The 1250 gallon tank has failed and is in need of immediate replacement. We are proposing that the existing tank be replaced with a new 1250 gallon (minimum) tank constructed and installed in accordance with MOA requirements. A polyethylene or advanced coated tank is recommended. As shown on the plan, the tank will be greater than 10’ from the house foundation. The tank shall be covered with a minimum of 2” moa approved insulation and 3’ of cover. The repair shall be performed by a moa certified installer in accordance with MOA requirements. The engineer will inspect the tank before backfilling. Repair of the proposed system will not negatively impact adjacent lots. Upon completion of the installation, a record drawing will be submitted showing the location of the new tank, leachfield, well, and other applicable features. Health & Human Services On April 26, 1994, you requested that we install to 12' and observe for 10 days a monitoring tube at the above referenced lot. In addition, you recommended that we provide a confirming percolation test in the proposed accepting soils strata. Per your request, on April 29th I completed a new 15 1/2' test hole, installed a monitoring tube, and conducted a new percolation test. No water was found in the test hole and the soils strata was uniform from approximately 2 1/2 feet to 13 feet as identified on the attached soils log. The percolation test was run between 6'-7' with a percolation rate of 12 min./inch. On May 9th, I inspected the monitoring tube and found no water present. In addition, I found that the small pockets of standing water which I observed in the depressed area during my April 21st inspection had disappeared. Based on the new soils log, percolation test, and proposed location we are requesting your approval of a redesigned septic field consistent with the attached supporting documents. April 21, 1994 This subdivision is serviced by a public water system; the service line enters from Sheltering Spruce Loop. As shown on the site plan, there are no conflicts with existing on-site sewer systems or with potential reserve areas. This lot has a elevation difference, as illustrated on the soils log, across its southern portion generally running southwest to northeast. As a result, the builder is planning on siting the house, as indicated on the site plan, to take advantage of the grade differential, and the sewer system will be installed in the flatter, well drained area to the northwest of the proposed house site. There is adequate area directly south of the proposed sewer site to install a replacement system. The natural slope will provide positive drainage away from the proposed absorption site. During my inspection on April 21st, small pockets of standing water were observed in the depressed area. Probing the area indicated that portions of the organic layer were still frozen. Based on the location of these pockets and the probing conducted, it is anticipated that these pockets will dissipate upon completion of breakup. There is no surface water within 150 feet of any portion of the proposed installation. The design is based upon testing and percolation data performed by AECS in 1986. At that time, no water was encountered. The monitoring tube was not located during field inspections of April 15th or 21st, 1994. There are,Wo-pefcolation rates reflected on the soils test: the C36 between 3' and 6' was rated t 25 /in; the strata between 6' and 9' was rated a in./in. A weighted average soi s rating is calculated at 0.54 gpd/sf. Refer to drawing 94-S1-0416 for computations. The trench depth will be limited to 8' below grade due to the shallow depth test hole. The design will incorporate two parallel trenches, each 42' long, each with 5' of gravel depth. For design purposes we will assume an average home of 3 bedrooms is to be built on the lot. For smaller or larger size homes, the reserve area for the sewer system can be adjusted. The water supply of this lot is from the Northwoods Community Water System. The type of soil encountered on this lot is ML, a silt and SM, a sandy silt The percolation test results show the soil rating to be 246 square feet per bedroom. No ground water was encountered. The soil log and site plan is included as a part of this report. For this lot we recommend a trench type of system be designed. The approximate size of this system would be 85 feet long x 9 feet deep with 5 feet of gravel for a total of 850 square feet for a 3 bedroom house.
