Ref: Proposed Septic Tank Upgrade for Magnificent Mountain Slopes, Lot 4;
To whom it may concern:
The existing 2 bedroom house is served by a private well and an undocumented septic system. GEG, Ltd.
recently documented the drainfield (see attached documentation). Because of the tanks age and lack of
documentation, the MOA directed that it be replaced. We are proposing to remove the existing tank and
replace it with a new 1000 gallon septic tank at the location shown on the attached design.
A septic adequacy test was performed on 10/26/10. Prior to the start of the test the drainfield was dry.
Over a period of 201 minutes, a total of' 840 gallons of liquid was introduced into the drainfield. It was
noted that the liquid depth in the drainfield rose 19 inches. The drainfield was checked 823 minutes later
and the liquid level in the drainfield was dry. Based upon this data, the drainfield meets the MOA
requirements for a 2 bedroom septic system.
During the excavation of the test hole, large boulders were observed at a depth of 14.5'. These boulders
were noted to be in excess of 2'-2.5' in diameter with layers of dirt between them. The excavator then
attempted to dig test holes in two additional locations and the large boulders were again encountered at
approximately the same depth (14.5'). These rocks could not easily be removed by the equipment due to
their large size. It is important to note that the boulders did not appear to be fractured bedrock, but smooth
rocks. It is reasonable to believe that given the boulders large size, the presence of bedrock does not exist
to a depth of at least 16.5'.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Ref: Waiver Request for Reduced Separation Distance Between a Drainfield & a Cutbank
for Magnificent Mountain Slope; Lot 4,
To whom it may concern:
The existing 2 bedroom house is served by a private well and septic system. The septic system is
has been recently documented. The drainfield is too close to an existing cutbank so a waiver is
needed. We are requesting a waiver be granted from the drainfield to the cutbank. As can be seen
on the attached drawing, at a 25% slope (starting 2.5 feet above the drainfield) the potential point
of daylighting is greater than 35' horizontally from the trench. The following are mitigating
factors for the justification of this waiver:
• The encroachment has existed for over 20 years.
• There is no sign that there has ever been any effluent daylighting now, or in the past.
• There is no reason to assume that daylighting will occur in the future.
• The slope area is heavily vegetated.
In short there appears to be minimal risk associated with the granting of this waiver.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As codified by troy seal affixed hereto and as of the validation date shown below, i verify that my
investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. i further verify that based on the
information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.
