The Environmental Services Division, On-site Services Section,
of the Department of Health an Human Services has reviewed
the following cases and has these comments:
S-9492:
Lot 2 Block 10 Riverview Estates Subdivision
Information submitted in support of this subdivision
does not satisfy preliminary plat submittal
requirements specified by AMC 21.15 and AMC 15.65.
Specific deficiencies include but may not be limited
to:
1. Soil percolation and groundwater monitoring has
not been submitted.
2. Areas designated for the original and replacement
wastewater disposal systems have not been
identified.
3. Supporting documentation on water availability
has not been submitted.

Lot 3A Block 1 Voyles Subdivision
The subject property is served by an on-site
wastewater disposal system approved by DHHS. The
applicant must provide DHHS with a copy of the well
log and site plan showing location of the well.
