The subject permit, issued June 22, 1994 by this office for a
single family well and/or on-site wastewater system, has
expired as of June 22, 1995.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

We under~tand that you hesitate to g~ant the sub,eot easement to
~he owners of the Kwik Log Subdivision as you feel the easement
will de~ract from your p~operty v~lue.
This letter is ~o explain the necessity of the easemnt and to
urge you ~o ~sstst the owner of the system and th~a office in our
efforts to bring the water supply into compliance with applicable
codes,
The Alaska Administrative Code establishes protective radii around
all se~d-publie ~md public water supply systems. It is against
the law to install various po~ions of sewerage system~ within
these radii. The well involved is a Class A, Semi-public and the
protective r~dli are ~a

Irregardless of the existence or the nonexistence of the required
easement, it is illegal to violate the protective radii established
in the code.
The easement t~ ~rely a method of assuring that the well is not
encroached upon without re~uiring continued surv&tltance by the
Department.

We are sure tha~ you can appreciate this need if you col~sider that
many of these a~mll subdivisions' wells stay in use for 20 yeare or
~ore. Reoording of the easement provides a permanent ~eord and an
effeottve method of preveutinE eneroaohment.
As the situation stands today, we are unable to approve the water
syetem Eo~ eervtce ko additional homes~ conaequemtly, 1£ you were
to develop you~pro~erty, ~ater from the existing s~$~em would not
he available.
A[ain, we urge you to provide the neceseary easements at this time

cesspools~ as regulated by the Greater Anchorage Area
Borough Health Depamtment.
To have and to hold the said easement, right, and right of
way unto the party of the first rapt, successors, or assigns for a
period of perpetuity and under the conditions, restrictions and
considerations as set forth above.
