Ref: Proposed Septic Upgrade for McMahon gl; Lot 4, Block 7,
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The septic system is in a state
of failure and needs to be upgrade. We are proposing to install a deep trench type drainfield. One test hole
was excavated on the property. The drainfield will be designed around the 30 foot radius ofthis test hole.
Comments regarding the design are summarized as follows:
I. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPiiY: As can be seen on the attached design drawing the average topography below the
drainfield is a 5-10 percent slope running approximately south to north.
5. LOT LINE WAIVER: We are requesting a 0 foot lot line waiver from the proposed drainfield to the
north lot line. The property line abuts a road right-of-way. As a result, there are no encroachment
concerns,
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
t,ffe sole benefit of the owner listed above. Any reliance upon or use of this report by any

Nitrate Advisory
Certificate of On-Site Systems Approval # OSC111126
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 7, Lot 4 of
McMahon #1 subdivision. This inspection revealed a nitrate concentration of
10.7 milligrams per liter (mg/L) was reported for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 mg/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

The primary drainfield on the property was installed on 7/23/2008. It was installed and documented by
GEG, Ltd. Per our as-built drawings, there were two monitoring tubes installed in the drainfield. On
3/18/2011, we inspected the property for the purpose of obtaining a C.O.S.A. During our inspection, only
one monitoring tube could be located. After we had installed the system, the owner had arranged
landscaping work that apparently concealed the east monitoring tube. Since only one monitoring tube is
necessary, we request you approve the system in its current condition, noting that one pipe will be
missing from the current as-built survey.

Well was inspected by camera down to 43'. No perforations or leaks were found. Well
was then nm for 20 hours and re-sampled for nitrates. Second nitrate test came in at 8.79

FLOWED INTO THE CRIB. ON SEPTEMBER 4, WATER WAS AGAIN ADDED TO
THE TRENCH AND THE WALER LEVEL MONITORED.A TOTAL OF 400 GALLONS
WERE ADDED CAUSING A RISE OF 7 INCHES IN THE WATER LEVEL. AFTER 4
HOURS THE WATER LEVEL HAD SUBSIDED TWO INCHES INDICATING THAT 100
GALLONS HAD BEEN ABSORBED. EXTENDED TO A 24 HOUR PERIOD THIS IS A
DAILY RATE OF 600 GALLONS.
TEST RESULT: THIS SYSTEM MEETS THE CODE REQUIREMENTS OF
iTHE MUNICIPALITY OF ANCHORAGE.
The operational life of all septic systems depends on the local
soil conditions, groundwater Ievels that may fluctuate during the
year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator
of this septic system. We can therefore not give any estimate of
how long the system will continue to meet the operational requi-
rements of the Municipality and State.

The Municipal requirement for well flow is
150 gallons of water per bedroom per 24
hours.This well surpasses this requirement.
The assessment of the condition of this well
applies only to the conditions as of this
date. The flow rate of the well may change
due to subsurface conditions that may not be
observed from the surface, and changes in
land use and other factors that may impact
the conditions of the aquifer feeding the
well.

Dear ~lr. Speziale:
Per your request of July 7, 1978, we conducted a test of the sanitary
sewer system on the above described property.
The septic tank was pumped and water was added to the log crib. During
the test, the liquid level in the crib was measured before and after the
addition of 200 gallons of water at which point the crib was full as
indicated by a backflow of water into the septic tank. The total depth
of the crib was 11.4 feet. All liquid levels were measured below the
top of the standpipe and are summarized in the following table:

The water level rose 1.9 feet or 22.8 inches with the addition of 200
gallons of water, indicating an acceptance rate of 8.8 gallons per inch
in the crib. Twenty-four hours later the liquid level was again measured
and found to be at 6.6 feet. It had dropped 36 inches. This indicates
an average effluent acceptance rate of 316.8 gallons per day for the
surrounding soils. If the three bedroom residence on the property is to
house 6 people, the average load on the system can be expected to be 450
gallons per day. We can, therefore, conclude that the system is not
accepting effluent at an adequate rate for a 3 bedroom residence.
