Sonja, the original well located on Block 1, Lot 7 Lakewood Hills # 4, has been decommissioned
as per DEC and MOA requirements. The existing well could not be deepened because of
perforations. Depth of the old well was 100 feet. New well depth is 114 feet.

To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The existing well was
recently inspected by Dave Harper at Alpine Drilling and was discovered that the well is not producing
adequate water production. We are proposing to have the existing well decommissioned per UPC and
have a new well drilled near the same location as the existing well.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The septic system is in a state
of failure and needs to be upgraded. We are proposing to install a new 24 inch pump vault, a 1250 gallon
septic tank, and a dual deep trench type drainfield. One test hole was excavated on the property. The
drainfield will be designed around the 30 foot radius of this test hole. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfieid specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached topo drawing the topography around the drainfield
runs south to north at approximately 20% slope. In short, there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by'a private or Class C well and may

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the

A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 1, Lot 7 of
Lakewood Hills #4 subdivision. This inspection revealed a nitrate
concentration of 8.02 milligrams per liter (rog/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

Dear Steve:
This letter is a result of a conditional health authority
approval on the above property and my recent site investigation. The
enclosed sketch shows sewage system appurtenances and the well. We
were finally able to verify pipe connections, etc. with water and a
power snake.
As you can see, the well is 52 feet from the septic tank. This
was apparently approved in August, 1976 by your department after a
second trench was installed. The previous paperwork is enclosed.
The client, of course, would like a waiver for the septic tank to
well separation distance. If the enclosuress don't answer all of your
questions, please call.

Dear Ms. Oswalt;
This request for a separation distance waiver and a
conditional health authority approval is a little different.
Strange, in fact. I have gone through the file and looked at the
site and still am not sure how the on-site wastewater system was
built or connected (or approved).
Apparently, the first tank and drainfield were installed at
about the same time as the well; September, 1975. The well
wasn't approved becat~se of the separation distances, so a new
trench was constructed and was inspected August 31, 1976. That
inspection shows the trench to be 95 feet from the well, although
my measurements indicate that it may be 100 feet away.
The septic tank, however, is only 53 feet or so from the
well if it is still located where the original inspection showed
it. There are no pipes visible at this time so we want to expose
them after breakup.
Due to 2 feet of snow and the frozen ground, the owner has
requested that I ask for a conditional Health Authority Approval.
In the worst case, I estimate that relocating the entire septic
system and furnishing a new tank will cost about $6000 including
engineering, assuming the 1975 soils test is fairly accurate.
That price includes properly abandoning the old system, driveway
repairs, etc.
I can't locate all the neighboring wells and septic systems
w~th the snow cover, but relocating the well instead of the
septic tank may be the best choice if separation distances allow.
That action would leave two full sized drainfields on the lot
available for wastewater disposal. A new well and service line
should cost less than the $6000 mentioned ¢~bove, and both
upgrades would not be required. In other words, if the septic
system is upgraded, the well will not have to be moved and vice
versa.
When breakup comes, I can more easily determine the exact
location of the 1976 trench. As I said, it now appears to be lO0
feet from the well although the inspection report shows 95 feet.
If the well remains where it is, we should probably have a waiver
of 'the 100 foot distance requirement between the well and
drainfield. It's possible that the drainfield is arched which I
can't tell from the surface. I have included the well log for
your information but you may want to wait until June to see which
improvement is undertaken.
