based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, Lin- attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSO Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life craft wells and
septic systems depend on the local soils condition, groundwaterlevels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

On the behalf of Judy Carpenter, I am requesting a well to septic tank separation distance
waiver on the above referenced property. Ms Carpenter originally retained Tryck Nyman
Hayes Inc. (TNH) to obtain a Health Authority Approval for the property; however, upon
investigating the property, it was discovered that the existing well is approximately 82
feet from the existing septic tank.
The system was originally installed in August of 1980. At the time the septic system was
installed, the well location was proposed at the location it was eventually installed at. It
appears from the existing records, an inspector from your office approved the installation
of the system even though the proposed well location was shown on the site plan and is
within 100' of the septic tank location.
From the existing well log and soil test pits constructed before the installation of the
septic system (attached), the soil down to approximately 15' is a well graded gravel with
a trace of silt. Water was encountered at 12' in the test pit; however, this water appears
to be shallow groundwater as the well log indicates bedrock occurs below 15'. The
overall depth of the well is 140' with a static water level of 33' as of March 2, 1993.
Over all, the lot slopes at 14%-18% from west to east (from the well towards the septic
tank). There is also a slight cross slope, approximately 2 % from the north to the south.
The size of the lot is 30644 square feet. From the existing records, the subdivision has
grandfather rights for this size of lot. A recent appraisal of the property has determined
that the house is considered a two bedroom, single familiy residence. The maximum
expected effluent flow would be 300 gallon per day. ¢/O' O0

Investigations of the records for properties which surround the subject property reveal that
the wells in the area are approximately the same depth and configuration of the subject
properties. Additionally, 2 lots have had recent Health Authority Approvals conducted
upon them and all water tests were within the acceptable limits (attached). Water
sampling was conducted for the subject property for the Health Authority Approval.
These samples were also within the acceptable limits.
From the records, it appears that there is a drainage ditch 52' to the north of the existing
septic system and another 40' to the south. Mr Dan Boles of your office indicated that
an additional waiver of the separation distances between the drain field and these ditches
would be required. Additionally, Mr Boles voiced concerns that one of the ditches was
actually a stream. Conversations with Ms. Carpenter on this subject revealed that this
was a concern before the system was installed and was resolved at the time of
construction. The separation distance requirements were waved at the at that time by
your office.
These ditches were investigated during a site visit on March 8, 1993. The ditch to the
north was walked to approximately 1/2 mile above the residence. Holes were dug in the
snow periodically and no evidence of this being a stream was found. As shown on the
records, this ditch is approximately 52' from the drain field. The ditch to the south of
the field also does not appear to be a stream. This would require that the "stream" cross
the road to Ms. Carpenter's house as there is no culvert present. There was no evidence
of excessive erosion over this road which would suggest a stream. In any event, this
ditch appears to be approximately 100' away from the drain field and should not be a
factor.
The granting of this waiver would allow Ms Carpenter to obtain a Health Authority
Approval required by the financial institution of the prospective buyer of her home.
There are no negative impacts in granting of this waiver. The system has been operating
for approximately 13 years with no apparent negative impacts on either the property in
question or the surrounding properties. Wells in the area have been tested within the past
2 years and found to be within the required limits of nitrates and bacteria/coliform. The
ground slopes significantly away from the well towards the septic tank and due to the
differences in elevation between the well and the septic tank, the top of the septic tank
is below the elevation of the bedrock at the well location.
The only adjoining property appears to be lot 3, blk 2, Hocker Subdivision. The address
of this property is 10040 Flat Top Rd. The owner is Mr. Timothy Connolly, 345-1690.

Please find enclosed for your consideration two soils logs for Hocker
Subdivision. Also included is a plot showing log locations. We are
presently unable to locate a boring on lot 4 block 2 due to lack of
access. However we feel the soil conditions as shown are very similar
for this lot.
The soils log for lot 4 block 2 shows a water table at 12 feet. This
particular boring is located near a small stream (10 to 15 feet). The
ground water encountered appears to be seepage from this stream. The
septic system for lot 4 will be located approximatley 150 feet from this
stream.
Further investigation of water well drill logs for this area also supports
the above borings.
The general area slopes 14 to 18% from east to west.
This will serve as our request for approval to construct a septic system
on lot 4 block 2 Hocker Subdivision.
