Subject: Henkins Subdivision, Block 3 Lot 15 - Henkins Road
Septic system design
Dear On-Site Services Engineer:
The owner of the above lot would like to replace their aging holding tank, so we are submitting
this permit application for the construction of a new advanced treatment septic system. The
attached site plan identifies the location of the home, well, and septic location.
The ground surface on the lot slopes very mildly toward the east at between a 1-5% slope. There
are no slopes greater than 25% within 50 feet downslope of either the primary or alternate site.
Stormwater drainage will not impact this septic system. The new trench will be constructed
parallel to the slope as much as possible. Wells on this and adjacent lots are shown. No conflicts
exist between this proposed system and any other well or septic system, whether on this lot or
adjacent lots. The new system will be a minimum of 50’ from all wells and surface water, and
more than 5’ away from the septic tank.
Please refer to the attached test hole log, plan and profile pages for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

Please refer to your files on the referenced property for the history
of attempts to achieve a suitable on-site wastawater disposal system to
serve the 3 bedroom, single family dwelling located on this property.
The property owner, A.H.F.C. has elected to abandon efforts to insta~
an innovative on-site system on this property. Instead, they w~sh to
install a 2000 gallon wast~wat~r holding tank as shown on the attached
site plan.
This property is generally flat with a gentle slope to the east. There
are a couple of small surface water sources influencing the lot as
shown on the site plan. The entire ar~a is known for its elevated
groundwater conditions especially during the spring.
The location of the holding tank as shown should not influence any of
the surrounding properties.

Dear Mr. Ward,
This l~tt~r is in response to your l~tter of October 2, 1991 on the
referenced property.
I received a call from W. J. Tack~t~ and Associates to inspect the
installation of a holding tank on the referenced property. When I
arrived at the time Mr. Tack,ti had designate~ he still did not have
the holding tank installed as expected. Instead, he had a hole
excavated where he had proposed the tank to be placed. At that ~ime I
went over the following deficiencies with Mr. Tackett.
The hole excavated for the holding tank was located only
approximately 70 ft. from a stream locatzd near the property.
This stream is d~signated in our design which Mr. Tack,ti had
in his possession. On the design we showed the proposed tank
to be located at least 100 ft. from the stream. When I
informed Mr. Tackett of the required separation distance, he
expressed some concern over the location of the sou~h property
line and whether or not the tank cou~_d be plac~d 100 ft. from
the stream and still r~ma/n on the property. At that time we
took some measurements and found the property line to be near a
power pole which would allow more than enough room on the
property to s~t the holding tank. At that time we designated
on the ground where the n~w hole was to be excavated for the
installation of the holding tank. Mr. Tack~tt agreed to this
position.
Tack,ti and Associates, Inc. was not on the Department of
Health and Human'Services approved excavators list and prior to
our approval, he m~st be placed on the list of approved septic
system installers.

3. Fhe sewer pipe which Mr. Tackett was using was not of an approved
mo~al and needed to be replaced.
4. Mr. Tacket~t did not have a high water alarm for the holding tank as
required by Municipal Ordinance.
The holding tank did not have a 6 inch ~eano~t as required by
Municipal Ordinance.
During this first inSpection Mr. Tack~ expressed some concern over the
ac~urancy of our site plan. I explained to him that I felt he was being
deceived by the way the house was angled on the property, and that he had
plenty of room to install the holding tank and maintain a~l proper s~tbacks.
To the best of my knowledge the site plan we dev~oped is compl~ely accurate
and it appears to be "somewhat deceiving" to only Mr. TackieSt.
A ~ew days later Mr. Tack~tt called and stated that all the concerns of our
first inSpection have be~n resolved, the tank is s~t, and he was ready to
backfill. I then contacted a representative from this firm who was in the area
to inspect the tank so it could be backfd21ed. My Inspector arrived thereafter
and found they were only finishing the excavation of the new hole for the
holding tank. Mr. Tackett apologized for not being finished and they discussed
the difficulties of the job due to the muddy high water conditions. Mr.
tack,ti was to call us when he was ready for an inspection.
Once again Mr. Tackett called for an inspection. When my Inspector arrived on
the property, he found the holding tank had already been covered. When
questioned, Mr. racket~ said he had to cover the tank prior to inspection due
to the high water conditions which caused the holding tank to keep floo~ng and
shifting out of position. We did not take measurements at this inspection
since the tank was already inStalled, the site was extremely muddy, and the
equipment on the property would hinder the measurements.
Once Mr. Tack,ti was off the prope~y, he called us for a le~t~r of comple~ion.
We went out to take the final measurements and found the tank was instated
only 92 ft. from the creek. I called Mr. Tack,it on Septemb~ 26, 1991 and
asked him why the tank was not set 100 ft. from the creek as we had agreed. He
stated that he was afraid of encroaching onto the property line a~d figured we
could simply g~t a waiver. When I told him the cost and difficulties in
obt~ning a waiver, he said he would talk to Bob MonSon and g~ back to me.
I fe~l the inStallation of the watt,water holding tank within the creek s~back
was either a result of Mr. Tack~t's attempt to deceive my inspector or due to
Mr. Tack,it's lack of understanding of the importance of the separation
distance requirement. At any rate, I never approved the inStallation of the
holding tank in its present location, in fact, Mr. Tack~tt was. well informed of
the required separation distanae both v~rb~y and by our site plan.
Therefore, I do not fe~l that we should be h~ld responSible for the improper
installation of the holding tank.

Furthermore, sinc~ th~r~ is littl~ r~ason why the holding tank cannot
located I00 ft. from th~ cre~k, we cannot justify the issuance of a waiver.
Therefore, obtaining a waiver for th~ l~ss~r separation distance may not
possible.

This letter is in response to the letter from S & S Engineering dated
October 3, 1991. We would like to clarify what we believe are incorrect
statements within the five noted deficiencies and in the body of the letter:
Deficiencies:
1. Our records indicate that no measurements were taken to locate the
property line and that if exact designation was given for the location
of the tank, then we must have completely misunderstood, as no marking
or staking was used.
2. This is a correct statement. However, we have since obtained all
necessary permits and MOA approval for this work.
3. This is a correct statement. 'However, piping was changed per the
engineer's requirements,
4. This is an incorrect statement. The inspector for the engineer
requested that we move the alarm which we had installed from the inlet
end of the tank to the opposite end of the tank, which we did.
5. This is a correct statement. However, a 6" clean out was installed
per the engineer's requirements. ·
With reference to the site plan developed by the engineer as being
completely accurate, it would have eliminated the problem if we could have
determined the exact location of the tank placement with reference to not
only the creek, but also the property line. When the inspector arrived
and told us to backfill, we had already partially covered'the tank to hold
it down, but the connections and cleanouts were visible. We fully admit
that we are new to this type of excavation work, and that we made some
mistakes. It was never our intention, however, to deceive the inspector,

but rather our lack of understanding as to the exact placement location
of the tank and the fact that we were concerned about placing the'tank
on the neighbor's property.
We hereby rescind our position that the work was completed per the
engineer's requirements and stand ready to do whatever is required to
complete our contract. It seems redundant at this point to re-excmvate
the site and move the tank 8' if a waiver can be obtained from the health
department for the existing placement.

Request you issue a permit to install a recirculating sand filter
type wastewater disposal system., qn ~h.e ~ef~erence~d p~roperty.
The system consists of a 2500 gallon s~t~ng/recirculation tank.
Attached is a detailed drawing of the proposed tank by ANCHORAGE
TANK AND WELDING, INC. This tank size is to be sufficiency large
enough for a 3, 4 or 5 bedroom home.
Wastewater from the recirculation tank is pumped into a sand filter.
This filter has been designed using guidelines established by the
Environmental Protection Agency. (EPA Design Manual - On-site Wastewater
Treatment and Disposal Systems, 1980)
The filter will be contained in the lower half of an eight foot
diameter tank, 12 f~et long. Th~ bed area size is based on a hydraulic
loading of 5 GPD/Ft for a three bedroom house (450 GPD). The distribution
pipes will be embedded in a 6 inch layer of washed rock (3/4" - I~)
b~ow which will be a 2.5 foot sand filter. Below the sand filter
will be 4 inches of pea gravel. Below the pea gravel will be a
4" collector pipe embedded in 8 inches of washed rock (3/4" - I~").
The distribution pipe network will consist of 3 - 10 foot laterals
spaced 2 feet apart. The pipes will be I inch in diameter with
5 - ~ inch holes in each lateral. The proposed lateral discharge
pressure will be 5 feet of head. Given these parameters, the discharge
rate will be approximately 25 gallons per minute onto the sand filter.
The dosing volume is to be 120 gallons. This will equal a dose
of 2 inches over the sand fi~ter. The dosing interval is to be
30 minutes. Therefore, with a discharge rate of 25 gpm, the pump
within the recirculation tank is to be set on a t~mer to run for
approximately 5 minute, ~vo times per hour.
Once the water reaches a certain level within the recirculation
tank, an electric solenoid valve is opened and the treated water
is discharged into an existing drainfield. The drainfield was installed
and inspected in July, 1982 under permit #820604. The drainfield
is approximately 4 feet deep, 5 feet wide, 75 feet total length,
and has an effective depth of I foot. However, it encroaches the
water table, especially during periods of seasonal high groundwater.

The changes include a larger sand size specification to prevent
clogging of the filter media. We have also added an Ultra Violet
water treatment unit to kill micro-organisms which might not be
taken out by the larger sand filter media. We have deleted the
4 inch half rounds and installed infiltrators over the distribution
pipes to act as splash plates which will better distribute the effluent
over the sand f~ter.

outlined in our letters dated November 28, 1988 and December 5, 1988.

For each of the referenced properties we request you issue a permit
for the installation of an on-site wastewater disposal system utilizing
the Clearwater Waste System for treatment of the effluent prior to
disposal into an absorption area.
We are under the impression that you have given verbal approval of
the Clearwater Waste System so attached for each property is a site
plan showing the configuration of the wastewater disposal ..system and
other pertinent property features.
The reason each of these properties has been. selected for the Clearwater
Waste System is as follows:
LOT 2; TIMBERLINE:
On this property exists a house and a septic system which was permitted
and the installation approved in November of 1977. However, since
that time there has been a small body of flowing water discovered
to the north of the property which has been designated as a surface
water source. This surface water source is located just 34 ft. from
the existing leachfield. There is only one place on the property in
which a leachfield could be located 100 ft. from the surface water
source. (This is located in the Southeast quarter of the property).
However, a soils test in this area shows the soils unacceptable for
a standard on-site wastewater disposal system.
We had request'ed approval of an "innovative" recirculating sand filter
for this property, but due to approval delays we have switched to
the Clearwater System.
We now propose the inst~Zlation of a Clearwater System which will
discharge into a new absorption area. This system is unique in that
it wdll req~r¢ a separation distance waiver of 69 ft. between the
well and the e~ting septic tank. There will also need to be a waiver
of the distance from the new leachfield to the surface water source
of 66 ft. We feel this waiver should be granted because of the treatment
the wastewater will receive prior to being discharged into the leachfield.

There will also need to be a ~iver for the d~tance between the leachfield
and a cutbank to 10 ft. We feel this wuiv~r should be granted in part because
of the effluent treatment, but also since the leachfield will be installed
at a sufficient depth as not to allow the effluent to surface through the
cutbank.
LOT 11; BLOCK 9; RABBIT CREEK HEIGHTS:
On this property exists a 3 bedroom home which is nearing the completion
of its construction. Soils investigation on this property show the soils
to be unsuitable for a traditional septic system. The lot is highly vegetated
with soi~ consisting of a 2 ft..organic layer overlying a clayey, silty,
sandy grave~ layer. Groundwater level~ on the property were high with groundwater
periodically high enough to saturate the organic layer. The mounded absorption
bed type leachfield for this property is to be constructed above the ground
surface with a 4ft. intermittant sand filter layer which will bring the absorption
bed 4 ft. above the seasonally high groundwater level. This system is unique
in that it will require a waiver of the required 2 ft. of natural, permeable
receiving soil above the groundwater level.
This property is also one which was proposed for the "innovative~' recirculating
sand filter system.
LOT 15; BLOCK 3; HENKINS:
On this property exists a $ bedroom home with~a septic system which was permitted
and whose installation was approved in July of 1982. However, since that
time, groundwater monitoring in the area has shown groundwater levels as
shallow as I ft. below the ground surface. Installation of the Ridgewaste
System was approved for this property but construction was o~ly partially
completed before financial and other complications halted the p~oject. The
mounded absorption field for this property will be constructed by first removing
I ft. of organic overburden and then installation of a 4 ft. intermittant
sand filter. This will bring the absorption bed 4 ft. above the seasonally
high groundwater level.
This property is uniqu,¢ in that th~ proposed leachfield w~ be constructed
over the existing leachfield area. This property will also reqaire a waiver
of the required 2 ft. of natural, permeable receiving soil above the groundwater
level.
This property is a~o one which was proposed for the"innovative" recirculating
sand filter system.

On this property exists a 3 bedroom home. There exists no well or septic
system. Construction of the home was performed with the owner unaware that
a land use permit was required in their area. A soil test on the property
showed the soils on the east end of the property to be suitable for a conventional
septic system, however, there is an existing drainage ditchwith surface
· water in it which prevents a septic from being installed in that area due
to separation distance requirements. The soils on the rest of the property
have been found to be unacceptable for a conventional on-s~te wast~water
disposal system. The soils on the property consist of an organic layer approximately
2 ft. thick. Below th~ organic layer is a layer of clayey, silty, sandy
gravel. Groundwater appears to travel under the organics on and within the
underlying layer. Construction of the absorpiton area is to be performed
by removing the 2 ft. organic layer and installing a 4 ft. intermittant sand
filter which would bring the absorption bed 4 ft. above the groundwater level.
This property will a~o require.a waiver of the 2 ft. of natural, permeable,
receiving soil above the groundwater level.
Lots 4 & 5; BLOCK 6; RABBIT CREEK HEIGHTS:
On this property exists a small 2 bedroom house~ A land use permit was issued
for the property in 1980 with the understanding that there will be no water
and sewer faci~es. However, a well has been drilled and the owner now
wishes to install a wastewater disposal system. Soils investigation on the
property has found soils similar to that of Lot 3 ; Block 6; Mann #2. The
mounded absorption bed is to be located on the high portion of Lot 5. As
with Mann Subdivision #2, the 2 ft. organic layer is to be removed and a
4 ft. intermittantsand layer installed. This property will also require
a waiver of the 2 ft. of natural, permeable, receiving soil above the groundwater
level.
We feel the Clearwater Waste System should be treated as an "innovative"
system and each of the systems on these properties should be monitored closely
for a designated period of time with monthly inspections of the mechanical
operation. We al~o ~eel that discharge samples should be taken and tested
for various quality parameters as prescribed by your department. We feel
the sampling should begin after a "starting up" period and should end by
mid-summer of 1990. At that time the system ~ no longer be labeled "innovative"
and it wi~ be either approved or disapproved for general installation. Ending
the sampling period in mid-summer of 1990, will allow time for new installations
du~ing the 1990 construction season.

We are of the impression the Municipality of Anchorage has monies set aside
for innovative septic systems. We understand that some of this money was
used for the construction of two innovative peat systems installed within
the Municipality. Therefor, we ask that the Municipality bear the cost of
labor and lab fees for monitoring and effluent testing of the systems once
construction has been completed.
During construction of the proposed systems, Nick Zorich Jr. of Clearwater
Waste Systems, Inc. will be present for installer tra~ni, ng and construction

The five owners of the referenced properties have been made aware that the
proposed systems are innovative in nature and they will not be fully approved
un~il such time that your depart~ent~feels the mechanical functioning and
discharge characteristics of the systems is sufficient to warrant final approval.
As a final note, if the Clearwater System is~not 'found satisfactory enough
for fin~ approval, the contents ~f~ the ste~ tank which is lo hold the Clearwater
System is to be removed and a high water alarm installed at the owners expense.
At that time the septic tank and Clearwater tank combined will act as a holding
tank with a capacity in excess of 2000 gallons. With approval of the installation
of these Clearwater Systems we a~o ask that you approve holding tanks for
these properties in the event the Clearwater System is not found satisfactory.

One of the affected lots is only 14,365 s.f.
Water table has increased in recent years causing systems to
fail and sewage to leak onto the ground Surface. Lots most
affected are cross-hatched on Figure 1. Based on soil tests
conducted in 1982 on several of these lots, water levels have
increased 4-6 feet.
System failure in this case can contaminate the shallow water
table, and the potential exists for contaminating the deeper
drinking water aquifers although no problems have surfaced to
date.

to a combination of 3 factors;
Natural phenomena - A natural increase in the water table
due to changes in the hydrologic cycle. There is historic
evidence that the area affected was very wet, saturated by
ground water, prior to development. There are presently
many indications of saturated soil as evidenced by small
streams, springs, and weeps within the affected area.
Monitoring well data helped establish the boundaries of
the affected area.
A stream diversion around lots 8,9, and 10 may be a factor
in fixing the water table elevation on Lot 15, Block 3,
although the degree of impact is difficult to assess and
is not the sole factor.
Wastewater loading from septic systems probably aggravates
the situation by contributing to the water table rise.
Lots 12-15, Block 3 and Lots 5-6, Block 4 will be required to
abandon their existing systems and hook up to a holding tank
or package treatment plant (innovative) by November 1, 1985.
Lots 3-4, Block 4 will be required to raise the absorption
field to meet the proper separation distance to ground water.
Curtain drain systems are not viable and have been excluded as
an option.
High fecal coliform levels found in a monitoring well on Lot
3, Block 5 are currently being investigated. The water supply
well for this lot is in close proximity and is only drilled to
a depth of 44 feet with a static water level of 6 feet from
the ground surface.
