The bed and tank were installed. An inspection report was submitted by the engineer. MOA had additional comments on the inspection report that were never addressed. This permit application serves to address the MOA comments from June 1997. An additional 1250 gallon steel septic tank was installed in 2001 without the benefit of a permit. This permit application serves to document the install of that septic tank. MOA provided the following comments June 26, 1997. Their resolution is described. Middle distribution pipe 0.3' higher than outside distribution pipes. A profile level was performed April 2020 and all the distribution pipes are at the same elevation. Monitor tube not deep enough and not perforated. A steel pipe was driven deeper into the existing monitor tube and is now at the correct elevation. It is perforated in the area where the septic rock is present. No topsoil/seed over mound. The ground over the septic system has been topsoiled and seeded. A nice green lawn now grows over the field. Structures (shed) over septic. The shed was moved by the homeowner and is now 5' minimum away from the septic tank. Soils. Two test holes were dug in July 1995. Percolation tests were performed in a soil strata that contained sandy gravel with boulders. The percolation rate was 2.6 and 2.9 mpi. The engineer used 0.7 gpd/sf to calculate the required size of the bed. Groundwater was discovered while digging the test holes. This information was previously provided to MOA and is on file. Test hole locations shown on the currently submitted drawings are based on dimensions provided on the 1995 soils logs. Soil Absorption System Design. See Sheet 1 of the design package. This field was tested for adequacy in March 2020. 798 gallons of water were inserted into the double cleanouts downstream of the septic tank near the garage. The water level in the monitor tube was measured before, during and after the test. At no point was water detected in the monitoring tube. This field is adequate as a 4 bedroom field. 3. Wells. This lot and the surrounding lots are served by private water wells. The location of wells are shown on the drawing. The quality of the water was tested in March 2020 of the subject property. The results of those tests are included. The adequacy of this well was also tested. It produced 798 gallons of water in 253 minutes Neighboring Wastewater systems. The neighboring septic systems areal I greater than 10' from the property line. 5. Surface Water: There is no surface water within 100 feet of the septic tanks and drain field. The drain field maintains at least 100 feet from all surface water. b. Topography: The area where the field was installed has a surface slope of approximately 2%. The regulations clearly require that the septic system be designed and, the construction of same, be inspected by a qualified engineer. It may have been misunderstanding on my pad but I was not aware that an owner, under the supervision of a qualified engineer, is not allowed to do the labor involved in constructing a septic system. I have known Mr. Banach for five or six years and knew him to be capable and conscientious. He constructed his house. I provided full time inspection and assisted with the construction. All materials used were per MOA Regulations. Quantities equaled or exceeded those calculated per original design. 2. "Date installed wrong." Date has been corrected to 10-16-95. 3. "Need elevations." See amended drawing, Page 4 of 5. "Need tle-tns. See amended drawing, Page 3 of 5. 5. "Need to show sand filter installed and sand specs." See amended drawings. A two foot sand filter was installed per the approved design. Sand was from MOA approved pit in the valley providing filter material per Paragraph D, 15.65.060 Subsurface Disposal Fields. 6. "Was leachfield or tank insulated?" No. The septic tank and leachfield have three feet or more of cover. The drain line from the house to the septic tank was insulated with 2 inch blue board for eight feet, at which point the cover exceeded three feet. RESPONSE CONT. PAGE 2 RECEIVED 7. Mr. Banach is aware of the fact that the driveway cannot be located over an active leachfield and must therefore relocate the driveway should replacement of the existing leachfield be required. 8. "Is water present in ditch shown along Rickey Way?" There is normally no flow in this ditch, even during periods of high rainfall. However, intermittent flows may be experienced during breakup. 9. "Need to show test hole location." See amended drawing, Page 3 of 5. 10. "Need X- Section showing separation from the water table and to explain the 2 to 7' variation in total depth from original ground." See amended drawing, Page 4 of 5. Separation distance between bottom of gravel and water table is greater than four feet. Variance from finish grade has been corrected to read, "varies 0 to 4 feet." 11. "No legal on sheet 2/3." Topo is illustrated on re-submittaL Main road drainage along Sue Tawn Dr. is on the east side and no surface drainage has been seen in this ditch, even after the heavy rains experienced the past month. I suspect there is surface flow during breakup but that it is very short lived. There is a natural swale along the west side of Sue Tawn which is heavily grassed and shows no signs of surface flow. A culvert diverts the east ditch flow along the south side of Rickey Way via a roadside ditch. Again, since my first visit to the site in May, I have not observed any surface flow in this area. 2. A second test pit was dug and TP#1 & TP#2 are indicated on the Site Plan. 3. Pero Tests are being submitted herewith on MOA forms. 4. Used falling head test for sandy soil with little clay. Pre-soak period is noted on new forms. 5. Noted. 6. The excavation will vary from roughly 7 feet on the south end of the bed to 3 feet on the north end. This is required primarily because we are picking up the sanitary waste from the one bedroom appartment, the south half of the garage. The owner plans to live in the appartment until he can afford to build his four bedroom house. He understands that the kitchen must be removed from the appartment when he finishes his house. He has designed the garage/apt, so that he can expand his garage after the house is completed. The drainline from the garage is for sanitary waste from the appartment, only. He has therefore requested that the septic system be designed for four bedrooms. Topography: The lot generally slopes (10%+/-)to the north. There is a ridge that parallels Little Peters Creek that divides the property with steep slopes (25%+/-) to the west and gentler slopes (8%+/-) to the east. The hill to the east of Sue Tawn Drive has steep slopes. The property north of Rickey Way continues to slope to the north. Surface Drainage: Main road drainage along Sue Tawn Dr. is on the east side and no surface drainage has been observed in this ditch, even after the heavy rains experienced the past month. Properties to the north and east are separated by 60 foot right-of-ways and are undeveloped within 100 feet of the proposed absorbtion bed and its replacement area.
