A test hole was excavated and a percolation t~st p~rformed. The approximate location of the test hole is located on the attached site plan. The monitoring tube within the test hole has been checked and found to be dry. Attached is the proposed upgrade design. This property 2~ service by a Community Water System. If the integrity of the existing septic tank is poor the existing septic tank is to b6 excavated, pumped, crushed, and abandoned in place and a new 1000 gallon septic tank installed. Construction shat~ b6 in accordance with the approved site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wustewater Disposal Regulations. 3. ]'he contractor shall be responsible for obtaining any necessary underground ut~y locates. U~less specifically agreed otherwise, the property owner shat~ be responsible for final grading areas subsequently depressed from soil s~tling. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. 0wn~rs instaIZing their own systems must also receive prior approval from the Municipal Health Department. SEPTIC TANK INSTALLATION: A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. 2. The septic tank sha/l be sufficiently bedded to prevent settling or shifting of the tank. 3. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfi~ld there shall be two adjacent ~eanouts (u~ess an effluent pumping system exists within the septic tank). These cleanout~ shall be located on undisturbed soil not more than 10' from the tank. The first cleanout, in line, sha~ be to clean toward the leachfield. The second cleanout shall be to ~ean toward the septic tank. Final grading over the septic tank shall be such that a positive slope exist~ away from the septic tank. ABSORPTION TRENCH/DRAINFIELD INSTALLATION: Excavate the proposed t~ench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level· If the sidewalls of the excavation become smeared, they must be raked or scratched (roughed-up) before gravel (sewer rock) placement. Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. A silt barrier must be installed between the final gravel layer and the native soil backfit~. Ensure the sdlt barrier covers the entire gravel surface before placing backfill. Monitor tubes shall be of four (4) inch diameter and installed approximately in the locatio~ shown on the design. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as noted on the design. MINIMUM Backfill over the final gravel layer must not be less than twenty-four (24) inches· I~ulation must be installed when the backfill depth ~ less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a depression after settling. MATERIAL SPE~IFI~ATIONS: I. Any septic tank proposed for installation must be constru~ed by a Municipally approved septic tank manufacturer. Page Three Lot 7; Block 3; Meadow Brook Subdivision February 2, 1995 2. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). Septic tank i~lets and out~et~ shall be fitted with watertight couplings (Cau~der, Fernco, or equal). A permeable nontoxic silt barrier (Typar 3401, Mirafi 140/N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. All leachfield gravel (s~wer rock) shat~ be 0.5"-2.5" screened gravel with less than 3% passing the #200 sieve. When sand is being used as a filter material, its gradation specificatio~ must conform to current M.0.A. or D.E.C. requirements. INSPECTIONS: Typically there will be a minimam of three (3) inspections required during the installation of the wastewat~r disposal system. These inspections wd~l occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. Page Four Lot 7; Block 3; Meadow Brook Subdivision February 2, 1995 3. The final inspection is to occur upon final grading of the property. Often there wi~ be more than these 3 inspections required, especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre-construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractor's activities. The owner shat~ contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and wd~l inspect the work as stated above to document the co,actor's activities. Final acceptance of the contractor's work rests with the owner and the M.0.A. S & S Engineering shall have no liability to the owner or to others for acts or omissions of the contractor or any other perso~ performing work on this project or the failure of the contractor to carry out the work in accordance with these construction documents. S & $ ~ngineering's inspecting engineer will not be responsible for the construction means, m~thods, techniques, sequence, procedures or the safety precautions incident to this project. BLK 3 Meadow Brook $/D May 30, 31 and June 1, 1984 System Deseription~ The onsite disposal system consists of a 1000 gallon septic tank and a subsurface absorption trench. The orginal system was installed in July 1975 and upgraded in November 1980. The system serves a three bedroom house. Water is supplied to the house from the Municipality of Anchorage-Eagle River water system (PWS ID # 210859). Test Procedure: The water use in the 24 hour period ~rior to the start of the test was estimated to be 30 gallons. The static water level in the absorEion trench was at the same level as the water level in the septic tank before the test. A surge load of 500 gallons per day was added to the system for the first two days o[ the test. The water level in the septic tank and the absortion field raised 4" and returned to the static water level within 24 hours. Based on the length of the trench as shown on the Nov. 29, 1980 asbuilt the absorption capacity of the soil was determaned to be eight gallons per square foot per day. The septic tank was pumped and verified to have a capacity of.lO00 gallons. The absorption trench w~s charged by a continuous flow of water over a period of 48 hours. At '~he end of the test period it ~s concluded that the absorption trench would adequately accept approximately 150 gallons for a 24 hour period. The septic tank is adequate for the three bedroom residence; however, I regret to inform you that the absorption trench is only adequate for one bedroom. It will be necessary to upgrade the trench to accommodate two additional bedrooms. In reference to ot~r telephone conversation and my conversation with Norman Brake on November 25, 1980', I will make arrangements to obtain a permit from the Municipality and to have the system upgraded.
