The original design for the absorption trench upgrade on the subject lot required an 8'
effective depth deep trench. During construction, however, very clean sand was found
within 5' of the original ground surface which caused the trench to slough and cave in. It
was not possible to achieve a depth of 14' in the clean sand. The trench was therefore
redesigned for a 5' wide by 4' effective depth by 39' long absorption trench. The effective
absorption area is the same as the trench originally designed for the tin'ce bedroom home.
Our soils log subnfitted with the design shows the clean sand beginning at the 9' level.
Excavations for the absorption trench revealed this level at 5' to 6' below the surface. The
entire trench is constructed in the clean sand layer. The application rate for this material is
1.2 gallons per day per square foot.
The trench extends 8.5' into the driveway serving the house. The distribution line 6'
below the driveway surface is covered with 2" of direct bury insulation. A cleanout was
placed at the driveways edge with a sweep 90 fitting to the line. The line can be cleaned
either ttn'ough the diverter valve or in both directions through the sweep 90. We am
confident the absorption trench as constructed will effectively treat the effluent generated in
the septic tank and will be easily maintainable with the cleanouts in the configuration that
they were placed.
The owner of the home is anxious to complete the closing transaction on the property. We
appreciate your expedited review of the attached information so the Health Authority
Approval Certification can be issued. Thank you for your cooperation with the upgrade of
the septic system on this lot.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The absorption trench on Lot 27, Block 1, Hidden Hills Subdivision has
failed and must be replaced. We are therefore applying for a permit to
construct a new absorption trench on the lot to serve the three bedroom
home. The attached site plan and backup documentation identify the size
and location of the new absorption trench designed to serve the house. It
also shows the location of the existing wells and their respective 100'
protective radii.
One testhole was placed on the lot at the locations shown. Silty sand was
found to a depth of 9' which was underlain by aclean well graded sand to
the bottom of the hole. Percolation tests completed indicated rates in the
silty sand of 17 minutes per inch and faster than 1 minute per inch in the
clean sand layer. The average absorption rate for the new trench based on
the two percolation rates is 1 gallon per square foot per day. No
groundwater was encountered nor was any measured during the monitoring
period. We are therefore proposing to construct a deep trench system
with 8' effective depth. Total length will be 29' with the distribution
piping at 6' below the existing ground surface. The total depth of the
system will be 14' below the existing ground surface. A diversion valve
will be installed and connected to the existing trench for possible use in
the future. The existing septic tank has been uncovered and inspected and
is suitable for continued use.
The surface of the lot is fairly flat with only a minor slope from east to

west. No conflicts will exist between the new system and wells in the
area.
If the system is constructed as designed the following statements apply:

CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 27, BLOCK 1, HIDDEN HILLS SUBDIVISION
GENERAL:
1. The scope of this project includes the construction of A
new 29' long by 3' wide by 8' effective depth absorption
trench at the location shown on the site plan. The
distribution piping must be placed at 6' below the original
ground surface. Total depth of the trench will be 14' from
the original ground surface. Grade finished surface to drain
away from trench. Two post tank cleanouts and a diversion
valve must be installed after the septic tank at the location
shown.
2. Construction shall be in accordance with the approved site
plan, design drawings, Municipal Permit with any special
provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.
3. The Contractor shall be responsible for obtaining all
underground utility locates and for the layout of the septic
system and verification of the location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall
be responsible for final grading areas subsequently
depressed from soil settling. Property owner shall be
responsible for revegetation of affected areas unless
specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must
be certified by the Municipal Department of Health and
Human Services for system installations. Owners installing
their own systems must receive prior approval from D.H.H.S.
before beginning system installation.
SEPTIC TANK INSTALLATION
The septic tank was recently uncovered and inspected. It
has been found suitable for continued use. No septic tank
will be placed at this time.

2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts
for pumping access.
3. The septic tank shall be sufficiently bedded to prevent
settling or shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of
12 inches above final grade.
5. Tanks installed without 4' of cover shall have a minimum of
2" of direct burial insulation.
6. A foundation cleanout shall be installed one to four feet
from the building foundation. Two cleanouts are required
between the tank and the drainfield.
7. Final grading over the tank shall be such that a positive
slope exists away from the septic tank.
DRAINFIEI..D CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown
on the design. The bottom of the trench shall be within 2"
of level.
2. Distribution piping must be placed level with perforations
down atop alevel bed of drainfield rock. Rock should then
be placed over the pipe to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between
the drainfield rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the
locations shown on the design. The portion below ground
must be perforated.

5. Contractor shall verify the septic tank and drainfield are a
minimum 100' away from any private water wells in the
area, 150' from a Class "C" Well or 200' from any community
well.
6. Direct bury insulation must be placed over the distribution
system if less than 3' of backfill depth is available. Finish
grade over the trench must be mounded to prevent
settlement or depressions.
7. Grade area surrounding the absorption trenches to drain
away.
8. A minimum 2' of accepting soil is required below the
drainfield rock for a 5' wide trench. Contractor shall verify
this condition prior to placement of the rock. All pockets of
unacceptable materials must be removed and replaced.
MATERIAL, SPECIFICATIONS:
1. Septic tanks must be constructed by a Municipally approved
septic tank manufacturer.
2. The following pipe materials are approved for use in septic
system installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C.
(perforated and solid), ASTM F810 or H.D.P.E. (perforated,
but not solid) and ASTM D2662 or A.B.S. (perforated and
solid).
3. Insulation shall be at least 2" thick extruded direct burial
polystyrene (Dow Chemical Co. Styrofoam I--II or equal).
4. Septic tank inlets and outlets shall be fitted
watertight couplings (Caulder, Fernco, or equal).

To whom it may concern:
The current Nitrate-nitrogen concentration for the well serving
the subject lot is 5.8 mg/1. EPA maximum contaminate
concentration for Nitrate-nitrogen is 10 mg/1. Water samples
obtained in October 1990, indicated a Nitrate-nitrogen
contaminant ].evel of 11.9 mg/1.
In order to receive an unconditional approval additional water
samples must be obtained and the Nitrate-nitrogen level
determined. Samples will by taken every thirty (30) days
between April 1, 1992 and June 1, 1992. The results of each
test will be recorded with this office, a final report will be
submitted no later than June 15, 1992. Should the
Nitrate-nitrogen concentration rise to 10 mg/1 the conditional
approval will be voided. It may be necessary to deepen or
possibly abandon the existing well.
This conditional approval expires on June 15, 1992.

TEST PROCEDURE: Well was pumped at a constant rate while the drawdown was monitored
with an accoustic probe. At the be.q. inning of the test the water level was found at 125 feet below
the top of casing As soon as the pump started the water level dropped two feet and remained at
i 27 feet 1or the duration of then test, 2 hours. As soon as the pump stopped the water level
recovered l.o 125.
lEST FOR E.COLI AND TOTAL NITROGEN: A sample of water taken from the well was tested
for' E.Coll and nitrates on October 12 1990 0 E.Coli, 11.9 Mg./1 Nitrates

TEST RESULTS: This well meets the flow requirement of the Municipality of
Anchorage, but the Nitr~te measurement is above the established safe drinking
water limit. This result ls quite unexpected and may be a caused by non-use of
the well. The cause for the nitrate concentration must be found and removed
before this well can be approved by the Municipality.
THIS WELL WILL PRODUCE MORE THAN ~ GALLONS PER MINUTE FOR MORE THAN
FOUR HOURS,
The Municipal requirement for' well flow is 150 gallons of water per bedroom per day, This
well exceeds this requirement. The assessment of the condition of the well applies only to the
conditions as they were on the day the well was tested. The flow rate may change due to
subsurface conditions that can not be observed from the surface, or from changes in the land use
or other' factors that rrlLT¢ impact the aquifer feeding the well.

TEST PROCEDURE: System was inspected and measured on October 12, 1990. Tank was found
with 5 feet of cover and 46 inches of liquid. Cleanout to trench was found plugged at the tee
with the distribution pipe. A 2-inch steel monitor was installed approximately 3 feet into the
gravel.
800 gallons of clean water was added to the trench while the waterlevels in the tank and
monitor' tube were monitored. The water level in the tank did not rise while a water depth of 60
Inches was measured in the monitor. 24 hours later the water depth was 3 inches.
On October 13 another 500 gallons of water was added to the trench, Again the water level rose
to 71 ~nches. 24 hours later the measured water depth was 3 inches.
TEST RESULTS: Tills septic system meets the code requirements of the Municipality of
Anchorage.
The operational life of all septic systems depends on the local soil conditions, groundwater
levels that may fluctuate during the year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator of this system. We can
therefor-e not give any estimate of how long this system will function satisfactory for' current or
future occupants.
