The Environmental Services, Division, On-site Services Program,
of the Department of Health and Human Services has reviewed
the following cases and has these comments:
S-9591S:
Lot iA Edward D Coffey Subdivision
No objections, proposed lot will be served by
public water and sewer.
S-9593:
Lot 15A Block 2 Earl Ray .Subdivision
1. Per AMC Section 21.15.110.B.4.B, all test results
to substantiate the availability and disposal
capabilities of an adequate water supply must be
submitted to DHHS on a form provided by DHHS,
and must bear the original seal and signature
of the engineer who conducted the tests.
None of the data submitted with this application
meets this requirement.
2. The well log submitted with this application
bears no physical location or address.
3. The on-site sewage disposal system inspection
report submitted with this application is
illegible.
4. If this property is to be developed commercially,
any well serving the property must meet ADEC
requirements.

1. A topographic survey map of the proposed plat
needs to be submitted.
2. The availability of a safe and adequate volume
of water must be substantiated.
3. Soils and percolation tests conducted in
accordance with AMC 15.65.090B for each lot must
be submitted.
4. The location of existing water sources, on-site
wastewater within 200 feet of the subdivision
must be shown on the plat.
5. Septic reserve areas must be identified in areas
with slopes less than 25%.
