Ladies and Gentlemen:
1 am writing to request a permit for the upgrade of the septic system for the above referenced
property. The proposed system will serve a 3 -bedroom single-family residence. The existing
septic system is backing up and must be pumped on a frequent basis. A soils log, site plan, design
drawings and construction specifications are enclosed for your review.
Design Calcs: No ground water observed to a depth of 18.0 feet (6/2/09)
Use 1000 gallon tank with a deep trench absorption field
Soil Rating. From Testhole 6/2/2009

Total area required: 250 x 3 = 750 sgft
System: The existing septic tank was installed in 2003 and should be sound. We propose
installing a diverter valve and a new deep trench absorption field. The absorption field will
consist of 2 trenches 45 feet long and 2 feet wide and contain 5.5 feet (effective) of sewer rock.
Soils: A test hole was excavated on June 2, 2009. See the attached soil logs. Ground water was
monitored for through June 10'h, 2009. Ground water was not observed.
Surface Nater: No surface waters were observed within 200 feet of the proposed septic
system.

Topography: The lot is generally level.
Waivers: A Class "C" well to proposed absorption field separation distance waiver has been
applied for through the Alaska Department of Environmental Conservation.
The installation of this septic system will not prevent wells and septic systems from being
installed on the adjacent lots. The proposed septic system will not change the general slope of
the area. Ponding and/or concentration of surface runoff will not result from this installation.

Wastewater Soil Absorption System-ADEC Plan Tracking Number 7843
Dear Mr. Spurkland:
On July 1, 2009 the Department received the final information for an engineering plan
submittal for a waiver to an onlot wastewater soil absorption system (Deep Trench Type
5.5' effective depth) for the three bedroom home at Heritage Hills, Block 4, Lot 1.
The Department has reviewed the engineered plans, your engineer's recommendation, the
sampling results on the Class "C" source well, and the well logs of nearby wells
indicating silty sands and clay layers between the source waters and the soil absorption
system. Samples taken from the well were for nitrate (1.96 mg/1) and no detection of
arsenic or bacteria. The following waiver is hereby established and approved.
The distance from the existing source well, a Class "C" Public Water System to
the deep trench type soil absorption system, is reduced from 150 feet to 120 feet
to the closest point of the deep trench.
This approval does not imply the granting of additional authorizations nor obligate any
state, federal or local regulatory body to grant required authorizations.
Any person who disagrees with this decision may request an adjudicatory hearing in
accordance with 18 AAC 15.195-18 AAC 15.340 or an informal review by the Division
Director in accordance with 18 AAC 15.185. Informal review requests must be
delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within
15 days of receiving the decision. Adjudicatory hearings requests must be delivered to
the Commissioner of the Department of Environmental Conservation, 555 Cordova
Street, Anchorage, Alaska 99501, within 30 days of the decision. If a hearing is not
requested within 30 days, the right to appeal is waived.

Single Family On-site Wastewater Disposal System
Construction and Material Specifications
Subject Property: Heritage Hills Block 4 Lot 1
Date: June 23, 2009
General:
1. The owner shall contract with a contractor to perform the work outlined in these
specifications and plans in accordance with the attached MOA permit. There is
not contractual arrangement between the contractor and the Engineer. The
Engineer shall be the owner's representative and will inspect the work to
document the contractors activities. Final acceptance of the contractors work
rests with the owner and the MOA.
2. The scope of the project includes the procurement and installation of a 3 -
bedroom on-site wastewater disposal system at the location depicted on the
attached site plan drawings. The scope of the work also includes the installation
of, a drain field comprised of 2 trenches, 2 -feet wide x 45 -feet long, 12 feet deep,
with 5.5 feet of septic rock below the distribution piping. The location of the
drain field is shown on the attached site plan drawings. The total depth of the
field will be 12 feet below the existing ground surface. All components of the
proposed septic system must be constructed a minimum of 100 feet from any
surface water or water wells in the area. The integrity of the existing septic tank
must be verified. A minimum of two feet of cover is required over the drain field.
In addition to the required earthen cover, 2 -inch thick, 40 psi, direct bury board
foam shall be installed directly over all installed piping.
3. Construction shall be in accordance with the approved site plan and design
drawings and any special provisions noted on the Municipal well and/or septic
permit. All materials, components, workmanship and construction practices shall
comply with the Municipality of Anchorage's latest ordinances regarding wells and
septic systems, as well as MOA, Development Services Department, On-site
Water and Wastewater Program (DSD) policies.
4. The contractor is responsible for obtaining all underground utility locates, layout
of the proposed system, and verifying and maintaining all required separation
distances to lot lines, surface water, wells, steep slopes, etc.
S. The property owner shall be responsible for final grading any depressions formed
from settlement of backfill material. The property owner is also responsible for
vegetation of all areas disturbed by septic system installation activities.

6. Contractors installing wastewater disposal systems must be certified to do so by
the MOA DSD, On-site Water Wastewater Program. Property owners installing
their own systems must receive advanced approval from MOA DSD prior to
system installation activities.
7. The contractor shall ensure that ALL materials being used are in compliance with
the most current guidelines established by the Municipality of Anchorage. Refer
to "Standards and Specifications for Component Parts and Materials Used in the
Construction of On-site Wastewater Disposal Systems", Dated December 2000, as
provided by MOA DSD, On-site Water and Wastewater Program.
Drain field Construction:
1. The drain field shall be excavated to the dimensions outlined in the design
drawings. If any discrepancies are noted, contact the engineer immediately for
clarification.
2. The drain field shall be installed parallel to slope contours.
3. The bottom of the drain field shall be within 2" of level. If the sidewalls or
bottom of the excavation become smeared, they must be raked or scarified prior
to the placement of filter sand or drain rock.
4. The drain field must be a minimum of 5 feet from the test hole excavation.
S. Distribution pipe is to be installed level with perforations facing downward, atop a
level bed of drain field rock. Septic rock shall then be placed atop the distribution
piping to provide a minimum of 2" of cover, level across the entire width and
length of the trench, above the piping.
6. A silt barrier must be installed at between the final drain rock and the native soil
backfill. The silt barrier shall cover the entire drain rock surface.
7. Monitor tubes must be 4" in diameter, installed in the locations depicted on the
design drawings, and extend a minimum of 12" above final grade. The portion of
the monitoring tube below the septic rock must be perforated and the portion
above the drain rock shall be solid.
S. Backfill over the final gravel layer must not be less than 2 feet. Insulation must
be installed when the backfill depth is less than 3 feet.
9. Drain field back fill shall be mounded to prevent depressions from forming due to
subsequent settlement of backfill material.
10. The contractor shall verify that the drain field is a minimum of 100 feet from any
residential water well, 150' from any class'C' well and 200 feet from any
community water wells. Additional separation distances must also be maintained.

Environmental Consulting and Design
Inspections:
1. A minimum of two inspections are required by the Municipality of Anchorage.
These Inspections must be conducted under the supervision of a Professional
Engineer licensed to practice in the State of Alaska.
2. The first inspection must be conducted after the excavation of the drain field and
before the placement of any gravel. The septic tank may be set in place, but may
not be backfilled.
3. The second Inspection must be conducted after the placement of the silt barrier,
gravel, distribution lines, stand pipes, cleanouts, and insulation, but before the
placement of any backfill.
4. The contractor shall provide a legible copy of all field notes relating to system
layout and construction to the engineer for the use in preparing the record
drawings.
S. The contractor shall coordinate all inspections with the Engineer. A minimum of
24 hours notice Is required prior to the start of construction. If necessary, a
preconstruction meeting will be held on-site.

Installation Requirements for Septic Tanks in the
Municipality of Anchorage:
• Steel - Steel septic tanks shall be placed on undisturbed soil at the bottom
of the excavation for the tank and backfilled with a granular material free
of large rocks or boulders to a maximum depth as prescribed on the exterior
of the septic tank. The tank may then be backfilled with 12 -inch maximum
layers of granular soil and trampled evenly following the placement of each
layer.
• H.D.P.E.- H.D.P.E. tanks shall be placed on a compacted or undisturbed
bedding of sand/gravel mixture or clean granular soil. Allow clearance
around the tank to properly place and compact backfill. All H.D.P.E. tanks
shall have a vertical length of 4 inch perforated drainpipe installed beside
the tank extending from the bottom of the tank to grade to monitor the
ground water table. H.D.P.E. tanks shall be installed so that a maximum of
one foot of the tank bottom will set in the seasonal high ground water table.
The tank shall be backfilled with 12 -inch maximum layers of granular soil and
trampled evenly.
• Fiberglass Tank - Excavate at least twenty-four inches beyond all sides of
the tank to allow room for proper compaction of fill. Set tank level on a
minimum 4 inch thick compacted sand or granular bed overlying a firm
compacted and uniform base. Backfilling is a layering process that involves
backfilling, compacting the backfill, and filling the tank with water to a level
just above the compacted backfill layer. Backfill should have a uniform
gradation and be free of any deleterious materials. Typically 2" crushed rock
or pea gravel or sand is preferred. Each backfill layer should be uniform, no
greater than 24 inches thick, and of nearly equal height around entire
perimeter of the tank.

Approval for your individual sewer and water facilities
cannot be granted until the following items have been
completed:
(1) The water analysis report be delivered to this
department from Chem Lab, 5633 B Street, for
our review.
(2) The septic tank pumped with a receipt submitted to
this department.
(3) Locate and expose the standpipe to the seepage pit
for ouz inspection. This is to insure the minimum
distance requirements are met between your well and
sewer system.
(4) An adequacy test be performed on the existing leaching
area. This test will determine if the system is adequate
according to National Standards. A listing of private
firms performing the test is enclosed. This report
needs to be submitted to this department for our review.
(5) We will need to inspect the well for proper construction.

department at 264-4720.

ENCROACHMENT PERMIT
This permit is between Chugach Electric Association, Inc., an Alaska non-profit electric
cooperative, (herein"Chugach"),whose address is PO Box 196300,Anchorage,Alaska 99519 and
Rebecca Sue Brandon Revocable Trust, (herein "Permittee"), whose address is 4661 Natrona
Avenue, Anchorage, AK 99516.
1. Easement. Chugach is the grantee or user of an easement for the construction and
maintenance of electrical facilities, as follows:
Dedicated by Plat No. 63-41,recorded on the 8th day of July 1963,on file in the Office of
the District Recorder, Anchorage Recording District, Seward Meridian, Alaska,
(herein "Easement") which pertains to the following described real property:
The North Ten Feet(N 10')and the West Ten Feet(W 10')of Lot One(1),Block Four(4),
Heritage Hills Subdivision, according to Plat No. 63-41, recorded on the 8th day of July
1963,on file in the Office of the District Recorder,Anchorage Recording District,Seward
Meridian, Alaska,
Permittee acknowledges the validity of the Easement. Permittee warrants and represents that
Permittee is the owner of the fee simple interest in the land subject to the Easement and the
following described real property to which the Easement is adjacent:
Lot One (1), Block Four (4), Heritage Hills Subdivision, according to Plat No. 63-41,
recorded on the 8th day of July 1963, on file in the Office of the District Recorder,
Anchorage Recording District, Seward Meridian, Alaska,
(herein "Parcel").

the real pect to ement:
Two Septic Vents that encroach a width of approximately Four Feet(4')by a length of
approximately Two Feet(2')into the south side of said north easement and a Well that
encroaches a width of approximately Two Feet (2')by a length of approximately Two
Feet (2') into the south said north easement, which also intersects said west easement,
(herein "Encroachment").
3. Permit. Chugach hereby authorizes Permittee to continue the existence of the
Encroachment in its existing location subject to the covenants set forth in this permit.
4. Indemnification. Permittee shall indemnify and hold Chugach harmless from any
claims for personal injury,property damage or other loss arising in any way from the continuation of
the Encroachment.
5. No Expansion or Increase. Permittee shall not in any way expand or increase the
extent to which the Encroachment occupies any of the real property encumbered by the Easement
without the prior written consent of Chugach. Chugach has no obligation to provide such consent.
6. No Interest in Real Estate Acquired. Permittee hereby acknowledges that the
Encroachment and any additions thereto, even if in violation of this permit,has not and shall not in
the future give rise to any interest or estate in the real property subject to the Easement or any other
real property. Without in any way limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the
permission of Chugach for purposes of the common law doctrine of adverse possession. This
permit creates no interest in the real property subject to the easement.
7. Modification of Utility Facilities. If the electric utility facilities located within the
Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving the Encroachment. If such payment is not made within thirty (30) days of written
demand for same, Chugach may terminate this permit in accordance with paragraph 9, below.
8. Covenants Running with the Land. The obligations of the Permittee arising from
this permit shall be covenants running with the land which shall burden the Parcel and the
Permittee's interest in the land subject to the Easement and shall benefit the Easement.
9. Termination. Chugach may terminate this permit after the giving of thirty(30)days
written notice of such termination if:
a. The Encroachment is enlarged, increased or extended within the real property
affected by the Easement.

Vb. The Encroachment is damaged by any means to an extent of more than thirty percent
(30%) of its replacement cost at the time of destruction.
c. Permittee fails to make the payment described in paragraph 7, above.
10. Term. This permit shall, if not sooner terminated by Chugach, expire forty (40)
years from the date hereof.
11. Entire Agreement. This written permit constitutes the entire agreement between the
parties with respect to the subject matter hereof and supersedes all other prior or contemporaneous
agreements, oral or written, between the parties. No modifications, amendments, deletions,
additions or alterations of the permit shall be effective unless in writing and signed by all of the
parties hereto.

FT)wide utility easement, lying adjacent to and coincident with the north boundary of Lot 1 and situated
on said Lot 1, Block 4,Heritage Hills Subdivision,according to Plat No. 63-41,Records of the Palmer
Recording District,Third Judicial District, State of Alaska.
Acceptance and use of this letter of non-objection by yourself,your heirs,your assigns,or your
successors,will constitute agreement to the following stipulations:
• Landowner/Contractor working near ENSTAR gas facilities shall contact the Alaska Digline,
Inc., (907)278-3121 or 811 for line locating two(2)business days prior to any related
excavation. This service is free of charge.
• ENSTAR will be held harmless,now and forever for any damages or injury to any person or
property as a result of this encroachment.
• Any ENSTAR facility damaged or destroyed, as a result of this encroachment will be repaired at
no cost to ENSTAR.
• Any costs incurred by ENSTAR for special construction necessitated by this encroachment will
be borne by the land owner.
• All applicable safety code regulations will be observed and maintained.
• This letter of non-objection will in no way preclude ENSTAR from full use and enjoyment of its
rights within any portion of its right-of-way.

additional and extraordinary costs incurred during any future required construction,repair
or reconstruction of GCI's facilities to accommodate any or all of the encroachments shall
