The attached paperwork shows the new system location, the treatment of the existing system to accommodate the new system, and the proposed well location. Resp~tfully Submitted, Robert .~miefel, P.E. I SYSTEM DESIGN/CONSTRUCTION GUIDELINES AND NARRATIVE LOT 35, BLOCK 9, PARADISE VALLEY 1. System Size - 3 bedrooms x o00 sf/bedroom = 900 sf Recommend Absorption Bed System 20' x 45' = 900 sf This ~vill be an owner installation, gA1 materials, construction methods and required inspections to follow MOA rules an([ regulations. The owner is responsible for notif.ving the Engineer and the MOA ar least four hours in advance of all inspection needs. The Owner will insure no additions or changes have been made to the location of well or septic systems on the adjacent lots prior to the rime of construction of this system. If any changes to those systems have occurred, the Engineer should be immediately notified for review and possible changes. The previous sewer rock installed in the initial trench system (trench system never used) will be removed to at least two feet below the level of r. he bed and backfilled vAth sand. The lot generally slopes down to the west at, 5 - 6% and slopes down at a 2 - 4% to the south. The installation will have little or no effect on the surface drainage, ~ound water, or the adjacent systems in the area. The septic system should be properly maintained to include septic tank inspection. pumping on an annual basis and no garbage disposal grinder in the kitchen. If a garbage disposal grinder is used. the septic tank size shmfld be increased to 1.500 gal. The well must be installed ac least 105' from the nearest point on the septic tank or field. As you are aware, under AMC 15.65.150.E, it is mandated that the as-built drawing prepared and approved by a professional engineer must be submitted to the Department within thirty (30) days from the date Of the final inspection. The subject as-built was received eighteen (18) months following the installation. Also, AMC. 15.65.150.A requires that the Department must be notified at least two hours prior to any inspections made during the installation. This was not done. When you submitted the application for the On-site Well and Wastewater Permit, the attendant supporting information indicated in one place that the septic system would be installed by "the contractor", and inferred on the site plan that the installer would be Acreage Systems, Inc. The above referenced as-built drawing shows that the system was installed by the owner. When it is proposed that the owaer be permitted to install his own system, it is required that the owner provide proof to the Department of his experience and ability in this type of construction, and the department then approves (or disapproves) of the owner installation. This was not done. During a site inspection 6onducted by Messrs Roth and Robinson on April 29, 1993, numerous major problems and discrepancies were noted with the septic system installation which are as follows: Cleanouts at BOTH ends of the absorption trenches were not installed in accordance with the approved system design, and as required under AMC 15.65.060.E.12. The monitoring pipes adjacent to each absorption trench are perforated throughout the length of the monitor tube. ''Robert Kniefel, P.E. '.April 30, 1993 Page Two The approved system design specified a total depth of the absorption trenches to be 5.5 feet below the original ground surface. Assuming that the monitoring tube extends to the bottom of the gravel layer, the total depth of the trench is 6.5 feet. The approved system design specifies a minimum of 3.5 feet of backfill over the top of the gravel. AMC 15.65.060.E.7 requires a minimum of 24 inches of backfill over the gravel layer and E.8 requires insulation over the gravel layer if the backfill is less than 36 inches. The backfill observed during the recent site visit appears to be only 1.5 feet. During the recent site visit, water was observed in the original ground water monitoring tube at a depth of 6.0 feet below ground level. During the recent site inspection, water was observed standing in the monitoring tube adjacent to the west trench at a depth of 5.5. feet below ground level. From these observations it is obvious that the subsurface disposal field in installed in ground water. Based on the above noted problems and discrepancies, the subject on-site wastewater disposal system is hereby disapproved by this office. The Building Safety Section of the Public Works Department has been advised of this situation, and has flagged the building permit accordingly. There must be no remedial work commenced on the subject disapproved disposal system before you meet with this office and a mutual decision is reached concerning how to bring the system into compliance. By a copy of this letter the property owners, Chris J. and Lisa M. Brehmer, are also being advised that the subject on-site wastewater disposal system has been disapproved by this office.
