Lopes-Gomes: The subject permit, issued September 10, 1993 by this office for a single family well and/or on- site wastewater system, has expired as of September 10, 1994. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. If you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If the on-site wastewater system has been completed and a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspectioo reports must be submitted within 30 days of construction completion. SPECIAL PROVISIONS RECEIVED BY: DATE: DATE: DESIGN CRITERIA AND NOTES Project Scope: Well and On-Site Septic System Design Legal Description: Riverview Estates, Lot 1, Block 7 Date of Report: August 12, 1993 DESIGN CRITERIA This design is for a four (4) bedroom single-family residential on-site well and septic system. Two test holes were excavated at locations indicated on the attached site plan. Percolation rates were 3.2 min/inch and 8.3 min/inch for Test Holes #1 and #2 respectively. Water was not encountered during the testing, nor six days into the monitoring period. Based on four bedrooms, the daily design flow is 4 X 150, or 600 gallons/day. The application rates are 1.2 and .8 gpd/SF respectively. The required area is thus 600/1.2, or 500 SF. We have selected an area for the drainfield down-gradient from the proposed house. The bottom of the drainfield (bottom of the washed rock) will be at a maximum depth of 12 FT below existing grade. The septic tank will be a 1,250 gallon steel septic tank, with two compartments, Anchorage Tank or pre-approved equal. The lot slopes generally downhill to the north, with no significant reaches of greater than 5% slope.
