Two test holes were excavated and percolation test performed. The approximate location of the test holes are located on the attached site plan. At the time of excavation no water was encountered and after seven day water monitoring water was found at 6.5 feet and 8 feet for test hole #1 and #2 respectively. This property has enough area for a future septic upgrade which can be seen on the attached site plan. This property is served by a Community Water System. There are no protective well radii which encroach upon the property. We do not anticipate any adverse effects on neighboring properties by the installation of the proposed septic system. T.E.P.) system and a pressurized absorption bed to serve the three bedroom residence located on the referenced property. Construction shaft be in accordance with the approved site plan and design drawings; Municipal permit with any special provisions or conditions; and afl applicable State and Municipal Wastewater Disposal Regulations. The contractor shaft be responsible for obtaining any necessary underground utility locates. Unless specifically agreed otherwise, the property owner shaft be responsible for final grading areas subsequently depressed from soft settling. On aft leachfield mound systems, the property owner shaft be responsible for ensuring a satisfactory vegetation growth over the mounded area. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. SEPTIC TANK INSTALLATION: A septic tank is to be constructed by a certified septic tank manufacturer. Construction shaft include two 4" cleanouts for pumping access. The septic tank shaft be sufficiently bedded to prevent settling or shifting of the tank. Aft standpipes on the septic tank shaft extend a minimum of 12 inches above final grade. Page two Lampert Estates Subdivision, Block 2, Lot 18 December 1, 1993 4. Septic tanks installed with less than 4 ft. of cover shall be insulated. A foundation cleanout shaft be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shaft be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shaft be located on undisturbed soft not more than 10 ft. from the tank. The first cleanout, in line, shaft be to clean toward the leachfield. The second cleanout shaft be to clean toward the septic tank. Final grading over the septic tank shaft be such that a positive slope exists away from the septic tank. PRE$$UFIIZED MOUND SYSTEM INSTALLATION: 1. Any peat or organic matter must be removed from the elevated mound site. The bottom of the basal bed area as weft as the top of the sand filter is to be within two inches of level. 3. The distribution piping is to be of PVC (ASTM D3034 or equal). Aft joints are to be solvent cemented. The side slopes of the top layer of the mound system must not be steeper than 33% ¢:1). The top of the mound shaft be covered with a minimum of 6 inches of topsoft and vegetated sufficiently to prevent erosion. The distribution pipes are to be embedded in sewer rock. Care should be taken to backfill in such a way as to prevent damage to the piping system. Silt barrier material must be installed between the final grave/layer and the native soft backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. Page three Lampert Estates Subdivision, Block 2, Lot 18 December 1, 1993 MINIMUM MA TERIAL SPECIFICATIONS: Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or equal). A permeable nontoxic silt barrier (Typar 3401, M/raft 140N, or equal) must be installed between the final leachfteld gravel layer and the native soil backfill. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the #200 sieve. When sand is being used as a filter material, it's gradation specifications must conform to current M.O.A. or D.E.C. requirements. Page four Lampert Estates Subdivision, Block 2, Lot 18 December 1, 1993 INSPECTIONS: Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. o The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. 3. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. We had first discussed my test report and the COSA application on Monday afternoon (October 25), at which time you expressed your concem that one of the two reported liquid level depths in. the seepage bed appeared to be within the distribution piping, prior to adding any water for the test. It is important to note tim while measurements for liquid levels fi:om two (2) Monitor Tubes (IvlTs) were provided, on the COSA application, measurements were obtained from all four (4) MTs and only the one (I) appeared to have a liquid depth that would be within the distribution Piping. 'l~ae two (2) measurements not provided on the application were 5" and 0". In summary, the four (4) measurements from the MTs in each. comer of the seepage bed were 8", 4", 5" and 0" (dry), prior to us adding any water for the test. These :inconsistencies are obviously troubling and may be indicative that thc MTs were not installed properly. To .help evaluate the problem, we re-visited 'the site yesterday with the necessary eqttipment to obtain precise elevation data on the water surface in the MTs and the bottom of the four MTs. These data showed, as it should 'be, that the water surthce in the bed was level. The bottom, elevation.s in the MTs varied considerably however, most dramatic'~ly with that MT that had the greatest depth of liquid (8") prior to our earlier testing, we did find yesterday that the liquid depths were somewhat reduced from our earlier measurements, finding them at 5.5", 1 ~5", 5.0" and 0" (dry). None of these measurements would dictate a failure by' MOA criteria, prior to adding flow :for the test. It' should also be noted that during our earlier test on the system, a total of 480 gallons was completely absorbed 60 minutes ateter stopping the test :flow. It would appeox that the MTs were not properly installed, as the3, should be with the bottom of the MTs at the same elevation as the bottom of the seepage bed. It is readily apparent that the bottoms are at different elevations, impacting the interpretation of test data and therefore the evaluation of the system's ly,.~ormance. I had initially thought that we could go in with a backhoe, excavate into the two (2) corners of the bed with the most problematic MTs and reset those at the proper elevation. However, in viewing the bed... I feel that the harm in digging into the bed would be greater than the benefit derived. In my opinion, the system performed satisfac4~orily during the teSt and should not be corrsidered to be in a failed condition. ~ere would not be any public health or envkonmental problerms associated with the continued operation of this system. While it is apparent that there are problems with the original installation of the MTs, correction of this problem by excavating into the ~epage bed and resetting the MTs, would be more harmful than hellpful and would have no ~ng on 'the actual, performance of the system. It is my recommendation that you :issue the COSA based on the em-lief test on the system and the data obtained from our inspection work yesterday.
