Narrative & Permit Application
Dear Mr. Roth:
Eagle River Engineering Services (ERES) was contacted by the owner of the property noted
above to design a septic system upgrade. It was determined that the leachfield was surcharged,
and a new leachfield would have to be installed. Two test holes and soils percolation tests have
yielded silty sand and gravel SM and GM, with percolation rates of 7 and 9 minutes per inch in
the deeper sillier GM soil layer. The upper SM -GM layer from 1.5-3 foot layer has less silt and
will most definitely percolate faster than the deeper soil layer that was perc. tested. No water
table was noted after our 7 -day water monitoring. We propose to install a 2' deep pressurized
drainfield, as shallow as possible without having to mound over the system. The proposed 4
bedroom leachfield is located on flat and 2% slope and is located more than 50' from any slopes
steeper than 25%.
The proposed septic system upgrade will have very limited impact on adjacent properties for the
following reasons:
1. The surrounding lots are all connected to public water, so there is room for
primary and reserve sites on neighboring lots.
2. Immediate neighboring septic systems are all +20' distance, and no private (or
community) wells within 200' of the proposed septic system.
3. Drainage will not be affected and is not a major consideration in our design.
Installation of this 4 bedroom advantex septic system upgrade, to our best available knowledge,
will not adversely affect the wells or septic systems or reserve areas on adjacent lots. If you have

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM — MOA CERTIFIED INSTALLER

A. GENERAL
1. The proposed septic plan is for a 4 bedroom single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State Department
of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi -family wells. A licensed surveyor should locate any utility easements and exact 100'
radius from the existing and neighboring on-site wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. Any remaining open test hole excavations shall be filled and monitor tube removed.
B. SEPTIC TANK WITII LIFT STATION
I. The septic tank shall be an Anchorage Tank 1,500 gallon septic tank with integral OSI lift station
pump model OSI -05-20-1111F, or other engineer approved configuration (example: 1250 gal septic
tank with auxiliary lift basin of 250 gal min.)
2. A receipt and/or statement from a licensed electrical contractor shall be provided to the engineer
verifying lift station wiring to all applicable codes.
C. DRAWFIELD
1. The drainfield is to follow the natural land contour to maintain uniform total depth of the drainfield
bottom.
2. The bottom of the drainfield shall be level, plus or minus 1.5".
3. The elevation of the gravel layer is not to be more than 4.5' below grade as measured at the monitor
tube for test hole # 1.
4. The effluent lines (2) are to be 1.25" PVC lines with 1/8" holes drilled 42" OC within the leachfield,
placed face down, or up with orifice shields placed over holes. Line from tank to field is to be 1-1/4"
HDPE.
5. The drainfield gravel is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of Y or equivalent is to be placed
over the bed. Mounded side slopes not to exceed 3:1.
7. The septic tank and leachfield must not be closer than 100' to any existing private well, I50' to any
Class "C" well, or 200 feet to any community well.
RECOMMENDED LEACHFIELD DIMENSIONS:
BOTTOM OF GRAVEL LAYER = 4.5' max Below Grade at monitor tube
GRAVEL THICKNESS = 2.0' under pipe, 2" over pipe
DRAINFIELD LENGTH = 105' DRAINFIELD WIDTH = 5'
SOIL RATING = 0.8 GPD/115 BEDROOM CAPACITY = 4
SEPTIC TANK = 1,500 Gal S.T.E.P or Engineer approved equal

During my :inspections of the upgrade of the sewer system on the above
referenced lot, it was discovered that the existing system extended over
the .lot line to the west. The upgrade area was attached to the east
side of the existing system. This upgrade area is 33 feet long by 28
feet wide for 924 square feet of absorption area plus a small section
that connects to the existing system that equals 36 square feet for a
total area of 960 square feet. The soil rating for the upgrade area.is
226 square feet per bedroom. This calculates to an equivalent of 2.8
bedrooms for the upgrade area.
A portion of the existing system had to be abandoned due to it crossing
the lot line. Its total area was 540 square feet. In order to leave
enough absorption area in the existing system for a combined total area
equivalent to 4 bedrooms, two perc tests were performed to verify the
soil rating of the existing system. One was done on the, north side of
the system and the other on the south side, each 8 feet from the system
and at a depth representing the soil at the bottom of the system.
Attached are the soil log/pereolatioD test results. The perc test on
the north side of the system calculates to be 263 square feet per
bedroom, while the perc test on the south side calculated to be 150
square foot per bedroom.
Averaging these ratings gives us 206 square feet per bedroom for the
existing system. The original soil rating was 160 square feet per
bedroom. Using the 206 average, 370 square foot of absorption area had
LO remain in the existing system to give us the total area necessary for
4 bedrooms. Therefore, the existing system had to be out off 3 feet
from the west lot line. This left 378 square feet of effective
absorption area and abandoned 162 square feet. The remaining 378'square
feet is equal to 1.2 bedrooms for a combined total of 4 bedrooms.
The abandonment consisted of excavating a trench through the system
along the lot line, cutting the distribution lines at 3 feet from the
lot line, capping them, removing the sewer rock from the trench,
replacing it with soil and compacting it; recovering the 'exposed
remaining portion with fabric and then backfilling.
We request that you grant a waiver of the lot line set back distance of
-"3 feet':This will not encroach on the neighbors sewer system as it is
located on the north-west corner of the lot. There are no wells in this
area as it: is served with a community wager system.

1.0 GENERAL
1.1 THE DRAWINGS, SHEETS 1 THRU 3, SHALL BE PART OF THIS
SPECIFICATION.
1.2 ALL MATERIALS AND WORKMANSHIP SHALL MEET THE
REQUIREMENTS OF THE MUNICIPALITY OF ANCHORAGE,
DEPARTMENT OF HEALTH & HUMAN SERVICES (DHHS), THE
CONDITIONS OF THE PERMIT, AND ALL APPLICABLE
RULES AND REGULATIONS CURRENTLY IN EFFECT.
1.3 ALL EXCAVATIONS AND DEPTHS ARE ADVISORY, AND ARE TO BE
VERIFIED OR MODIFIED IN THE FIELD BY A DHHS APPROVED
INSPECTOR.

2.2 THE SEPTIC TANK SHALL BE A UPC—APPROVED TWO—COMPARTMENT
TANK, CONSTRUCTED OF 12—GAUGE STEEL WITH BITUMASTIC
COATING, SET LEVEL ON UNDISTURBED SOIL, AND INSULATED
WITH AN OVERLYING LAYER OF 2 INCH BURIAL TYPE
POLYSTYRENE.
2.3 THE SEPTIC TANK SHALL BE A MINIMUM OF 5 FEET FROM THE
HOUSE FOUNDATION, AND A MINIMUM OF 5 FEET FROM THE
ABSORPTION AREA.
2.4 THE SEPTIC TANK AND BED SHALL BE A MINIMUM OF 100 FT.

D3034 OR CAST IRON, SLOPED A MINIMUM OF 1/4 INCH PER
FOOT WITH A MINIMUM OF 4 FEET OF COVER.
2.6 CLEANOUTS SHALL BE INSTALLED AS DESIGNATED AND CAPPED
WITH AIR -TIGHT RAIN CAPS (JIM CAPS OR EQUIVALENT), AND
EXTEND A MINIMUM OF 2 FEET ABOVE GROUND LEVEL.
3.0 SEEPAGE BED
3.1 THE GRAVEL FOR THE BED SHALL BE 0.5 TO 2.5 INCH,
SCREENED ROCK WITH LESS THAN 3% PASSING #200 SIEVE
RESIDUAL. ALL SUBSTITUTES MUST HAVE PRIOR DHEP
APPROVAL.
3.2 THE BOTTOM OF THE EXCAVATION SHALL BE LEVEL AND RAKED
WITH THE BACKHOE BLADE TO INSURE THAT THE BOTTOM HAS NOT
BEEN COMPACTED DURING EXCAVATION.
3.3 THE DISTRIBUTION PIPE SHALL BE 4 -INCH RIGID PVC WITH
A MINIMUM CRUSH STRENGTH OF 1500 LBS. ALL PIPES SHALL
BE LAID LEVEL, AND SPACED ACCORDING TO THE DRAWINGS.
3.4 MONITOR STANDPIPES SHALL BE PLACED AS SHOWN IN THE
DRAWINGS. THEY SHALL BE RIGID PVC ASTM D-3034, OR
OR 4 INCH DIAMETER CAST IRON. THE SECTION SHOWN WITH
HOLES MAY BE EITHER DRILLED 0.5 INCH HOLES ON THE 6 INCH
CENTERS ON OPPOSITE SIDES OF THE PIPE, OR A SECTION OF
REGULAR PERFORATED SEWER PIPE MAY BE CLAMPED TO THE
SOLID SECTION WITH A NO -HUB COUPLING OR SOLVENT JOINT.
A RUBBER RAINCAP (JIM CAP OR EQUIVALENT) SHALL BE PLACED
OVER THE TOP OF THE PIPE.
3.5 IF THE FINAL GRADE OVER THE BED IS LESS THAN 4 FEET
ABOVE THE GRAVEL, INSULATION IS REQUIRED, USING DOW
EXTRUDED BLUE STYROFOAM BOARD. THERE SHALL BE 1 INCH OF
INSULATION FOR EVERY FOOT OF SOIL LESS THAN THE REQUIRED
4 FEET OF COVER, BUT THERE MUST BE AT LEAST 24 INCHES OF
SOIL EVEN THOUGH INSULATION IS USED. THE SOLID PIPE
EXTENDING FROM THE SEPTIC TANK TO THE DRAINFIELD SHALL
ALSO HAVE 4 FEET OF COVER OR AN EQUIVALENT LAYER OF
INSULATION COMBINED WITH SOIL TO PREVENT FREEZING OF
THE LINE.
3.6 IF INSULATION IS NOT NECESSARY, THEN THE GRAVEL MUST BE
COVERED WITH A LAYER OF A NONWOVEN FABRIC (SUCH AS
MIRAFAI, FIBRETEX 200 GRADE, POLY -FILTER X, OR
EQUIVALENT).
3.7 THE TOP AND SIDES OF THE BED SHALI, BE PLANTED WITH A
WHITE CLOVER AND RED FESCUE MIX OR BLUE GRASS.
4.0 INSPECTIONS
4.1 THIS BED WILL REQUIRE TWO INSPECTIONS. THE FIRST
INSPECTION WILL BE OF THE OPEN EXCAVATION, TO ASSURE
THAT THE SYSTEM IS INSTALLED IN PROPER STRATA AND
DEPTH.
4.2 THE SECOND INSPECTION WILL BE AFTER PLACEMENT OF THE
GRAVEL, MONITOR STANDPIPE(S) AND DISTRIBUTION PIPE TO
VERIFY PROPER INSTALLATION AND MATERIALS PRIOR TO

On July 11, 1986 we took a drill rig,and post hole digger and tried to
determine if the sewer system onI=k�BTox�lc t',.1%nhw od- II''I`
$tdi'oA'=has on the lot. It had been implied by the next door owner
and the Dan from your office that the system was over the lot line. We
used both the post hole digger and the drill rig and found that the
system is between 6 and 7 feet from the lot line. The orientation of
the field is off by 90 degrees. I have made a sketch of the approximate
location. The 22 foot distance apparently to the back lot line and not
the north -south lot line between the two lots. Attached is the sketch
which shows the way the systems actually lies.
Since the system is less than 10 feet from the lot line I would like to
request a waiver of the distance. The system on lot 2 is on the
north-west corner of this lot so there should be no interference between
this system and the system on the lot. Since this area is served by
public water there is no impact on any wells.
One thing that I did note was that this system only has 540 square feet.
I think this is one of those beds that was put in when the permits were
not using the 1.5 factor. The permit reflects this.

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

Dear Les:
On June 29, 1983 I performed a final inspection of the sewer
system on Lot 1 Block 14 Northwoods Subdivision Phase III.
The ground over the trench was well graded so as to
prevent ponding. The required stand pipes were present and
protruding three feet above finish grade. All of the stand
pipes were properly capped with friction fit P.V.C. caps.
The four pipes observed were the cleanout, two septic tank
pipes, and the pipe at the end of the trench. All of the
Municipal codes appear to have been conformed to.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.
