The subject permit, issued February 24, 1994 by this office for a single family well and/or on-site
wastewater system, has expked as of February 24, 1995.
A new permit must be obtained from this office for a well and/or on-site wastewater system NOT
installed by the expiration date.
If you have drilled the well, a well log must be sent to this office roi' documentation of the
installation and to close the permit.
If the on-site wastewater system has been completed and a licensed Professional Engineer has
inspected the installation of the on-site wastewater system, the original as-built inspection report
must be sent to this office for review, approval and documentation. All inspection reports must
be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 fei' an on-site wastewater permit; $120.00
for a well permit and $440.00 for a combined on-site wastewater and well permit.

RequeSt you reneW permit 11900351, PID #051-511-20 for well and septic
located on the referenced property. I have visited the site and note
that some construction has been completed. However, it w~ll st~ be
necessary for th~ benched area as denoted on the drawin~ to be
completed pJ~or to the installation of the septic system. It is my
understanding that our ~lient intends to complete this project within
the next 12 months, therefore f~rther extention of this permit should
not be necessary.

31, 1990.
A new permit must be obtained from this office for a well and/or
on-site wastewater system Dot installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this office for documentation of the installation and to close
the permit.
If a private engineer inspected the installation of. the on-site
wastewater system, the original as-built inspection report
(three-part form) must be sent to this office for re~iew,
approval and documentation. All inspection reports must be
submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $90.00 for an
on-site wastewater permit; $50.00 for a well permit; $140.00 for
a combined on-site wastewater and well permit.

Dear Robbie,
This letter is in respo~e to your request for more information on the
referenced property to complete your review for well and septic permit
approval. It also serves as the narrative required with permit
applications under the new ordinance passed recently.
As you may recall, you and the property owner disc~sed this property
in September with respect to the proposed w~l and septic installation.
Your visit was requested due to the complications arising from the
steep slope on the lot and the lack of reserve area. However, you were
unable to visit the site and left the discussion up to me. A design
was developed based upon information exchanged at that meeting.
However, amendments to the Wastewater Disposal Septic Ordinance which
were passed subsequent to our design completion but prior to your
receiving the permit application has caused our previous design to be
outdated. Th~, please refer the new design attached.
As noted on the design and referring to the lot slope increases
dramatically to the north. This change in slope would be considered a
cutbank. The owner has created a leveled clearing on the lot in excess
of 50' in width. The seepage trench will be placed at the southernmost
edge of this clearing so as to remain 50 ft. from the newly ~eated
cutbank. (Where the change in slope exceed~ 25%. ) This area proposed
for the septic system is the only area available due to the steep lot
slope to the north and the well location on the adjacent lot to the
west.
The proposed well location does not interfere with any of the adjacent
properties' reserve areas, however, the proposed septic location will
need to be taken into consideration when developing the lot to the
east. Since the lot to the east is very large, as are most lots in the
immediate area, finding a suitable well location should not be a
problem.

This is a dry mountainside lot with very good soils and no drainage problems.
