Septic System Permit Upgrade Request, Revised
Ladies and Gentlemen:
I am writing to request a permit to construct an upgraded septic system be issued for this property. The proposed
systems will serve an existing three-bedroom house. Currently the lot is developed. The lot is served by a private
well. The existing septic system is in failure. The surrounding lots are served by private wells.
1. Soils. A single test hole was excavated by Pannone Engineering Service in May of 2011. See the soil log
attached. Ground water was monitored for seven days. No ground water was monitored to a depth of 14.5 feet
below ground level after the monitoring period. Bedrock was not encountered in the test hole. It is my opinion,
based on the results of the percolation tests and overall soils appearance; an application rate of 0.8
gallons/day/square feet should be used, using a 1,250 gallon S.T.E.P. tank and deep trench type drain field.
Soil Absorption System Design.

3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and the existing drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: The topography in the area slopes generally from north to south. There is a high relatively
flat area where the purposed drain field will be located. Just uphill from the tank there is a fill slope that is over 50
feet from the purposed drain field.
So
Waiver Request: I request the following waiver:
a. Well to Septic Tank/Lift Station from 100 feet to 60 feet.
The existing drain field serving this lot is located over the property line and is in failure. It is located approximately
90 feet from the well. The existing 1000 gallon tank is located 63 feet from the well. Both will be abandoned per
code. A water sample was taken and analyzed on May 6, 2011 and found to have nitrates of 0.413 mg/L. The
purposed S.T.E.P. tank will be located 60 feet from the well, and will be coated to STi-P3, Underground Fuel Tank
Standards, minus the sacrificial anode. There is an area to the south west of the well that is outside the well radius.
We looked at installing the system in this area and found it unsuitable for the following reasons: 1) The well is 24 feet

from the house, there is not enough room to install a sewer line outside the 25 foot protective radius. It is easier to
control the coating on a tank, than to protect the raw sewage line from leaking; 2) The area slopes at approximately
six percent and the proposed tank and drain field would not fit in this area and meet code; 3) installing a S.T.E.P. tank
in this area would require pumping the effluent approximately 300 feet, uphill and across a drive way. There would
be possibilities of freezing and clogging over that length. The soils we encountered in the area of the drain field
(uphill from the tank) were sandy, gravely, silts to 14.5 feet below ground with a percolation rate of 3.8 minutes per
inch. The ground at the tank is located approximately level to the ground at the well casing and contamination is
unlikely. The well is located on the southern portion of the lot. The static water level in the well is 220 feet below
the top of the casing (218 feet below ground level). The hydraulic gradient is 6 to 7 percent from northeast to
southwest. Approving the waiver will not create a health problem for this or the surrounding lots.
In review of the ADEC Waiver Memorandum the following points are tabulated:

16-25 points: Almost sure to be free from any form of contamination from household sewage.
6. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the
review is complete and that there are no further comments is received from MoA On-Site Department, the note will
be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There are no
wells or septic systems within 100 feet of the proposed septic location.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined In the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with ail applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition; ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
