investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate

information obtained from the Municipality of Anchorage files and from my investigation and inspection, the

guidelines and regulations established by the Municipality of Anchorage and indusby practices. The reported results describe the
condition of the systemAs on the decals of the evaluation. separation distances were measured to maddy identifiable features.
Hidden defects or encroachmentsmay exist that werenetidentified during the evaluation. The operational Nis of all wefts and sopa
systems depend on variety of variables inducting, but not limited to, soil conditions, groundviate( levels (that may fluctuate dunn
the year), quatdyof constmdlon (materials and workmanship), and the water usage of the family utilizing the systeads. These
conddions can vary, and are outside the control of GEG. satisfactory test results do not guarantee future pedormance of the

• Per Mr. Pannone's soil percolation test, the allowable soil application rate is 0.8 gpd/sq.ft.
• The owners are requesting a four (4) bedroom COSA which assumes a peak design flow of 600 gallons per day
• Per the 1970 MOA inspection report, the existing seepage pit is 60 feet long, 10 feet wide, and has an effective
depth of 6 feet for a total absorption area of 840 sq.ft
• 840 sq.ft and 0.8 gpd/sq.ft. equates to 672 gallons per day peak flow
Based upon Pannone's percolation test results, and the above calculations, the existing seepage pit is adequately sized
for four (4) bedrooms.
