The property at the time was listed as a duplex and the engineer of record, Michael N. Anderson, P.E. apparently determined that no permit was required according to Alaska Statute 18AAC 72 as administered by the Department of Environmental Conservation (ADEC). An as built of the new system was completed, but no documentation can be found in the ADEC files. A copy of the system as built was submitted with the COSA application. We verified based on field measurements the septic system is as represented in the as built prepared by the engineer. It is further verified by the as built survey completed by the surveyor, Shane Holt, R.L.S.
