Conditional Certificate of Onsite Approval Request

I am writing to request a permit to construct a new septic system be issued for this property and a
conditional Certificate of Onsite Approval (COSA) be issued. The septic tank ser4ving this property was
replaced on January 22, 2007 under permit Number SW070006. In the letter requesting that permit I
mentioned that the septic system would need to be upgraded. This letter is requesting a permit to install the
upgrade septic system. The proposed systems will serve the existing three-bedroom house on this lot. This
lot and the lots north, east, and west of this lot are served by AWWU Class A water system. The existing
septic system shall be reused. A diverter valve will be installed between the new tank and the existing field.
The lots developed to the south are served by private water systems. They are located over 100 feet from the
proposed soil absorption system.
1. Conditional COSA. The Owner is in the process of selling this property. The existing system was
located and inspected. The system was tested for operational condition and was found that it could not
accept the required 150 gallons per bedroom per day (450 gpd). (The test was conducted through the top of
the old tank. The water level started out below the invert of the distribution pipe and rose into the
distribution pipe after 200 gallons were induced into the system. The water level returned below the pipe
invert 24 hours later.) The system is able to absorb approximately 200 gallons per day. The new Owner is a
single gentleman with no children. There will be no health risk in approving this Conditional COSA, as we
will be installing the new system in June.
2. Soils. Two test holes were excavated. See the attached soils logs. Ground water was monitored for
seven days. Ground water was encountered during the excavating of the soils test at a depth of nine feet.
Ground water was monitored at a depth of nine feet below ground level after the monitoring period. Bedrock
was not encountered in the test hole. It is my opinion, based on the results of the percolation tests and
overall soils appearance of the soils, an application rate of 1.0 gallons/day/square feet should be used, using
a conventional treatment system.

4. Surface Water: There is no surface water within 100 feet of the existing septic tank and the
proposed and existing drain fields. The proposed installation will maintain at least 100 feet from any surface
water.
5. Topography: The average topography in the area of the proposed septic system is approximately
two to three percent based on the survey plot plan. The proposed installation will be greater than 50 feet
from the shoulder of any steep slopes.
6. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Released for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no private wells or septic systems within 100 feet of the proposed septic location and no Class A water
wells within 200 feet of the proposed system.

Emergency Septic Tank Replacement Permit Request
Ladies and Gentlemen:
I am writing to request that a permit to install a new septic tank be issued for this property. The
system currently serves an existing three-bedroom house. The lot is currently developed and is
served by pipes AWWU water system. The top of the existing septic tank has corroded through
and is a danger and health threat.
1. Tank Replacement. The original 1000 -gallon septic tank was installed in 1973. During a
recent investigation, a worker stepped through the top of the septic tank. We found that the tank
has approximately 0.5 feet of soil covering the tank. This situation poses a serious health risk
and a potential dangerous situation. See the attached site plan. The existing drain field was
inspected and found to be approximately 100% saturated. Liquid is being absorbed, but it is
being absorbed above the invert level of the distribution piping. A soils investigation will be
conducted on January 22, 2007 for a design of a new drain field. The design and request for a
condition COSA (with the new drain field to be installed in the spring of this year) will be
submitted on January 29, 2007 under a separate letter.
2. Surface Water: There is no surface water within 100 feet of the proposed system.
3. Topography: The average topography in the area of the proposed septic system is
approximately two to three percent to the southwest. There are no changes in slope greater than
25% within 50 feet of this lot.
The proposed installation will not affect the future development of the surrounding or existing
lots. There are no wells or septic systems within 100 feet of the proposed septic tank location.

engineer registered In the State of Naska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of Issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions In the professional.engineer's work.
