The subject permit, issued July 27, 1994 by this office for a
single family well and/or on-site wastewater system, has
expired as of July 27, 1995.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Dear Sirs,
With the submittal of the attached design package and Permit Fee check in the amount
of $320.00, the owners of the above noted lot, Mr. Floyd Bayless and Ms. Marlene Story
request the granting of an on-site wastewater disposal permit for a two bedroom house.
We have relied on and verified the percolation logs of the original engineer to investigate
this lot. The percolation logs of Corwin & Associates were taken from DHHS files. Our
percolation test 1 was a verification of the tests performed by Corwin & Associates.
We note that the logs indicate substantially the same material at depth, a Sandy Gravel
to Gravelly Sand with Silt. The water level observed by Corwin is somewhat lower than
that recorded after 7 days in percolation Hole 1, but we note that a measurement made
by Mr. Ban Belles and Ms. Susan Oswalt on 10/25/90 indicates the water level to be
substantially the same as our observation in Hole 1.
After presoaking both holes for the required period of 4 hrs., we perked the holes at 25
min./in. & 9.04 min./in, respectively. We have used the more conservative figure of 25
min./in, on which to base our design because of our confidence in our methodology and
the somewhat silty nature of the soils.
The oversized septic tank will provide added retention time of the wastewater, and
enhance treatment.

Site Narrative
The following site narrative describes the probable impacts of an on-site wastewater
disposal system for Lot lB, BIk. 2, Hideaway Forest Subd.
The subject lot is located in Sec. 24, T12N, R3W, at 11561 Trails End Rd. The lot
contains 1.1 acres more or less. Access to the lot is off Trails End Rd., to the lower
portions of the lot. The lot is covered by willow and alder shrub, with few birch or spruce
trees. Lot slope is generally at '13%, except for a benched portion in the middle of the
east/west direction leveled for the system.
The lot is bounded on three sides by developed lots, and on the west by Trails End
Road. The design drawings indicate the surveyed separation distances between
improvements on Lot lB and adjacent lots. All Municipal separation distances have
been observed.
All well separation distances have been maintained from all portions of the proposed
wastewater disposal system. The closest well to this system is located on Lot lA, and
is 105 ft. from any portion of the system. Wells to the east, south and west are a
minimum of 255 ft. from any portion of the system. The wells to the east, north and
south are all up-gradient of the proposed system. Wells to the west of the system are
more than 150 ft. from any portion of the system, and are separated from any potential
surface flow of septic effluent by the north/south roadway ditch on the east side of Trails
End Rd.
The well on Lot lB is located in the extreme northeast corner of the lot. As shown on
the system design drawings, the well is separated from on-site wastewater disposal
systems on other lots by a minimum of 160 ft. This well is up gradient of all on site
septic systems except the one to the east on Lot 2. It is felt that since this system
occupies a position somewhat to the north of the well, any septic effluent which escaped
to the surface would flow down gradient to the north of this well.
All lots directly adjacent to and therefore affected by the proposed system are
developed. They presently have both wells and on-site wastewater disposal systems.
The remaining portions of these lots, available for reserve area, are more than adequate
should any of these systems fail, and require installation of new systems.

The site drainage for all lots on the east side of Trails End Rd., in the immediate area
of Lot lB, is to the west. The general topography for all these lots is similar, having
slopes of 10% to 15% toward the road. The east side of Trails End Rd. is bounded by
a drainage ditch which flows to the north. All run-off from these lots accesses this ditch
to be carried to Upper O'Malley Rd.
On-site drainage for Lot lB is handle by drainage swales at the upper (east) end of the
lot, which direct run-off to the south and then to the west to access the drainage ditch
at the east side of Trails end Rd.
This lot is not affected by run-off from adjacent lots to the north, south and west. Run-off
from the east is directed to Trails End Rd. via swales.
The installation of this on-site wastewater disposal system will not adversely affect any
adjacent lots.

All percolation tests conducted for this project have been completed using a 4"
diameter tube in a 6" diameter hole. The annular space between the tube and the
hole walls s filled with 3/8" pea gravel.
The following adjustment has been made to all readings to account for the volume
occupied by the annular gravel pack.

The porosity of the gravel has been determined using two
identical 64 oz. containers. One filled with water, the other with
gravel. The depth of water (h) was measured. Water was

The following design package is being resubmitted after revision per comments on
attached pink transmittal sheet.
The system dimensions have been adjusted to conform to the maximum 15 ft. width for
absorption beds per DHHS letter dated June 1, 1994.
Per my telephone conversation with Mr. Dan Roth, the surface water originating at the
east side of the existing building will be caught in a top inlet surface drainage structure
and piped via 4" perforated drain pipe to the 12" ADS drain pipe installed by the owner
along the south edge of the driveway.
The cut-off drain will be installed in accordance with the attached sketch.
The owner will install this cut-off subdrain line prior to beginning construction on the
proposed septic system. In compliance with standard practices and our agreement, the
owner will provide the Department with two hours notice for inspection of the catchment,
ditching and piping prior to backfilling.
As this drainage pipe is within 100 ft. of the proposed system, it ~s our understanding that
a waiver from Field to Surface Water must be obtained. Attached is a check in the
amount of $920.00 to cover the waiver fee.
We are requesting an On-site Septic System permit be issued, conditional to the prior
installation of the above noted subdrain.

The following design package is being resubmitted after revision per comments on
attached pink transmittal sheet.
Per my conversation, this morning, with Mr. Dan Roth, I have revised the attached design
to reflect the comments discussed.
A catchment design is shown on Figure B, attached. Mr. Bayless will construct the
catchment as shown, from materials he has on site.
Please note that the pipe exiting the existing catchment is 6" to approximately the last 15
ft. of its length, where it is 12". Mr. Bayless indicates that the last 30_+ ft. are insulated
with 4" of rigid Insulboard. We have enclosed a flow calculation indicating that the 6"
plastic pipe is capable of handling up to 1.25 cfs (561 GPM). This capacity is more than
adequate for the less than 5 GPM the pipe will carry during non runoff periods.
Attached are copies of utility locations for Gas, Electric and Cable. None of these utilities
are effected by the location of the subsurface drainline. The Electric and overhead
Telephone utilities are more easily accessible due to the location of the driveway on this
lot.
I have visited this lot today, and noted that no water is flowing from the lot to the east.
Flow from the east side of the house is less than 0.5 GPM..
I have spoken with Mr. Bayless concerning the importance of installing the drainage
upgrades and new septic system to acceptable standards. Mr. Bayless has indicated that
he is very aware of the significance of proper installation, and has assured me he will
comply with all DHHS requirements.

This letter is confirmation of the sewer and water disconnect on Lot 1 B, BIk 2,
Hideaway Forest.
On August 17, 1991 the sewer line connecting the existing structure to the
septic tank was disconnected at the septic tank and plugged on both sides of
the disconnect (see photos 1,2, 3, & 4).
As well, the water supply line between the existing structure and water well was
disconnected inside the structure from the water distributiOn system (see photos
5&6).
At this time I'd like to inform you that the seep pit located 80 feet east of my well
on Lot 2, BIk 2, Hideaway Forest has been abandon as outlined in my August
12, 1991 letter (see photos 7, 8, 9, 10, 11 & 12).
I stated in my August 12, 1991 letter to your office that I would submit a septic
redesign to your office by October 1, 1991. I would like to revise that date to
October 31, 1991 due to other unforeseen problems not associated to this
project.

This letter is confirmation of our meeting August 9, 1991.

It is understood that existing perk tests will be utilized in the redesign unless a
portion of the redesigned system falls outside of the existing perk tests' 30 foot
radius. Should this occur additional tests will be conducted where necessary.
Concerning the existing outhouse seep pit located 80 feet east of my well on Lot
2, BIk 2, Hideaway Forest, it is my understanding that the seep pit must be
properly abandoned in order to utilize my well. To properly abandon the seep
pit it must be treated with chlorinated lime and Clorex, then backfilled. This will
be accomplished and certified by an engineer no later than August 31, 1991 At
which time it is understood that can utilize my well after the installation of an
approved sewer system.
Discussed was the possible usage of the current well and sewer system. It is
understood that neither system can be used until approved by your office and
that until such approval the existing structure cannot be used as living quarters.
To assure your office that neither system is utilized, the sewer line connecting
the structure to the septic tank will be disconnected, plugged and backfilled at
the septic tank. Reconnection of the line to the septic tank will not take place
until Spring 1992 when the tank is relocated to an approved location. As for the
well system, the main supply line shall be disconnected inside the structure
from the water distribution system. Both disconnects shall be accomplished no
later than August 19, 1991. As well, a letter from the engineer certifying that the
disconnects have been accomplished as outlined will be sent to your office no
later than August 23, 1991.
As discussed I will retain the services of Carey Meyer P.E. to redesign the newly
installed sewer system on my lot and submit the redesign to your office by
October 1, 1991.
As stated in our meeting, your office has unlimited access to Lot 1 B, BIk 2,
Hideaway Forest at any time to verify that neither the well nor sewer system are
being utilized and that the existing structure is not being used as living
quarters. Please contact me at 279-0543 or 272-8619 should you want access
into the structure at any time.
In addition I will take this opportunity to inform you that work will be in progress
through out the forth coming fall and winter to upgrade the condition of the
existing structure. As well, for the purpose of security against vandalism, all
window areas will be covered with plywood by October 31, 1991.

~WATER AND WASTEWATER NARRATIVE FOR LOT lB, BLOCK 2, HIDEAWAY
FOREST SUBD.
The location of the well and wastewater disposal system on the
subject property should have no negative affects to the
adjacent properties or the environment for the following
reasons:
WELLS
The adjoining wells on Lots iA and Lot 2, Block 2, Hideaway
Forest and the on Lot 1, Block 1, Sunny Subd. are properly
separated from the planned wastewater extension by at least 100
ft. and the existing well is located such that there will be no
negative impacts to neighboring properties.
WASTEWATER SYSTEM
The wastewater system proposed for this property has been
located so that there will be no effects to any surrounding
properties. All setback distances have been satisfied as
required. Further the soils are of such a nature that
treatment will be assured over a long term period. The
wastewater system is also located so as to prevent any
contamination of the surrounding area.
RESERVED SPACE
The wastewater system on this property has no effect in taking
away reserve area set aside on other adjacent lots and there is
adequate reserve space on this lot lot for future replacement.
SURFACE AND SUBSURFACE DRAINAGE
The wastewater system will have no impact to surface water
drainage due to the location and the fact that the contours
will remain unchanged. Subsurface drainage will be unaffected
since treatment of wastewater is assured before it reaches
groundwater tables. The topography of the lot contains some
areas of slope greater than 10 per cent, but the area selected
to receive wastewater systems is fairly level with only minor
sloping.
JUSTIFICATION FOR BED SIZE
since the proposed wastewater system requires 1,500 sq. ft., a
15 ft. wide bed would need to be 100 ft. long or a ratio of 1
to 7 for width to length. We believe that a better fit is to
make the bed 24 ft. wide thus allowing a 62.5 ft. length or a
ratio closer to 3 to 1 as is normally desired. During
construction the contractor should ensure that excavation
