Ref: Proposed Septic System Upgrade for P & S Subdivision; Lot 2
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The existing drainfield is in a
state of failure and needs to be upgraded. We are proposing to install a new 1500 gallon steel S,T.E.P
tank, excavate out all contaminated material, import MOA approved sand as needed and rebuild the
proposed bed to an elevation equal to or higher than the existing bed. Per the existing code, a soil that
percolates at 20 min/inch requires an application rate of 0.4 GPD/FT2. The previous engineer used an
application rate of 0.5 GPD/FT2. In short, the existing bed is slightly undersized per MOA code. We are
therefore proposing to expand the existing drainfield from 18 feet in width to 20 feet. The drainfield is
designed in the vicinity of an existing 1990 testhole (see page 2/2) performed by Robert Shafer, P.E.
Comments regarding the design are summarized as follows:
1. SOILS: See the attached 1990 soil log which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average topography
around the proposed drainfield is flat.
5. WAIVER: We are requesting that your department issue a variance from the maximum bed width of 15
feet to 20 feet. The existing bed is approximately 18 feet wide per the 1990 inspection report. Extra care
should be taken during the excavation to not walk equipment over the excavation prior to placement of
sand. Please note this as a special provision on the permit.
We are unaware of any adverse impacts this installation or waivers would have on adjacent wells or

Dear Mr. Boster:
In 1990 you applied for and were granted a permit to upgrade the
sewer system on the above lot. When the permit expired and we
had not received an inspection report, this department sent you
a letter notifying you of the omission. You did not respond to
that letter.
Currently we have a complaint that your system has been
discharging sewage onto the surface of the ground. A notice of
violation was issued by this office on May 28, 1993, instructing
you to stop the dicharge and chlorinate the area.
In speaking with your engineer, I found that repairs were made
to the system recently, thus ending the overflow problem.
However, your system remains out of compliance since we do not
have a stamped, signed inspection report of the original
installation in 1990. Furthermore, we will require a letter
from the engineer detailing the repair work that was done.
Within ten (10) days you must submit a stamped, signed
inspection report to this office, and a letter of explanation
from your engineer. Should you fail to do this, this office
will proceed with legal action against you.

well and/or on-site wastewater system has expired as of December
31, 1990.
A new permit must be obtained from this office for a well and/or
on-site wastewater system not installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this office for documentation of the installation and to close
the permit.
If a private engineer inspected the installation of the on-site
wastewater system, the original as-built inspection report
(three-part form) must be sent to this office for review,
approval and documentation. All inspection reports must be
submitted within 30 days-of construction completion.
When applying for a new permit, the fees are: $90.00 for an
on-site wastewater permit; $50.00 for a well permit; $140.00 for
a combined on-site wastewater and well permit.

Dear Mr. Dahl:
We are submitting herewith the test boring results and our co~en~s
'regarding soil conditionsencountered at the subject site. This
investigation was performed in accordance with your request of
April 8, 1974'.
A single test hole was put down within the Lot 2 area for the
purpose of defining general subsurface soil conditions. Excava-
tion was accomplished with an auger type drilling rig and the test
hole was extended to a total depth of 20.0 feet below ground surface.
The general soil profile, as disclosed by the borehole information
was found to consist df a~surface layer of Bilt with some sand
overlying stratas of medium dense silty sands with some gravel
which extended to a depth of 10 feet below ground surface. Below
this' level a stratum of medium dense sand grading to a silty sand
was encountered a~d extended to the depth of investigation. The
final lo§ prepared for the test hole has been included in drawing
A-01.
Ground water was not encountered in the test hole.
