At the request of our client, Henry Stover, we are submitting documentation of a well flow test
that we performed on June 18,1998, at the referenced property. According to the log (see
attached), the well was drilled in the month of July, 1994 and produced approximately 60 gallons
per hour (1 gpm). Mr. Stover enhanced his well production prior to our well flow test by
hydrofracturing it (report attached). The flow test was ran for a four hour period and the well
produced 8.5 gallons per minute through the entire test (see attached data). This test does not
guarantee the well flow to remain constant and subsequent variations may occur.

The attached papers are for your files. I have included what originals I have and
requested Alaska Water and WasteWater to send a copy of the certification of the flow
rate on the Sullivan well that is cun'ently used at 20015 Scenic Dr. Lot 149b Lashbrook
Subdivision, Chugiak, AK.
I have supplied a copy of the invoice from Klondike Cement, they supplied the cement to
abandoned the Vern Drilling well we have discussed and "see attachment" The
abandonment was as per the Specification of 2 1\2 feet + down capped and cemented.
IfI have forgotten any thing or if you should requh'e something I have not included,

Jody Maus, your associate who completed the 4 hour certified water flow test located at
20015 Scenic Dr., Lashbrook Subdivision 149B, Chugiak, Alaska, ask me to address the
method I (Henry Stover) utilized to increase the tested well output. As you are aware, the
well was drilled in 1994 by Sullivan. The provided well log indicated a low flow rate.
Sullivan, at the time of drilling, advised the well flow could increase with usage since
other wells in the area are good producers.
From the well log, you will see that Sullivan made two perforations below the 40' casing
level. The static water level has been a constant 13' since he drilled the well. I rented a
185 cfm ah' compressor fi'om DJ rentals in Eagle River, welded a 3\8 cap with a l" Tee in
the center onto the top of the casing. I attached the air compressor to one side of the tee
and installed a ball valve on the opposite end of the tee. I utilized the ah' compressor to
flush the static water out of the casing. I shut down the compressor, opened the ball valve
for one hour then, again flushed whatever static water returned within the hour wait.
I preformed this procedure for more that 16 hours.
I then bought a new well pump, temporarily attached the wiring, and pumped the well
straight from a 1 1\4 plastic pipe. I pumped the well "off and on" for several days, to
ensure the flow rate was increasing. After the well maintained a constant casing level
while being pumped wide open, I then made the well operational and started utilizing the
well as the primary well for the home.
I advised Jody and I had consulted Dan with the City of Anchorage Division of Health

I also ask Jody to view the area (plat provided) and a section of the removed casing

abandoned was indicated on the "as built" that was faxed to you. Klondike Cement had
just hours prior delivered the cement to fill the well, and the abandomnent was as per the
City of Anchorage's specifications..
We had also "earlier" spoken about the approved upgrade of the current septic system
Jody advised he had been the inspector for the upgrade.
I request that you furnish Dan at the Health Services a copy of your testing for his
records.

in line, shall be to clean toward the leachfield. The second cleanout
shall be to clean toward the septic tank.
Final grading over the septic tank shall be such that a positive slope
exists away from the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
Excavate the proposed trench to the dimensions shown on the design. The
bottom of the excavation shall be within 2 inches of level. If the
sidewalls of the excavation become smeared, they must be raked or
scratched (roughed-up) before gravel (sewer rock) placement.
Once the gravel is installed, the distribution pipe is to be installed
level with the perforations faced downward. Gravel is then to be placed
over the distribution pipe to provide a minimum of 2 inches of cover
over the pipe.
A silt barrier must be installed between the final gravel layer and the
native soil backfill. Ensure the silt barrier covers the entire gravel
surface before placing backfill.
Monitor tubes shall be of four (4) inch diameter and installed
approximately in the locations shown on the design. The portion of the
monitoring tube extending through the gravel shall be perforated from
the bottom of the trench to the invert of the distribution pipe. This
is equivalent to the effective depth of the gravel as noted on the
design.
o
Backfill over the final gravel layer must not be less than twenty-four
(24) inches. Insulation must be installed when the backfill depth is
less than thirty-six (36) inches. The finish grade over the trench must
be mounded to prevent the formation of a depression after settling.
MATERIAL SPECIFICATIONS:

Use of a type of pipe other than listed above must be approved by the
inspecting engineer.
Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Company Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi 140/N, or equal)
must be installed between the final leachfield gravel layer and the
native soil backfill.
Ail leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel
with less than 3% passing the %200 sieve.
When sand is being used as a filter material, its gradation
specifications must conform to current M.O.A. or D.E.C. requirements.
INSPfL~TIONS:
Typically there will be a minimum of three (3) inspections required during the
installation of the wastewater disposal system. These inspections will occur as
follows:
The first inspection must be conducted after the excavation of ditches,
pits, trenches, or beds and before the installation of any gravel. A
septic tank may be set in place, but may not be backfilled before this
inspection.
The second inspection must be conducted after the placement of the silt
barrier, gravel, distribution lines, standpipes, cleanouts, and
insulation, but before the placement of any other backfill.

3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required, especially with the
installation of multiple trenches, sand filters, pressurized distribution
systems, etc. Thus, the inspecting engineer is to be contacted at least 24
hours prior to the start of construction. If necessary, a pre-construction
meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractor's activities.
The owner shall contract with the contractor to perform the work outlined in
these specifications and plans and in accordance with the attached M.O.A.
permit. There will be no contractual arrangement existing between the
contractor and S & S Engineering. S & S Engineering shall be the owner's

the owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or
omissions of the contractor or any other persons performing work on this project
or the failure of the contractor to carry out the work in accordance with these
construction documents. S & S Engineering's inspecting engineer will not be
responsible for the construction means, methods, techniques, sequence,
procedures or the safety precautions incident to this project.

Attn: Messrs: Bob Shafer and Bob Cowan
Gentlemen:
Subject: Lot 139B, Lashbrook Subdivision
On October 13, 1994, this office issued Conditional Health Authority Approval No.
HA940525 for Lot 149B, Lashbrook Subdivision. Under the terms of the conditional
approval, the newly installed bed (9/29/94) was to be enlarged 225 square feet by
November 15, 1994. Since this is a new system, we do not believe that it is urgent that
the upgrade be completed under the prevailing winter conditions, and we therefore are
extending the deadline for completion to June 1, 1995. Unless you have objection to this
extension date, our file will be amended accordingly.
Sincerely,
Robert W. Robinson
