As you are aware, I did inspect the construction of the system which appeared to substantially conform to the design size, DHHS materials specifications and construction installation requirements. However, because you elected to perform your own survey layout and establish the system elevations, I was unable to "Asbuilt" the system during installation. The following day when I came back to do the "Asbuilt" survey, I found that you had glued caps on all the cleanouts making it impossible to establish the elevations of the system. As you might remember, at that time I requested the caps be cut off and the proper, required ones be installed (which would also allow me access to "Asbuilt" the elevations) and that the final grading needed to be completed over the system. You explained to me that you didn't want to put removable caps on because the house was not built yet, would not be lived in for several months and you didn't want any one tampering with the system. In January and again in April I mentioned that the glued on caps needed to be replaced. The first of May, I was given short notice on a move to Kotzebue and drove by and found that the caps and final grading still not complete. I'm not sure when I'll be back in the area to complete an "Asbuilt" of the system and would recommend that you hire another Engineer to complete an "Asbuilt" of the system. If you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit; $120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit. Re: L 3 B3 McKinley View Estates Subdivision The attached Site Plan shows the proposed and alternate septic system locations for the subject lot and the existing septic systems on the adjacent lots. WELL IMPACT This subdivision is on a public water system. SEPTIC SYSTEM IMPACT The ground water is fairly high in this area the surrounding lots recorded ground water at 7'. The ground water in the area of the proposed system was recorded at 5.5' during the seasonal high in May. The alternate system recorded a seasonal ground water high at 3.5' and would require a mound system design if needed in the future. The receiving ground strata varies from a silty sand to a dense clayey gravelly till with perc rates varying from 10 min/inch to 60 min/inch. These materials and perc rates should provide adequate treatment of the effluent before reaching the ground water. There isn't any open water bodies in the area that are fed from the ground water. RESERVE SYSTEM There is adequate space for an alternate system as shown on the site plan, however it would need to be mounded to protect the ground water. DRAINAGE The lot slopes down to the NNW ranging from 10% to 2.5% with no cut banks. Box 671489 Chugiak, Alaska 99567 Subject: Septic Adequacy Test at Lot 3; Block 3; McKinley View Estates S/D 22730 Seneca Drive Dear Mr. & Mrs. Buyse, Per your request, we performed an adequacy test on the subject septic system. Our findings are summarized as follows: The septic system at Lot 3, Block 3, McKinley View Estates Subdivision was installed in September of 1995. Inspections of the septic installations were performed by Harry Bates, P.E. and the As-built documentation was performed by James F. Sizemore, P.E. It is our understanding that the As-built drawing was either not submitted to the M.O.A. or lost and that Mr. Sizemore will be re-submitting the drawings today (7/7/99). On the day of our inspection, 7/6/99, the monitoring tubes in the drainfield were checked and all were found to be dry. The septic tank was pumped by J.R's Pumping prior to the test. Seven-hundred & thirty-six (736) gallons of water was introduced into one of the monitoring tubes in the drainfield caused the liquid level to rise two inches. Five (5) minutes after the introduction of water was stopped, the monitoring tube was checked and found to be dry. Based upon this data, it can be concluded that the drainfield has an absorption capacity of greater than 450 gallons per day, as required for a three (3) bedroom house. The property is served by a community water system. Prior to submitting the necessary paperwork to the MOA to obtain a Health Authority Approval HAA), which is required for the sale of the home, we will need confirmation that the As-built drawing of the septic system has been submitted. DHHS Permit No. SW950131, PiN No. 051-792-35 Dear Doug, Recently I received a copy of a notice from the MOA DHHS that the permit has expired for the installation of the septic system that I desi~iied and you installed last fall. As you are aware, I did inspect the construction of the system which appeared to substantially conform to the design size, DHHS materials specifications and construction installation requirements. However, because you elected to perform your own survey layout and establish the system elevations, I was unable to "Asbuilt" the system during installation. The following day when I came back to do the "Asbuilt" survey, I found that you had glued caps on all the cleanouts making It impossible to establish the elevations of the system. As you might remember, At that time I requested the caps be cut off and the proper, required ones be installed (which would also allow me access to "Asbuilt" the elevations) and that the final grading needed to be completed over the system. You explained to me that you didn't want to put removable caps on because the house was not built yet, would not be lived in for several months and you didn't want any one tampering with the system. In January and again in April I mentioned that the glued on caps weeded to be replaced. The first of May, I was given short notice on a move to Kotzebue and drove Dy and found that the caps and final grading still not complete. I'm not sure when I'll be back in the area to' complete an "Asbuilt" of the system and would recommend that you hire another Engineer to complete an "Asbuilt" of the system. Sincerely, Harry R. J.
