Septic system design
Dear On-Site Services Engineer:
The owner of the above lot intends to build a 5-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the proposed septic location and
alternate site. No conflicts exist between this proposed system and any other well or septic system,
whether on this lot or adjacent lots.
The ground surface on the lot slopes upward toward the back of the lot, and the septic system is
place near the front of the lot. (There are no slopes greater than 25% within 50 feet downslope of
either the primary or alternate site.) Contours are shown on the site plan showing the grade and
direction of flow. Stormwater drainage will not impact this septic system. The new trench will be
constructed parallel to the slope as much as possible.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank. The alternate site is located
more than 6’ away.
Please refer to the attached test hole logs, plan and profile pages for the septic design. If this design
is followed, there will be no adverse impacts to adjacent properties.

An On -Site Water/Wastewater Permit, number SW070318 issued by this office for a single-family
system, will expire on 05 December 08. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer
The owner of Lot 27, Block 7, Kasilof Hills Subdivision intends to construct a new six -
bedroom home on the lot. We are therefore requesting a permit be issued for the
construction of a new well and septic system to accommodate the home. The attached
Site Plan and backup documentation identify the location and configuration of the
proposed well and septic system along with the alternate site. Also identified on the
plans are the existing wells and septic systems on surrounding lots. No conflicts exist
between the proposed septic system and wells or septic systems on adjacent lots.
Drainage arrows are shown on the site plan showing the grade and direction of flow on
the lot. The drainage pattern will not be changed as a result of this project.
Five test holes were placed on the lot in the area of the absorption trenches and
alternate site. All indicated a poorly graded gravel with silt (SP/SM) material with some
sand. The percolation rate of the soil was determined to be between 6 and 15 minutes
per inch. No groundwater was found during the placement of the test holes, but was
noted at 7' below the surface during the monitoring period, which included May 15,
2005. An application rate of .8 gallons per day per square foot was used in the design
of the absorption system. We are proposing to place a new 2,000 -gallon septic tank
and 4 - 50' long by 5' wide by 1' effective depth absorption trenches to treat and
dispose of the effluent generated in the house. The elevation of the distribution line in
the absorption trenches will be 1' below the surface and the total depth will be 2'. A
minimum of 3' of fill or 2" of insulation and 2' of fill will be placed over the absorption
trench and 4' over the septic tank. A total of 3 flow splitter valves will be placed to
insure even effluent flow to each absorption trench.
The ground surface on the lot in the area of the septic system slopes to the west. The

new trenches will be constructed parallel with the slope as much as possible in
conformance with Municipal requirements. The new septic system will be constructed a
minimum of 100' from all wells and any surface water and 10' the proposed and existing
water services in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.
