We are therefore requesting a permit be issued for the construction of a new well and septic system to accommodate the home. The attached Site Plan and backup documentation identify the location and configuration of the proposed well and septic system along with the alternate site. Also identified on the plans are the existing wells and septic systems on surrounding lots. No conflicts exist between the proposed well and septic system and wells or septic systems on adjacent lots. Drainage arrows are shown on the site plan showing the grade and direction of flow on the lot. The drainage pattern will not be changed as a result of this project. The test holes placed on the lot in the area of the absorption trench and alternate site indicate the presence of sandy silt with gravel. The percolation rate of the material was determined to be 16 minutes per inch. Groundwater was not found during the placement of the test hole but was monitored at 5' below the surface. An application rate of 3 gallons per day per square foot was used in the design of the absorption system. We are proposing to place a new Advantex Treatment System with a 1,500 - gallon tank and an AX -20 Filter Pod with a new 35' long by 5' wide by 1' effective depth absorption trench to treat and dispose of the effluent generated in the house. The elevation of the distribution line in the absorption trench will be at the existing ground surface and the total depth will be 1' below ground. A minimum of 3' of fill or 2" of insulation and 2' of fill will be placed over the absorption trench and 4' over the septic tank. The ground surface on the lot in the area of the septic system slopes to the west at a grade approximating 20%. The new trenches will be constructed parallel with the slope as much as possible in conformance with Municipal requirements. The new septic system will be constructed a minimum of 100' from all wells and any surface water and Lot 9, Block 3, Pennington Park Subdivision September 30, 2013 Page Two 10' the proposed and existing water services in the area. If the system is constructed in accordance with our design the following statements apply: 1. The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The system, if constructed as designed, will have no adverse impact on reserve space, either surface or subsurface, on any lots located in the area. 4. The absorption trenches for the lot will belong and shallow. A substantial area is required for the system. The area available is restricted and the new trench must be placed within 4' of the south property line. Soils in this area are clean sandy silts with percolation rates under 16 minutes per inch. Migration of effluent is unlikely. Placement of the trench in this area does not impact the property to the south or the well and septic system on that lot. The scope of this project includes the procurement and placement of a new Four Bedroom Category III Nitrate Reducing Septic System as manufactured by Anchorage Tank and Orenco Systems to serve the home on the lot which is proposed for construction. The distribution line in the bed will be placed .5' above the existing ground surface. The new system must be placed a minimum of 100' from the proposed well on the lot and those on adjacent lots and 100' from any surface water located near or on the lot. 2. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. 4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Development Services Department, Building Safety Division, On -Site Water and Wastewater Program for system installations. Owners installing their own systems must receive prior approval from the On -Site Water and Wastewater Program before beginning system installation. SEPTIC TANK INSTALLATION 1. The Category III Treatment System must be installed in conformance with specifications and regulations furnished by the manufacturer. No variances from the manufacturer's specifications will be allowed without permission from the manufacturer and Municipal representatives. Construction shall include two 4" cleanouts for pumping access. 3. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 4. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial insulation. 6. A foundation cleanout shall be installed one to four feet from the building foundation. Two cleanouts are required between the tank and the drainfield. 7. Final grading over the tank shall be such that a positive slope exists away from the septic tank. 'DRAINFIELD CONSTRUCTION: 1. The drainfield shall be constructed to the dimensions shown on the design. The bottom of the bed shall be within 2" of level. 2. Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. 4. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion below ground must be perforated. 5. Contractor shall verify the septic tank and drainfield are a minimum 100' away from any private water wells in the area, 150' from a Class "C" Well or 200' from any community well. 6. Direct bury insulation must be placed over the distribution system if less than 3' of backfill depth is available. Finish grade over the trench must be mounded to prevent settlement or depressions. 7. Grade area surrounding the absorption trench to drain away. 8. A minimum 2' of accepting soil is required below the drainfield rock for a 5' wide trench. Contractor shall verify this condition prior to placement of the rock. Septic tanks must be constructed by a municipally approved septic tank manufacturer. 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662 or A.B.S. (perforated and solid). 3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Co. Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the #200 sieve. SAND GRADATION: 1. Sand used to level the new absorption bed must have the following gradation: 50% Passed the No. 4 Sieve 2% Passed the No. 100 Sieve 1% Passed the No. 200 Sieve 2. The sand source must be approved prior to use. INSPECTIONS: Municipal Ordinance requires a minimum of two inspections. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. The second inspection must be conducted after the placement of the geotextile fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No backfill should be in place at the time of inspection. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as Advantex Wastewater Treatment System located at (legal description). Lot 9, Block 3, Pennington Park Subdivision 2. Definitions. Alteration. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate of On -Site Systems Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Tenn. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use oris operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations. Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. Our limited analysis (due to the off-season conditions and the fact that the site was considerably disturbed) suggested that the "P" wetland unit does not encompass the entire area that we have mapped. Due to the extenuating circumstances of the Federal Government furlough, we drafted a memo supporting the issuance of the Footing and Foundation permit with a caveat that the property owner was to contact the Corps to address the "P" wetland unit. After a discussion with the property owner's contractor today, they will be seeking a PJD from the Corps this week. "P" Background: The property owner of Pennington Park Block 3 Lot 9 became aware of mapped "potential" wetlands on his property during the review of a footings and foundation permit for his proposed single family dwelling building project. The property owner had already cleared and excavated for the proposed structure's foundation pad. This wetland area was added to GIS in March, 2008; therefore, the wetland unit was not shown on the Anchorage Wetland Atlas, 2008 edition. Planning staff conducted a site visit on October 2, 2013 and completed the vegetation portion of the Wetland Determination Data Form—Alaska Region. Photos of the site and the data sheet are attached to this memo. Summary: On May 15, 2013 a Jurisdictional Determination was issued for the Goldenview Drive corridor from Rabbit Creek Road to Romania Drive; POA -2o13 -23o, Little Rabbit Creek. The east edge of this parcel is located within this approved JD boundary; the JD shows that approximately 6o feet off of the Goldenview Drive at this parcel is upland. Because of the timing of our site visit occurring after the growing season and the current disturbance on site, a formal wetland delineation is not possible. Our preliminary field data suggests that the vegetation present in the north portion of the lot (see attached map for data point location) does not meet the tests for presence of hydrophytic vegetation. As of October 1, 2013 the Federal Government is on furlough status. The capacity of the US Army Corps of Engineers is unknown at this time As of the date of this memo the Anchorage Field Office is open until October 11, 2013. It is unclear if the Corps is processing fill permit applications. Response: Due to the unique situation, a note shall be placed on the Footings and Foundation permit regarding the mapped "P" wetland unit. It shall be up to the property owner to consult with the US Army Corps of Engineers regarding their development on this site.
