The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered In the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for prope~es served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions In the professional engineer's work.

investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation. GEG, LtD. attempted to provfdo a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results dosctfbed the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate d~ng the year, and the water usage of the family being served by the system.
These conditions are autside the control of the evaluator of the system. Satisfactory lest
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not prevido
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or pady is not authorized, nor will it confer any legal dght whatsoever.

Ret': Well repairs for Lot 5, Nulukatak Subdivision.
To whom it may concem:
The existing 3 bedroom house is served by a private well and septic system. The well is in the
driveway and is in a concrete pit and repairs are to be made in the spring/summer of 2006 by
Aarrow Pump and Well Services. Recent water sample results have shown fluctuating nitrate
results. Although nitrate levels have fluctuated there is no indication that there has been coliform
contamination, which would be the case if there was surface intrusion through the well head· The
repairs will be performed in May of 2006· In the interim, it is my opinion that the well can be
used safely until the repairs are made· If you have any questions, please contact me at 337-6179.

The Right of Way Division has reviewed a request for a letter of non-objection lo an existing drainfield,
which encroaches nine feet (9') into the ten foot (10') utility easement. On the as-built survey dated June
18, 1996, submitted with the request, the petitioner has shown the drainfield.
This letter of non.objection is issued With stipulations, and by using it the petitioner is agreeing to the
following:
Municipality of Anchorage (MOA) will be held harmless, now and forever, for any damages or
injury to any person as a result of the encroachments.
All applicable cedes and regulations will be'observed and maintained within the easement.
This letter of non-objection will in no way preclude MOA from full use and enjoyment of its
rights within any portion of the easement.
Additional and extraordinary costs incurred'during any future required construction, repair or
reconstruction of MOA improvements to accommodate any or all of the encroachments shall be
paid by the property owner.

Subject: Lot 5, Nulukatak Subdivision.
To whom it may concern:
Prior to the start ofthe well adequacy test the static water level was unable to be taken do to the
fact that the well head could not be removed. A total of 789 gallons of water was pumped out of
the well at which time the well was dry. The water was then shut-offand the well was allowed to
recover for 16 minutes. The water was then turned on and stabalized at 1.6 gallons for 104
minutes (168 gallons). Based upon this data it was determined that the well will continuously
produce about 1.6 gallons per minute, which exceeds Municipal requirements for a 3 bedroom
house (.31 gallons per minute).
The well is in a well pit in the driveway. The well pit is to be upgraded by Brian Wille w/Aarrow
Pump and Well Services with a new monitoring well cap and a drain in the spring/summer of
2006.

A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
5 of Nulukatak subdivision. This inspection revealed a nitrate concentration
of 8.45 milligrams per liter (rog/L) was reported for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 mg/L for public drinking
water systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

To whom it may concern:
The subject lot is served by a private well and septic system. I tested the well and the septic
system on 7/6/96. The results are summarized as follows:
WELL TEST: The static water level in the welt was determined to be 79 feet below ground
elevation. Water was pumped at a rate of 4.64 gpm for a total of 194 minutes (900 gallons). Tile
water level dropped to 365 feet. The well recovery was monitored 265 minutes later, and the
level had rose to 220 feet, a rise of 145 feet. Given a casing volume of 1.47 gallons/foot, this
corresponds to a recovery of 213 gallon in 265 minutes, or .8 gallons per minute. Based upon
this data, it is was determined that the well is adequate for a 3 bedroom house (450 gpd).
PIT WELL: The well is in a concrete pit in the middle of the driveway. The pit has a concrete
bottom and sidewalls, and the top is a steel, diamond plate, lid. By placing a bead of silicon
caulking on top of the concrete, prior to the setting the lid, a water tight seal is provided. The
only viable alternative would be to extend the casing slightly above grade, and put a convex steel
access lid over the top. Attached is a photograph of the current installation. Please provide
direction from your department.
SEPTIC SYSTEM TEST: The owners had moved out of the house several weeks prior to me
doing the adequacy test. Consequently, the crib had some period of "rest". Prior to introducing
water, the liquid level in the concrete crib was 53.25 inches. Water was introduced at a rate of
4.64 gpm for a total of 194 minutes (900 gallons). In the initial 64 minutes of the test (340
gallons), the water level rose 11.625 inches, or 29.2 gallons/inch. From t= 64 minutes, to t= 150
minutes an additional 385 gallons was introduced (725 gallons total), and the level rose an
additional 6.125 inches, or 62.8 gallons per inch. From t=150 to t=194, 175 gallons was
introduced, and the liquid level rose only 0.75 inches, or 233 gallons/inch. At the end of the test
the liquid level had rose a total of 18.75 inches (48 gallons/inch) to 72 inches. The recovery of
the system was monitored 285 minutes later, and the water level was 63.625 inches, a drop of
8.375 inches. This corresponds to an absorption of at least 402 gallons. Based upon this data,
the septic system was deemed to be adequate for a 3 bedroom house (450 gallons per day).

adequacW test are only vafid.for the specific day of the test. P)o'thermore, because (~ the Dmited
nalure of this investigation, it is possible that there are hidden d~fects which may tlO[ have been
detected. No warrantee is made regarding the ~tture petformance of this well or septic ~ystem
SEPTIC TANK: The existing 1000 gallon septic tank was installed in 1971, making it 25 years
old. The condition of it is unknown, however, given the age, it is reasonable to assume that it is
at the end of its useful life. Currently, there is no depression over the tank (indication of
structural failure). The engineer makes no warrantee regarding the future life of the septic tank
CRIB LOCATED IN UTILITY EASEMENT: The concrete crib was installed in 1971.
According to the inspection report, the edge of the system was 9 feet from the property line. On
the as-built survey done by Lantech (6/18/96), the clean-out to the crib is located on the utility
easement line. Assuming the cleanout is in the center of the 14 t~. X 15 t~. footprint of the crib,
the edge of the system is only about 2.9 feet from the west property line. Attached are letters of
non-objection from the utilities companies which use this easement.

Permittee acknowledges the validity of the Easement° Permittee

Chugach harmless from any claims for personal injury, property

continuation of the Encroachment.
5. ~D_~ansion or Increase. Permittee shall not in any way
expand or increase the extent to which the Encroachment occupies
any of the real property encumbered by the Easement without the
prior written consent of Chugach. Chugach has no obligation to
provide such consent.
6. ~L~_~~_~__i~~ate A_~ired. Permittee hereby
acknowledges that the Encroachment and any additions thereto, even
if in violation of this permit, has not and shall not in the future
give rise to any interest or estate in the real property subject to
the Easement or any other real property. Without in any way
limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its
continuation is entirely with the permission of Chugach for
purposes of the common law doctrine of adverse possession. This
permit creates no interest in the real property subject to the
easement.
7. ~_~ication of Utili~~. If the electric
utility facilities located within the Easement are upgraded, added
to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably
attributable to accommodating or preserving the Encroachment. If
such payment is not made within thirty (30) days of written demand
for same, Chugach may terminate this permit in accordance with
paragraph 9, below.
8. ~ovenants Running with thg~nd. The obligations of the
Permittee arising from this permit shall be covenants running with
the land which shall burden the Parcel and the Permittee's interest
in the land subject to the Easement and shall benefit the Easement.
9. ~ermi~ation. Chugach may terminate this permit after the
giving of thirty (30) days written notice of such termination if:
a. The Encroachment is enlarged, increased or extended within
the real property affected by the Easement.
b. The Encroachment is damaged by any means to an extent of
more than thirty percent (30%) of its replacement cost at the time
of destruction.
c. Permittee fails to make the payment described in
paragraph 7, above.

10. T~2~. This permit shall, if not sooner terminated by Chugach,
expire forty (40) years from the date hereof.
11. ~t~=re Agreement. This written permit constitutes the entire
agreement between the parties with respect to the subject matter

agreements, oral or written, between the parties. No
modifications, amendments, deletions, additions or alterations of
the permit shall be effective unless in writing and signed by all
of the parties hereto.

The Anchorage Water & Wastewater Utility has reviewed a request from Mr. David
Sears for a letter of non-objection to the existing encroachments into the utility
easement located on the western 10 feet of the subject lot. An asbuilt drawing, dated
June 18, 1996, identifies a septic system vent pipe encroaching approximately 0.4 feet
into the subject easement. The letter of request, dated July 9, 1996, also states that
the underground septic tank encroaches into the same easement a distance of four
feet.
AWWU does not presently have water or sanitary sewer mainlines in the area.
However, that does not preclude the possibility that we may provide service to the area
in the future and may have an interest in the easement.
AWWU issues this letter of non-objection with stipulations, and by using it the petitioner
is agreeing to the following:
1. AWWU will be held harmless, now and forever, for any damages or injury to any
person as a result of the encroachments;
2. All applicable codes and regulations will be observed and maintained within the
easement;
3. This letter of non-objection will in no way preclude AWWU from full use and
enjoyment of its rights within any portion of the easement;
4. Additional and extraordinary costs incurred during any future installation or
subsequent repair or reconstruction of AWWU facilities to accommodate the
encroachments shall be paid by the property owner; and,
5. AWWU will not support any future vacation of the same easement area.
