1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

Septic System Upgrade Permit Request
This is a design narrative for a permit to install a septic system upgrade on the subject property. The proposed
upgrade will serve an existing four‐bedroom (4) house. This lot and surrounding lots are served by private wells.
Required separation distances to all surrounding wells and septics will be met.
1. Soils:
A test hole was performed on this lot by Pannone Engineering Services, LLC on 8/16/2023. Groundwater was not
observed during the excavation of the test hole, and bedrock was not encountered. Groundwater was observed in
the test hole monitor tube after 7‐days at a depth of twelve (12) feet. Based on the results of the percolation test
and overall soils appearance an application rate of 1.2 gpd/SF was used for the design of a conventional wastewater
treatment system in the area of the test hole.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water:
There is no surface water within 100 feet of the proposed septic tank and drain field.
4. Topography:
See attached site plan for area topography. There are no slopes exceeding 25% less than 50ft from the proposed
drain field.
5. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the
review is complete and no further comments are received from MoA On‐Site Department, the note will be removed
and “For Construction” drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

~ne surficial geologic unit of the site appears to be part of tile Peter's
Creek Far, as it worked through the late Pleistccine lateral mDz-aine
deposits. ~e till of this deposit is corm~ly sandy and stoney with
cobbles and bo~lders co~m3n. Pebbles and s~nd predon~Lnate but silt and
rock dust are prese~nt to provide a bh~der ~or some soils. Cobbles and
Subsurface (bnditions: The %rround cover over the sit~ generally c~nsists
5~-~s t~ a foot of organic silt filled with z~otso Course sands
a~d gravels Lmderlay the surfac~ to a de,ah of 10 to 12 feet. This ].ayer
is intezmtDersed %¢ith cob~bles ~and~ s?n~. boulders, n ttuin organic layer (3 in0
was found at a depth of 4 or 3 teen in m~a]y test holes. 2~e entire area is
%mderlain Yzith weil crraded gravel occationally laced wid~ silt a~]d sand
layers. No gro~d water was encoLmtered in any of the holes except a damp
seepage from one of these s&nd layers. Bel~.; 12 feet, more ¢~-nsly packed
sand and gravel depo~ sits were encountered but still predominate!y %~11
grade~ and occationaly silty. One test hole that had hiqher silt-sand
oDnt~nt was chewed with a percolation test to obi ,ain an actusj. V-d~Ue. for
the soil in questi~]. The results of all invest/gations shcwed that even the
.~rest soils will s ~upport %rode ~rground waste water disposal facilities° All
having pco~r soils were made lar%mr then an acre. and a quainter in ortner
to ha~ sufficient z.~om for future zmplacement systen~; should such ever be
required. The soil profiles sh~ t2]at ~e entire subdi~sion area is sited
on gDod graval, .opt/muTn for seepage pits.
The soils %.~re rated accorfL%ng ['o the Unified Soils Classification System
were ter~mred by the results of the mechanical analysis, t~e experience of the
geolog/st and his observations. ~*~ere dual soils classifications are shown, it
is due to the instructions listed in the [hified Soils classification System
but in all c~ses a soils tat/rig is given, based on the ~nalyses conduc~ued and
the observations ~made. These ratings should be. used for the design of the
sL%bsurface disposal facilities by future property owners.
Conclusion: The soils underlying tJ~is proposed subdivision will support
~ disposal systems ~nich alto p~operly designed and sized. There is
s~fficient room on each of the proposed lots for 8d~tezT~ate disposal areas°
~]e soils a¢~uitable wit~] resrect to f~u, ndation bearing c~mpaci~ for
residential~ structure~. The s~ils are generally NFS wituh layers of F-l,
and F-3 Soils. ,
Soils tests were run in t~e ri~ht-of-way for the proposed ].~m~is and all roads
vzill be desi~ed and constructed to meet clu.-rent municipal road construction

During the period from February 22 through February 26,
1984, we performed research, investigations and testing
persuant to Health Authority Approval on the above
referenced lot.
We performed a flow test on the well and found the
output to be adequate for a four bedroom house. (Refer to
Water Yield Test, attached). We also took a water sample
for bacteriological analysis and results were negative
(Refer 'to Chemical & Geological Laboratory Analytical
Report, a'ttached).
TO our knowledge, we have assembled all available
information on the HAA Checklist and Application Form.
