RE: SEPTIC SYSTEM UPGRADE PERMIT
LEGAL: ROLLING HILLS ESTATES BLOCK D, LOT 14
The property owner has requested we obtain a permit to upgrade the failed septic system of the
above referenced lot. We propose to install one deep trench to serve the existing 4-bedroom
residence. MOA tax assessments show the property as 5 bedrooms. It is a 4BR house with a den
and the owner will correct the bedroom count to 4BR or a 5BR septic system will be installed as
shown on the attached design. The design is based on the existing test hole data found in MOA
record docs from 1996. Per these documents, no groundwater was observed at test hole
excavation or monitoring.
The slopes are flat at the proposed upgrade location. The lot and area are served by private water.
The design will not impact any of the neighboring properties. Please contact us if you have any
questions.

INSPECTIONS ARE REQUIRED. THE FIRST INSPECTION SHALL BE OF THE SUBGRADE, PRIOR TO PLACING
SAND FILETER. THE SECOND INSPECTION SHALL BE AFTER THE PLACEMENT OF GRAVEL, DISTRIBUTION
PIPING, STANDPIPES, TANK, AND OTHER COMPONENTS AS SPECIFIED. THE THIRD INSPECTION SHALL BE
AFTER THE WORK IS COMPLETED.
CONTRACTOR SHALL DELIVER TO THE ENGINEER A SET OF RED-LINED, AS-BUILT DRAWINGS SHOWING ALL DATA
AS SPECIFIED IN SEC. 15.65. F.2, 3, 4, 5, 6 OF MUNICIPAL WASTEWATER DISPOSAL CODE WITHIN TEN DAYS OF
THE FINAL INSPECTION. INVERTS AND SWING TIE LOCATES SHALL BE TO THE NEAREST TENTH OF A FOOT. THESE
AS-BUILTS SHALL BE USED TO DOUBLE-CHECK THE ENGINEER'S INFORMATION PRIOR TO SUBMITTAL TO DHHS.
STAKE ALIGNMENT OF SYSTEM WITH MARKERS SHOWING THE PROTECTIVE DISTANCES FROM WELLS AND
OTHER BODIES OF WATER PRIOR TO BEGINNING INSTALLATION.
ALL EXCAVATION DEPTHS ARE ADVISORY. THEY ARE TO BE VERIFIED AND MAY BE MODIFIED BY THE
ENGINEER.

Design Narrative:
This lot is one of the last lots to be developed in the general vacinity of Sand lake. All of
the surounding lots are served with private well and septic system. The lot is gently
sloping and covered with native vegitation, mostly birch trees with some alders and a few
evergreen trees.
The preposed new design field will be located appreximately 68 ft from and parallel to the
back preperty line of Lot 14 Rolling Hills Estates. Adjacent septic systems are greater
than 45 ft from the proposed field. Lots in this subdivision renge from 1/2 to 1 acre plus
in size. The proposed field has no negative affects on surrounding wells or septic
systems. There wil be no affect to drainage in the area, all water bodies and streams are
greater than 200 ft away. There is adequate space on the lot to allow for an alternate
drain field.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the
system and maintenance. The operational life of all well and septic systems are subject to
these various and dynamic characteristics and are outside the control of the evaluator of the
well and septic system. Therefore, any estimate of how long a system will function satisfactory
for current or future occupants or guarantee that no unseen encroachments, deficiencies or

Due to winter conditions and other factors, we are requesting a Conditional, COSA for the above
referenced property. The pending permitted 5 -bedroom septic system upgrade has been impeded
due to these conditions and will be installed once conditions improve. The system has been
working for the occupants and granting of this Conditional COSA will not impact any of the
neighboring properties or pose a public health risk. Attached are the installation bids and escrow

The undersigned parties authorize Stewart Title to escrow funds in the amount of
$30,201 from the seller, with $1,400 of those funds being contributed from the buyers.

It is the sole responsibility of the undersigned parties to provide invoices to Stewart Title for payment.
Upon receipt and with Municipality approval, Stewart Title shall pay invoices for the above-mentioned
work. In the event parties are requesting to be reimbursed for invoices they paid out of pocket, Stewart
Title will require evidence of paid invoices before any reimbursement is paid. Once work is complete
and invoices paid; any excess funds will be refunded to the seller.

Limitations
Project layout will be the responsibility of the engineering firm or a licensed surveyor if needed.
A. Bid amount includes installation, materials, and labor and equipment to install the septic system as
designed.
Unless stated otherwise, the bid amount does not include engineering services.
B. Unless stated otherwise, the bid amount does not include export of overburden, trees, stumps, etc. They
will be disposed of on-site unless otherwise stated.
C. Final grading will consist of backfilling the construction area with onsite native soils to create a rough
construction grade that meets the minimum M.O.A. soil cover requirements and provides drainage away
from, or around, the septic system. Prior to landscaping, most excavation sites will require hand raking and
shoveling to remove larger rocks, sticks and debris. This work is not included in the bid and unless stated
otherwise landscaping is not included in the bid.
D. There are limitations regarding the quality of final grading. Typically, there is settlement that may occur.
JR's Septic Pumping and Excavation LLC will not be responsible for any ground settlement.
E. Unless stated otherwise, the bid does not include reconstruction or restoration of decks, fences, driveways,
and asphalt, trees, grass that must be removed or disturbed as part of the construction.
F. If this is an engineered project, iayout will be the responsibility of the engineer.
All work will be in accordance with M.O.A. and State Guidelines
All work will be performed as per Engineering Plans and Specifications
Any alterations or deviations from the above specifications involving extra costs will be executed only upon
written orders or verbal and will become an extra charge over and above the estimate.
All work will be done in « workmanlike manner in accordance with standard construction practices.
This is a legally binding contract between the client and JR's Septic Pumping and Excavation.
All collection fees, attorney's fees, and late fees plus a 15% rate of interest will be applied to all accounts 30 days
past due.
Both parties acknowledge the right to lien the work site df the balance of this contract is not paid within 30 days
of invoicing the work performed
This proposal will be void df not accepted within 14 days of the proposal date.
