Permit #900173, PID #020-451-01
The subject permit, issued by this office for a single family
well and/or on-site wastewater system has expired as of December
31, 1990.
A new permit must be obtained from this office for a well and/or
on-site wastewater system not installed by the expiration date.
If you have drilled the well, a well log needs fo be sent to
this office for documentation of the installation and to close
the permit.
If a private engineer inspected the installation of the on-site
wastewater system, the original as-built inspection report
(three-part form) must be sent to this office for review,
approval and documentation. All inspection reports must be
submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $90.00 for an
on-site wastewater permit; $50.00 for a well permit; $].40.00 for
a combined on-sit:e wastewater and well permit.

The original design ¥or this system was placed in an area o.F high
water level, The new system ie similar in design but placed in
an area o-F lower water depth. Following is a cemparison between
the wat~, monitoring tubes at both the original location and the
new location.

Based upon this comparison, the high water level o¥ the old hole,
(2.7' depth) plus the minimum water height di-FYerence between the
old and new monitor tube locations (2.2') results in an estimated
least depth to water level at the new hole o-F 4,9' This design
sets the bottom e~: pipe 4.4' above that level.

1. Second Post Tank Clean-out -- Due to the installation date this cleanout is not required.
2. Cleanout at north end of bed was installed
3. The water monitor iuformation was included in the detailed presentation to Mr. Robbie
Robinson. Please let me know if you need any additional beyond this information.
This should include the information you requested and allow the as-built to be filed. Please
let me know as soon as possible if additional input is needed.

SUBJECT: TRACT D, HILLSBORO, As-Built Drawings
Dear Mr. Smith:
Attached is the as-built drawings for the septic system installed
at the above location. The system is currently providing service
to the garage. The owner intends to construct a house this year
and will connect the house to the tee at the Clean-out just
before the tank area.
I performed the original design and inspections on this system.
Since that time I lost the original field notes. I have
measured the system to the best of my ability to produce the
attached drawings. I have also included copies of the original
well log, the initial water tests, along with a recent water test
report. I would appreciate your review and approval of the
submitted as-built. If you have any additional questions or
comments, please don't hesitate to contact us.
