The was installed at a distance that measured over 100 -feet from the well. Upon receipt of the asbuilt survey, it was found that the tank was 98 -feet from the well. It is believed that the distance measured by the contractor did not account for the slope. We are requesting the separation distance between well and tank be waived to 98 -feet. This is a significant improvement over the placement of the original steel tank which was closer to the well. Properly installed plastic tanks are unlikely to leak in their lifetime which justifies this relatively minor encroachment, along with the history of approval for the previous tank. We are requesting that the waiver fee be waived, as has been done on similar installations where a waiver was in place for the existing tank. Thank you for your time in reviewing this request. The separation distance pending upon conditions being met has been waived ., The conditions of this waiver are as follows: 1. At the time of excavation the septic tank is exposed and the integrity of the tank verified and if not already in place, water tight calder couplings or the equivalent be placed on the tank fittings. 2. The upgrade of this system be constructed to the engineers design and completed by June 15, 1987. The owner wishes to replace the tank at once. Our review of available documentation and field investigation show that this project will not adversely impact any nearby Wells, Wastewater disposal systems, replacement disposal sites, or drainage flowing onto and off of the subject property. As shown on the plan, the tank will be greater than 10’ from the house foundation. The tank shall be covered with a minimum of 2” moa approved insulation and 3’ of cover or a minimum of 4’ of cover without insulation. The repair must be performed by a moa certified installer in accordance with MOA requirements. Repair of the proposed system will not negatively impact adjacent lots. Upon completion of the installation, a record drawing will be submitted showing the location of the new tank, leach field, well, and other applicable features. Owner River View Estates S/D, Block 5, Lot 2 21865 Barbara Falls Drive Eagle River, Alaska 99577 PERCOLATION TEST RESULTS AND GENERAL SITE INVESTIGATION REPORT On October 1, 2012, an adequacy test was performed on the well utilized by a three- bedroom house on the above -referenced 49,649 square foot property. The well passed with an average production of 3.8 gpm. in 4 hours and 30 minutes. The adequacy test on the septic system failed. Before the test, the leach field had 50" of water in Monitor Tube 41. Less than one hour into the test, water in Clean -Out #2, which was adjacent to Monitor Tube #2, measured 17" from the bottom of the pipe. This indicated that the water was above the invert of the distribution pipe. On October 5, 2012, two test holes were dug by Dean Construction. Test Hole #1 was dug to a depth of 6.0', at which time ground water was noticed (see attached site plan for location of Test Hole #1). Test Hole #2 was then dug in a different location to a depth of 15.01. Ground water was observed seeping into the hole at approximately 13.0'from ground level. The additional 2' was dug to verify that no bedrock was present. A monitoring tube was placed in the test hole and a perc test was performed with ten minute readings. The last three readings were recorded and a soil log was prepared. The pert rate was 2.1 minutes per inch. On October 12, 2012, after a seven-day monitoring period, ground water was observed in test hole #2 at a depth of 13.5' from ground level. The attached design drawing is for a 3' wide x 5' E.D. x 50' long trench. The lot slopes approximately 6-8% from the edge of the proposed leach field. The area is treed with spruce, cottonwood, birch, and alder, with birch being predominate. On-site observation and physical survey show that there are no water wells or private wastewater disposal systems within a 100' radius of the proposed system. Mr. John Oates. Our field measurements also shova the existing,' sept.ie Lnnl: to be at 87' from the existing well, In an effort to save the owners the expense of a new tank, I believe that our wai.ve.r application will demonstrate that the 87' horizontal distance scparati.on is adequate to protect Lire well. from septic contamination. The enclosed we.tl log; shows that the well draws from a bedrock confined aquifer, through sporadic fracl.ures at a depth of 80'-175'. The casing is continuous to 51.2' where it .is seated into bedrock. The surface topography :is such that any seepage would be di-rected away from the well location toward a -1-20% elope to the North Last. Any possible leakage frons the tank would be directedawayaway from the well. From our lest hole=s and knowledge of the area, i.t is safe to assurne that the bedrock plane Slopes away from the well toward the septi -c system at an approximate 20% slope. The subsurface soil. -is a GP type with a pert, rating of 15-0. A water sample- for coliform bacteria will be taken as a condition of final health approval. As a condition of permit on the leaehfiel.d upgrade we are Tari-lling to expose the existing tank and provide water tight inlet; & outlet connections, seal the manhole cover and provide insulation. SHAFER CIVIL ENGINEER 694.2979 A sewer system adequacy test was performed on the system located on the referenced property, as you requested. The septic tank was pumped and verified to have a capacity of 1000 gallons. The absorption trench was tested by continually flowing water through the system for a period of 48 hours, without any adverse effect on the system. The average flow each 24 hour period exceeded 600 gallons. It can be concluded rom this test that the waste water dispeoal system serving the two bedroom residence located on this property is currently functioning adequately. However, the system cannot be guaranteed against subsequent failure. At this same time a well inspection was performed and it was determined that the well casing is equipped with an adequate sanitary seal and the wiring is in conduit. The ground around the well casing adequately slopes away from the well. At this time a water sample was taken and submitted to Chemical and Geological Laboratories of Alaska for coliform bacterial analysis. The results of this test were satisfactory. The septic tank was pumped and verified to have a capacity of 1000 gallons. The absorption trench was tested by continually flowing water through the system for a period of 48 hours, without any adverse effect on the system. The average flow each 24 hour period exceeded 600 gallons. Tt can be concluded rom this test that the waste water dispoo al system serving the two bedroom residence located on this property is currently functioning adequately. However, the system cannot be guaranteed against subsequent failure. At this same time a well inspection was performed and it was determined that the well casing is equipped with an adequate sanitary seal and the wiring is in conduit. The ground around the well casing adequately slopes away from the well. At this time a water sample was taken and submitted to Chemical and Geological Laboratories of Alaska for coliform bacterial analysis. The results of this test were satisfactory.
