Roth, Civil Engineer, On-site Services~ / Department of Health and Human Services Request for Comments on Subdivision(s) May 14, 1993 The Environmental Services Division, On-site Services Section, of the Department of Health an Human Services has reviewed the following cases and has these comments: S-9391: Lot 18 Block 5 Sand Lake #2 Add a note to the plat that states: "existing home must not exceed three (3) bedrooms while utilizing on-site wastewater disposal.". S-9392: Lot 33A Tract A Eagle Crest #1 Add a note to the plat that states: "existing home must not exceed three (3) bedrooms while utilizing on-site wastewater disposal". S-9393: Lots 149A, 149B Lashbrook Subdivision This case is being returned for the following reasons[ 1. This office has as-built information for the septic system that serves the dwelling on the north half of the lot. However, our files do not have a well log for the well serving the dwelling. A well log must be submitted to complete the subject file and prove water availability for the proposed subdivision. 2. Developer must designate the option they intend to use to determine reserve area for the existing developed lot and the proposed new lot. Either 15.65.180 B1 or B2. 3. Soils information submitted with the proposed subdivision appear to be located in the west portion of the proposed new lot. If a permit to construct a new septic sytem were applied for, this office will not allow a septic system to be constructed in the west portion of the lot since · the existing septic system on 'the existing lot is constructed in the east portion of that lot. 4.
