home occupants. Page 4 Lot 4, Block 9. Paradise Valley Subdivlslon November 13. 2003 4. Air Compressor a. Air compressor shall be a DIOMAS LINEAR SERIES model number 5060-A or 5070V -A or M.O.A. approved equal. CONSTRUCTION SEQUENCE In order to insure proper installation, the system is recommended to be installed in the following order: I. Sewer line from house to tank II. Septic tank/lift station III. Sand filter IV. Leachfield bed (if required) V. Final grading CONSTRUCTION 1. Construction Instruments Contractor shall provide instruments such as transits, or levels, for establishing proper elevations and levelness of pipe or materials. 2. Pipe Laying All pipe shall be laid with material free of large or sharp rocks and organic matter. Each section of pipe shall be handled carefully and placed accurately. The spigot end shall be fully inserted pointing in the direction of flow and glued in place to make a water tight seal. Each section of pipe shall be properly supported to insure true alignment and an invert which is smooth and free from roughness or irregularity. At all times, when work is not in progress, open ends of pipe and fittings shall be closed so that no undesirable substance will enter the end of the pipe or fittings. Bedding of Pipe for Sewer Lines The pipe should be bedded with materials free of large or sharp rocks and organic matter, compacted. Native materials may be used as back fill material above the pipe, provided it is free of large rocks and organic matter. All lift stations shall be installed in accordance with the manufacturer's recommendations and standards. Electrical installation is to be in accordance with the National Electric Code (latest adopted edition) by a licensed electrical contractor, with M.O.A. inspection where required. If MOA inspection is not required, a licensed electrician is to provide a letter to the Engineer stating that the system meets or exceeds all applicable codes. 4. Excavation Excavate to proper depth to allow gravity flow, install tank level, tolerance 0.05 foot, on undisturbed or compacted classified sand fill material. Create a level sand surface over the tank and add 2" burial foam +35 PSI directly over tank. Mound unclassified soil over tank with a minimum of 2 feet of cover and anti -flotation weights that may be required in areas of shallow ground water. 5. Block 9, Paradise Valley Subdivision November 70, 2003 SAND FILTER The work under this section consists of the performance of all operations pertaining to furnishing and installing the sand filter. A. Materials 1. Sand Filter a. Liner (if required - not required for bottomless sand filter) - OSI Model LI or equivalent. 30 mil PVC cut and fabricated to provide water tight enclosure of filter sand volume. Seams to be permanent as per Manufacturer's recommendations. All connections, such as boot flanges, through sand filter to be permanently bonded to the liner. b. The Contractor shall provide a washed sand to meet ASTM C-33 Concrete Sand Specifications with the additional requirement that all but 1% of the materials passing the #100 sieve be removed. All sand shall be approved for use by the Municipality of Anchorage (M.O.A.). If the sample does not meet specifications, all sand will be removed and replaced. c. The Contractor shall be responsible for providing pea gravel that shall be a washed material containing no fines (0% passing the #100 sieve) and a maximum size of 1/4" diameter. d. The Contractor shall provide the manifold assembly. The manifold assembly shall be of approved materials with orifice shields and flushing assembly. All drilled holes to be cleaned of burrs and pipes flushed, prior to assembly. Flushing valve to include riser to finish grade. Lateral distribution piping is to be laid level (@ t 0.05'). e. Side walls and top of the filter box to be insulated with 2" direct burial polystyrene insulation. f. The Contractor shall be responsible for providing filter fabric. g. Pea gravel and filter sand are to be hand leveled to proper depth (tolerance within 0.05 feet) in filter, taking care not to allow mud or fine soil material into the filter or gravel layer. This would include any soil material picked up by excavation machinery buckets while loading specified material. The Contractor should estimate filter material volumes such that allowance is made for wastage of material in contact with the ground at site. The area over the sand filter to be finish graded with a minimum of 2 feet of cover and to prevent ponding of surface water run off. Side slopes shall not exceed 3:1. The Engineer will reject any sand or pea gravel that does not meet specifications or is contaminated in transit or while handling. If such material is placed in the filter, all soil material may be required to be removed and replaced with new material. I also was responsible for voiding the permit. This decision was a result of finding that the bed was dug 30 feet away from the original soil test, and a new soil test dated 9-9-85 showed water at 3.5 feet. Obviously then a bed could not be installed. The well had already been drilled prior to obtaining a well permit. It was my under- standing from Darcy that the curtain drain had in fact been installed and that solid pipe was also in place across Lot 33. I am surprised to hear that may not be the case. There are several considerations we need to give to this lot. First I would want to see the curtain drain issue resolved. It would appear that either the owners of Lot 33 or Mr. Tamberelli would be the ones to ask. No one from this department witnessed any of the installation. Then we would want to have forwarded to us your inspection record of the curtain drain installation. I am mostly concerned about (1) how the drain is installed; (2) where and how it empties and the measured separation to the bed; and (3) the status of the stream bed we identified along the back property line of Lot 4 which was present in 1985. On the contracted report your office prepared for the department in 1985, you identified a natural stream which came from the north and crossed Lot 4. By the time Mr. Tamberelli began his installation, the culvert which carried the water had been removed and the water re-routed to the ditch on the north side Page 2 Lot 4, Block 9, Paradise Valley of Romania. On several occasions since 1985 I have been in Paradise Valley and the last time I saw the excavation in the summer, there was water standing in it. Based on other information we have on Paradise Valley, there is a strong suspicion that there is still an underground stream making its way through this lot. Due to this possibility, prior to granting a new permit this department will want extended water monitoring. As far as granting a new permit, remember that all requirements set out in AMC 15.65 will apply. You will need to show that the lot has the required amount of reserved area and room for two systems. Based on the soil information in file, 3,000 square feet per bedroom will be needed. Since the natural, unsaturated soil was already removed in the existing excavation, that area may not be usable in calculating the reserved area. In addition, you will have to address the separation distance from known streams and perforated sections of curtain drains. It discharges onto the lot. At that time there was also a discussion of running the discharge pipe downhill on Lot 33 and crossing the culvert on Bulgaria. This apparently was not done. After construction was started, there was some work done o—n-drainage along Romania Drive. The water that ran on several of those lots was diverted from the culvert under Romania Drive and back into the road ditch running toward Golden View. The owner of the property never.finished the sewer system. A bed was designed for the lot and, is fact, the.actual excavation was done. The final effects of the curtain drain Rad not occurred so work was stopped. The redesign used two feet of sand to ensure that the system would, -,be four feet above the groundwater. A standard bed was designed. Earlier this autumn Paul Palmer called me and asked if I would go up and check the lot to see if it was still buildable. When I inspected the lot I found that the bed was excavated but not backfilled with sand. The curtain drain was working, but the tube that I thought I had seen earlier was missing. In early November of this year I had Paul Palmer install a monitor tube to a depth of 6 feet below the bottom of the excavated area. On November 10, 1988, I visited the lot again. The water level in the standpipe was 5.4 feet below the top of the bed. The design is to add 2 feet of sand on top of this soil, which would now put the water level at a depth of 7.4 feet below the bottom of the rock. Also, there had been a monitor tube installed approximately 10 feet above the curtain drain. It was also 6 feet deep and was dry. At the top of the lot, at approximately 50 feet from the northeast corner of the lot, the test hole was at 6.4 feet, and it also was dry. In summary, I believe that the major reason the lot is dry is that the drainage pattern was changed several years back, and has stopped water from running onto 1412 WESt 3312d Avenue * AnchORaGE, Alaska 99503 0 (907) 279-5553 the lot. The curtain drain, as installed, in 1985 is working and has dried out the lot sufficiently so that the water table is well over 2 feet below the bottom of the bed. Recently, the owners agreed to grant you a legal platted 5 foot easement along the eastern lot line of Lot 33, in which you could run the curtain drain culvert pipe you plan to install which will daylight in the existing drainage ditch along Bulgaria. The owners requested a site plan showing where on their lots they could place septic systems and wells, showing that the curtain drain would not hurt their chances of building. The curtain drain certainly will not hurt their chances of building, since it is to catch the seasonal groundwater which I have observed to fluctuate with rainy weather. Thus it will not run year-round and need not be treated as a water source to maintain a 100 -foot separation distance as some curtain drains require. In fact, the only possible effect of this curtain drain on Lot 35 would be good. There are small springs on this lot and perhaps the curtain drain built on Lot 4 will help dry up Lot 33 a little. As to site plans, we cannot make a site plan on any of those Lots showing suggested locations for wells or septic systems, because no soils tests have been done there, and it is not known where good soil is located on these lots. Indeed, before construction of homes or well drilling is done, a good soils test should be performed on each lot to find out where soil adequate for on-site septics is located. Alaska Environmental Control Services, Inc., has an excellent geologist who is quite good at finding good soils in notorious Paradise Valley. THERE SHALL BE 1 INCH OF INSULATION FOR EVERY FOOT OF SOIL LESS THAN THE REQUIRED 4 FEET OF COVER, BUT THERE MUST BE AT LEAST 18 INCHES OF SOIL COVER EVEN THOUGH INSULATION IS USED. THE SOLID PIPE EXTENDING FROM THE SEPTIC TANK TO THE DRAINFIELD SHALL ALSO HAVE 4 FEET OF COVER OR AN EQUIVALENT LAYER OF INSULATION TO PREVENT FREEZING OF THE LINE. 3.6 IF INSULATION IS NOT NECESSARY, THE GRAVEL SHALL BE COVERED WITH A LAYER OF NONWOVEN FABRIC (SUCH AS e/ MIRAFAI, FIBRETEX 200 GRADE, POLY -FILTER X OR EQUIVALENT). 3.7 IT IS RECOMMENDED THAT THE AREA EXCAVATED IN THE VICINITY OF THE DRAINFIELD SHALL BE PLANTED WITH A WHITE CLOVER, RED FESCUE MIX AND KENTUCKY BLUE GRASS. 4.0 CURTAIN DRAIN 4.1 THE CURTAIN DRAIN DETAIL SHEETS SHALL BE CONSIDERED PART OF THIS SPECIFICATION. 4.2 THE CURTAIN DRAIN, DRAIN PIPE, AND OUT FALL PIPE SHALL BE INSTALLED AT A GRADE OF NOT LESS THAN 0.45%. 4.3 THE CURTAIN DRAIN SHALL BE HANDLED ACCORDING TO MANUFACTURER'S INSTRUCTIONS. 4.4 THE PIPE BETWEEN THE CURTAIN DRAIN AND THE OUTFALL PIPE SHALL CONSIST OF SOLID PVC OR PE PIPE. 4.5 THERE MUST BE A SEPARATION DISTANCE OF AT LEAST 20 FEET BETWEEN THE SEPTIC SYSTEM AND THE CURTAIN DRAIN. 4.6 BACKFILL AND COMPACT AS SOON AS POSSIBLE AS INSTALLATION PROCEEDS. ALASKA MO1ROnMTAL COnTnOL SCR`uUS, IN. engineerinq & enuironmental Studies SPECIFICATIONS FOR BED WASTEWATER TREATMENT SYSTEM — LOT 4, BLOCK 9 PARADISE VALLEY 1.0 GENERAL 1.1 The drawings, sheets 1 thru 4, shall be part of this specification. 1.2 All materials and workmanship shall meet the requirements of the Municipality of Anchorage, Department of Health & Human Services, (DHHS) the conditions of the permit, and all applicable rules and regulations currently in effect. 1.3 All excavations and depths are advisory, and are to be verified or modified in the field by a DHHS approved inspector. 1.4 It is the responsibility of the installer to adhere to approved designs for installation, to maintain the specified separation distances, and to have the appropriate inspections. 1.5 If the installation is not inspected by an AECS Engineer, AECS will not be responsible for the installed system. An Engineer at AECS should be consulted prior to construction, to determine the number of Inspections that will be required and to explain what these inspections will involve. 2.0 SEPTIC SYSTEM 2.1 The septic tank shall be a UPC—approved two — compartment tank, constructed of 12—gauge steel with bitumastic coating, set level on undisturbed soil, and insulated with an overlying layer of 2 inch DOW extruded blue styrofoam board. 2.2 The septic tank shall be a minimum of 5 feet from The house foundation, and a minimum of 5 feet from The absorption area. 2.3 The septic tank and bed shall be a minimum of 100 ft, from any private well or body of water, 150 feet from Class C wells, and 200 feet from Class A or B wells, unless otherwise specified. Less than the required separation distance must have prior approval or waiver by Alaska Department of F Environmental Conservation (ADEC). 2.4 Piping shall be fitted with a mchanical w tertight 1200 fest 33rd Auenue, Suite B • Anchoraqe, �Iaska 99503 • (907 561-5040 Calder coupling on the outlet and inlet of the septic tank. Piping shall be 4 -inch solid PVC or cast iron, sloped a minimum of 1/4" per foot. 2.5 Cleanouts shall be installed as designated and capped with air -tight rain caps (Jim caps or equivalent), and extend a minimum of 2 feet above ground level. 3.0 SEEPAGE BED 3.1 The gravel for the bed shall be 0.5 to 2.5 inch, screened rock with less than 3% passing #200 sieve residual. All substitutes must have prior DHHS approval. 3.2 The bottom of the excavation shall be level and raked with the backhoe blade to insure that the bottom has not been compacted during excavation. 3.3 The distribution pipe shall be 4 -inch rigid PVC with a minimum crush strength of 1500 lbs. All pipes shall be laid level, and spaced according to the drawings. 3.4 Two monitor standpipes shall be placed as shown in the drawings. They shall be rigid PVC ASTM D-3034, or 4" diameter cast iron. The section shown with holes may be either drilled 0.5" holes on the 6 inch centers on opposite sides of the pipe, or a section of regular perforated sewer pipe may be clamped to the solid section with a no -hub coupling or solvent joint. A rubber raincap (Jim cap or equivalent) shall be placed over the top of the pipe. 3.5 If the final grade over the bed is less than 4 feet above the gravel, insulation is required, using DOW extruded blue styrofoam board. There shall be 1" of insulation for every foot of soil less than the required 4 feet of cover, but there must be at least 18" of soil even though insulation is used. the solid pipe extending from the septic tank to the drainfield shall also have 4 feet of cover or an equivalent layer of insulation combined with soil to prevent freezing of the line. 3.6 If insulation is not necessary, then the gravel must be covered with a layer of a non -woven fabric (such as Mirafai, Fibretex 200 grade, Poly -filter X, or equivalent). 3.7 The top and sides of the bed should be planted with a white clover and bed fescue mix or blue grass. 4.0 INSPECTIONS 4.1 Three inspections will be required 1 of the excavation, 1 of the sand in place, and a final of the sewer rocks/ pipes/insulation and tank in place. If AECS does not perform the inspections AECS will not be responsible for the bed. In consideration of the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as _T P. 5. Any change to the design or function of an AW WTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an AW WTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AWWTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of ($_.00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations, Installation and Removal of Additional Equipment. Owner agrees not to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's AWWTS Operating Permit is: C. Owner acknowledges that the fine schedule for failing to maintain and repair an AWWTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AWWTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B.
