The proposed system will serve a 3 -bedroom single-family residence. The existing septic system is backing up and must be pumped on a frequent basis. A soils log, site plan, design drawings and construction specifications are enclosed for your review. Design Calcs: No ground water observed to a depth of 18.0 feet (6/2/09) Use 1000 gallon tank with a deep trench absorption field Soil Rating. Total area required: 250 x 3 = 750 sgft System: The existing septic tank was installed in 2003 and should be sound. We propose installing a diverter valve and a new deep trench absorption field. The absorption field will consist of 2 trenches 45 feet long and 2 feet wide and contain 5.5 feet (effective) of sewer rock. Soils: A test hole was excavated on June 2, 2009. See the attached soil logs. Ground water was monitored for through June 10'h, 2009. Ground water was not observed. Block 4, Lot I -South Anchorage -Waiver of Separation Distance to Allow the Construction of a Replacement Field for an Onsite Wastewater Soil Absorption System-ADEC Plan Tracking Number 7843 Dear Mr. Spurkland: On July 1, 2009 the Department received the final information for an engineering plan submittal for a waiver to an onlot wastewater soil absorption system (Deep Trench Type 5.5' effective depth) for the three bedroom home at Heritage Hills, Block 4, Lot 1. The Department has reviewed the engineered plans, your engineer's recommendation, the sampling results on the Class "C" source well, and the well logs of nearby wells indicating silty sands and clay layers between the source waters and the soil absorption system. Samples taken from the well were for nitrate (1.96 mg/1) and no detection of arsenic or bacteria. The following waiver is hereby established and approved. The distance from the existing source well, a Class "C" Public Water System to the deep trench type soil absorption system, is reduced from 150 feet to 120 feet to the closest point of the deep trench. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195-18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within 15 days of receiving the decision. The owner shall contract with a contractor to perform the work outlined in these specifications and plans in accordance with the attached MOA permit. There is not contractual arrangement between the contractor and the Engineer. The Engineer shall be the owner's representative and will inspect the work to document the contractors activities. Final acceptance of the contractors work rests with the owner and the MOA. 2. The scope of the project includes the procurement and installation of a 3 - bedroom on-site wastewater disposal system at the location depicted on the attached site plan drawings. The scope of the work also includes the installation of, a drain field comprised of 2 trenches, 2 -feet wide x 45 -feet long, 12 feet deep, with 5.5 feet of septic rock below the distribution piping. The location of the drain field is shown on the attached site plan drawings. The total depth of the field will be 12 feet below the existing ground surface. All components of the proposed septic system must be constructed a minimum of 100 feet from any surface water or water wells in the area. The integrity of the existing septic tank must be verified. A minimum of two feet of cover is required over the drain field. In addition to the required earthen cover, 2 -inch thick, 40 psi, direct bury board foam shall be installed directly over all installed piping. 3. Construction shall be in accordance with the approved site plan and design drawings and any special provisions noted on the Municipal well and/or septic permit. All materials, components, workmanship and construction practices shall comply with the Municipality of Anchorage's latest ordinances regarding wells and septic systems, as well as MOA, Development Services Department, On-site Water and Wastewater Program (DSD) policies. 4. The contractor is responsible for obtaining all underground utility locates, layout of the proposed system, and verifying and maintaining all required separation distances to lot lines, surface water, wells, steep slopes, etc. S. The property owner shall be responsible for final grading any depressions formed from settlement of backfill material. Contractors installing wastewater disposal systems must be certified to do so by the MOA DSD, On-site Water Wastewater Program. Property owners installing their own systems must receive advanced approval from MOA DSD prior to system installation activities. 7. The contractor shall ensure that ALL materials being used are in compliance with the most current guidelines established by the Municipality of Anchorage. Refer to "Standards and Specifications for Component Parts and Materials Used in the Construction of On-site Wastewater Disposal Systems", Dated December 2000, as provided by MOA DSD, On-site Water and Wastewater Program. Drain field Construction: 1. The drain field shall be excavated to the dimensions outlined in the design drawings. If any discrepancies are noted, contact the engineer immediately for clarification. 2. The drain field shall be installed parallel to slope contours. 3. The bottom of the drain field shall be within 2" of level. If the sidewalls or bottom of the excavation become smeared, they must be raked or scarified prior to the placement of filter sand or drain rock. 4. The drain field must be a minimum of 5 feet from the test hole excavation. S. Distribution pipe is to be installed level with perforations facing downward, atop a level bed of drain field rock. Septic rock shall then be placed atop the distribution piping to provide a minimum of 2" of cover, level across the entire width and length of the trench, above the piping. 6. A silt barrier must be installed at between the final drain rock and the native soil backfill. The silt barrier shall cover the entire drain rock surface. 7. Monitor tubes must be 4" in diameter, installed in the locations depicted on the design drawings, and extend a minimum of 12" above final grade. The portion of the monitoring tube below the septic rock must be perforated and the portion above the drain rock shall be solid. S. Backfill over the final gravel layer must not be less than 2 feet. Insulation must be installed when the backfill depth is less than 3 feet. 9. Drain field back fill shall be mounded to prevent depressions from forming due to subsequent settlement of backfill material. 10. The contractor shall verify that the drain field is a minimum of 100 feet from any residential water well, 150' from any class'C' well and 200 feet from any community water wells. Environmental Consulting and Design Inspections: 1. A minimum of two inspections are required by the Municipality of Anchorage. These Inspections must be conducted under the supervision of a Professional Engineer licensed to practice in the State of Alaska. 2. The first inspection must be conducted after the excavation of the drain field and before the placement of any gravel. The septic tank may be set in place, but may not be backfilled. 3. The second Inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, stand pipes, cleanouts, and insulation, but before the placement of any backfill. 4. The contractor shall provide a legible copy of all field notes relating to system layout and construction to the engineer for the use in preparing the record drawings. S. The contractor shall coordinate all inspections with the Engineer. A minimum of 24 hours notice Is required prior to the start of construction. The tank may then be backfilled with 12 -inch maximum layers of granular soil and trampled evenly following the placement of each layer. • H.D.P.E.- H.D.P.E. tanks shall be placed on a compacted or undisturbed bedding of sand/gravel mixture or clean granular soil. Allow clearance around the tank to properly place and compact backfill. All H.D.P.E. tanks shall have a vertical length of 4 inch perforated drainpipe installed beside the tank extending from the bottom of the tank to grade to monitor the ground water table. H.D.P.E. tanks shall be installed so that a maximum of one foot of the tank bottom will set in the seasonal high ground water table. The tank shall be backfilled with 12 -inch maximum layers of granular soil and trampled evenly. • Fiberglass Tank - Excavate at least twenty-four inches beyond all sides of the tank to allow room for proper compaction of fill. Set tank level on a minimum 4 inch thick compacted sand or granular bed overlying a firm compacted and uniform base. Backfilling is a layering process that involves backfilling, compacting the backfill, and filling the tank with water to a level just above the compacted backfill layer. Backfill should have a uniform gradation and be free of any deleterious materials. Typically 2" crushed rock or pea gravel or sand is preferred. Each backfill layer should be uniform, no greater than 24 inches thick, and of nearly equal height around entire perimeter of the tank. No Expansion or Increase. Permittee shall not in any way expand or increase the extent to which the Encroachment occupies any of the real property encumbered by the Easement without the prior written consent of Chugach. Chugach has no obligation to provide such consent. 6. No Interest in Real Estate Acquired. Permittee hereby acknowledges that the Encroachment and any additions thereto, even if in violation of this permit,has not and shall not in the future give rise to any interest or estate in the real property subject to the Easement or any other real property. Without in any way limiting the foregoing, Permittee acknowledges that the Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the permission of Chugach for purposes of the common law doctrine of adverse possession. This permit creates no interest in the real property subject to the easement. 7. Modification of Utility Facilities. If the electric utility facilities located within the Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of Chugach's total costs for such modification which is reasonably attributable to accommodating or preserving the Encroachment. If such payment is not made within thirty (30) days of written demand for same, Chugach may terminate this permit in accordance with paragraph 9, below. 8. Covenants Running with the Land. The obligations of the Permittee arising from this permit shall be covenants running with the land which shall burden the Parcel and the Permittee's interest in the land subject to the Easement and shall benefit the Easement. 9. Termination. Chugach may terminate this permit after the giving of thirty(30)days written notice of such termination if: a. The Encroachment is enlarged, increased or extended within the real property affected by the Easement. Entire Agreement.
