An On -Site Water/Wastewater Permit, number SW010121, issued by this office for a single-family

If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

To Whom it may concern:
This letter is response to your comment concerning the system being put into the water table, the
water level in test hole #1 was high due to the utility line excavation as stated in the original letter.
The lot has been combined from two into one large with the utility lines being removed however
the surface water appears to be follow the power line excavation directly to monitoring tube. I
have moved the system to the second test hole which has no water problem. Both primary and
secondary systems can be shown in the second test hole radius with test hole #1 being abandoned.
The owner maintenance agreement has been included with this submittal.
The total number of bedrooms is 4 at this time for the new residence.

To Whom it may concern:
This is a request for a well and septic permit at the above referenced location. The lot is large, 3
plus acres, with a stream on the north end as the plot plan shows. The two percs were slow, 48
minutes per inch and 42 minutes per inch with a water seep found at 4 feet in test hole number
one. Only one test hole had any water to be concerned about, the second hole was good with the
softer material on the top just like the first one and no water. The water appeared to be from the
telephone line excavation and not any existing water table. The existing utility line will be
removed due to combining of the two lots. Numerous test holes have been excavated on the lot
for buying purposes and found mixed results concerning the water table and soils types. For this
reason a bottomless sand filter has been chosen for his lot. The maximum depth of excavation
will be two feet in to the existing material and then mounded over for cover of the bed. The soils
were very stiff the deeper the hole was advanced, therefore a shallow system will be installed per
the design.
This new system will not affect the future development of the neighboring properties due the large
lots and locations of the wells and septic systems. The general drainage is to the west and south
towards the road/driveway.

Impacts to Adjacent Properties
Dear On Site Services Engineer:
We hereby request a permit to place a septic system on the subject lot in
accordance with the attached design. Testholes placed on the lot revealed
a silty sand with a percolation rate of 55 minutes per inch. We are
proposing to place a wide drainfield with 4' of drainage rock beneath the
laterals. A total of 134' of trench will be required. A pressurized S.T.E.P.
tank will be placed to transport the effluent to the absorption trenches.
The topography of the lot slopes from west to the east from 3% to 8%.
Groundwater was found in Testhole No. 17 at a depth of 11.5' in late May
of 1994. The water table was monitored over a year's time and this was
determined to be the high level. The system has been designed to provide
4' separation from the water table's highest recorded level.
The alternate disposal site is a shallow bed designed for a percolation
rate of 56 minutes per inch. The twin beds are 15' wide and 70' in length
and spaced 10' apart.
If the absorption trench is placed in the location shown the following
statements can be made:
The system, if constructed as designed, will have no adverse impact
on the wells in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact
on existing septic systems in the area or those to be constructed in

The system, if constructed as designed, will have no adverse impact
on reserved space, either surface or subsurface, on any lots located
in the area.
